말초신경 손상 후에 나타나는 신경병성 통증은 자발성 연소통을 포함하여, 통각과민 (hyperalgesia), 이질통(allodynia) 등의 증상을 보인다. 신경병성 통증중 특히 교감신경계와 연관되어있는 경우를 교감신경성 유지통증(sympatetically maintained pain, 이후 SMP

로 기술)으로 분류한다. SMP는 교감신경계의 활성화에 의해 통증이 더욱 악화되는 특징을 보이고, 임상에서는 교감신경차단법이 치료목적으로 자주 이용되고 있다. 현재 교감신경계가 어떻게 통증을 악화시키는지에 대한 자세한 기전은 알려져 있지 않다. 서로 다른 동

물모델에서 밝혀진 바로는 교감신경계의 활성화에 의해 감각신경이 활성화되고, 이 과정은 α 수용체를 매개로 한다는 사실은 확립되어 있다. 그러나 교감-감각신경간의 coupling 과정에서 α 수용체의 매개가 어떻게 이루어지는지에 대한 구체적인 기전은 의견이 분

분하다. 이런 이견은 대부분의 실험이 in vivo 상태의 실험으로 신경의 network이 존재하는 상태에서 여러 인자를 배제하지 못한 것이 그 원인일 것으로 생각된다. 따라서 본 연구에서는 손상받은 배근신경절 세포를 단일세포로 분리하여, 전기생리학적 특성 중 norep

inephrine에 의한 흥분성 증가가 어떤 기전에 의하는지를 밝히고자 흥분성 증가에 관여할 수 있는 몇 가지 ionic channel에 대한 norepinephrine의 영향을 직접 조사하여 그 기전을 밝히고자 하였다. 실험동물로는 쥐를 사용하였고 제 4, 5, 6 요척수 신경 중 제 5,

6 신경을 결찰함으로써 좌골신경에 부분적인 손상을 준 SMP 모델쥐를 제작하고 배근신경절 세포를 분리하여 전기생리학적 특성을 조사하였다.

신경병성 통증모델쥐에서 손상받은 배근신경절 세포의 전기생리학적 특성을 조사한 결과 norepinephrine에 의해 흥분성이 증가하였고 내향성 Ca²^+ 전류와 외향성 K^+ 전류가 감소하였다. 이 변화는 α2 antagonist인 yohimbine을 전 처치한 결과 norepinephrine의 내향성 Ca²^+ 전류와 외향성 K^+ 전류에 대한 효과가 억제되어 이 과정에 α2 adrenergic receptor가 관여함을 알 수 있었다. 한편 Ca²^+ 통로 억제제인 cadmium 전 처치에 의해서는 K^+ 전류에 대한 norepinephrine의 효과가 억제되었으며 Ca²^+ 의존성 K^+ 전류 억제제인 iberiotoxin을 직접 투여해본 결과 K^+ 전류에 대한 norepinephrine의 효과가 억제됨도 확인할 수 있었다.

이상의 결과 신경병성 모델쥐에서의 손상된 배근신경절세포는 norepinephrine에 의해 흥분성이 증가하였으며 그 기전으로 Ca²^+ 의존성 K^+ 전류의 억제가 중요한 역할을 할 것으로 사료된다.

The neuropathic pain which induced by peripheral nerve injury shows spontaneous pain, hyperalgesia and allodynia. If the neuropathic pain is related to the sympathetic system, it is classified as a sympathetically maintained pain (SMP). The SMP is aggravated by sympathetic activation, and sympathetic block is used as a clinical treatment. Until now, it is unclear that what mechanism is involved in aggravation of pain by sympathetic activation. Many investigations about this topics using experimental animal models agrees that sensory nerve is activated by sympathetic activation and it is mediated by alpha adrenergic receptor. However, the detailed mechanism for the involvement of alpha adrenergic receptor on sympathetic-sensory coupling is controversial because previous many experiments were done using in vivo preparation which could not exclude many complicating

factors. Thus we isolate injured dorsal root ganglion cells and investigate the effect of norepinephrine on several ionic channels which is known to be involved in membrane excitability.

The neuropathic animal models were made by ligation of the L5, L6 spinal nerves of rat. The dorsal root ganglion cells were acutely isolated and then electrophysiologic properties were studied.

In current clamp mode, we confirmed the membrane excitability was increased by norepinephrine and in voltage clamp mode, inward Ca²^+ current and outward K^+ current were decreased by norepinephrine in DRG neuron of neuropathic model rat.

Yohimbine, an α2 antagonist suppressed the inhibitory effect of norepinephrine on inward Ca²^+ and outward K^+ current of neuropathic DRG neuron. Cadmium, a calcium channel blocker, suppressed the inhibitory effect of norepinephrine on the outward

K^+ current and iberiotoxin, a calcium activated potassium channel blocker, suppressed the inhibitory effect of norepinephrine on outward K^+ current of neuropathic DRG neuron.

These results suggests that at least direct actions of norepinephrine on DRG neuron contribute to sympathetic-sensory coupling and supression of K_Ca channel activity may be a important mechanism in this process.
