췌장암은 조기 진단이 어렵고, 따라서 수술 시 절제율도 낮고 근치적 절제술 후에도 예후가 매우 불량한 것으로 알려져 있다. 췌장암에 있어 분자생물학적 접근은 췌장암의 발생이나 병인을 이해하는 것뿐만 아니라 조기진단이나 새로운 치료법 개발의 가능성을 제시할 수 있다는 점에서 더 흥미롭다 하겠다. 본 연구는 췌장암에서 면역조직화학염색법을 통해 p16 단백 발현의 소실 및 p53 단백의 과발현 정도를 알아보고 이에 따른 임상병리학적 특징 및 생존율과의 관계를 조사하여 p16 유전자와 p53 유전자의 임상적 의의 및 상호 연관성에 대해 알아보고자 하였다. 1990년 3월부터 1999년 4월까지 연세대학교 의과대학 외과학교실에서 췌장암으로 진단받고 수술받은 환자를 대상으로 하였다. 이 중 근치적 절제술이 시행되고 병리조직학적으로 췌장선암(pancreatic ductal adenocarcinoma)으로

확진된 환자를 대상으로 파라핀 포매조직의 이용이 가능하고 추적관찰이 가능했던 44명을 대상으로 p16 단백과 p53 단백에 대한 면역조직화학염색을 시행하여 그 결과를 대상 환자들의 임상병리학적 특성 및 생존율과 비교하여 분석하였다. 44예 중 20예(45.5%)에서 p

16단백의 발현이 소실되었고, 14예(31.8%)에서 p53단백의 과발현을 보였다. p16단백 발현이 소실된 군에서 림프절 전이가 의의있게 많았고(p=0.040), 병기도 높았다(p=0.015). 나머지 임상병리학적 특성들 및 생존율과는 의미있는 연관성을 찾을 수 없었다. p53단백의

과발현은 조직학적 분화도가 나쁠수록 증가하여 의미있는 연관성을 보였고(p=0.038), p53단백의 과발현을 보인 군에서 무병생존율이 의의있게 낮은 것을 관찰할 수 있었다(p=0.0209). 나머지 임상병리학적 특성들과의 연관성은 찾을 수 없었다. 이상에서와 같이, p16단백 발현의 소실을 보인 군은 림프절 전이가 많고 병기가 높았으나 생존율에는 차이가 없었고, p53단백의 과발현을 보인 군에서는 조직학적 분화도가 나쁘고 무병생존율도 낮았다. 따라서 p16 유전자와 p53 유전자는 췌장암의 진행과정에서 중요한 역할을 한다고 할 수 있겠다.





[영문]

Pancreatic cancer has a very poor prognosis, mainly due to low resection rate and late diagnosis. A better understanding and the pathogenesis of pancreatic cancer and more effective screening technique are required to increase respectability and to improve survival rate. Knowledge about genetic and molecular alteration may suggest new methods to diagnose, detect and treat pancreatic cancer. Forty-four formalin-fixed, paraffin-embedded specimens of pancreatic ductal adenocarcinoma were investigated immunohistochemically with monoclonal anti-p16 antibodies and

monoclonal anti-p53 antibodies. Loss of p16 expression was noted in 20 cases (45.5%) and aberrant p53 protein expression was detected in 14 cases (31.8%). Loss of p16 expression was significantly correlated with lymph node metastasis (Without versus With, p= 0.040) and clinical stage (stage I, II versus III, IV, p=0.015).

There was no significant correlation between p16 expression and survival time. The p16 negative group showed higher incidence of lymph node metastasis (p=0.040) and more advanced stage (p=0.015) than the p16 positive groups. Aberrant p53 protein expression was correlated with histologic grade (p=0.038). In aberrant p53 protein positive group, disease-free survival rate was significantly lower than aberrant p53 protein negative group (p=0.029). We cannot regard p53 as a prognostic factor of pancreatic carcinoma, but according to the above results, p16 and p53 genes have important roles in the progression of pancreatic ductal adenocarcinoma.
