미주신경 절단 후 위장관 근충간신경총의 조직계측학적 연구

생후 일주일 된 흰쥐 (Sprague-Dawley strain)를 ether마취하에 식도의 원위부에서 미주신경을 절단하고 30일 및 60일후에 위저부위의 유문부, 공장의 근위부, 회장의 원위부및 맹장의 근층간 신경총에서 신경세포의 수 및 크기의 변화를 계측하였다.

조직의 염색은 NADH-tetrazolium reductase activity를 검출하는 조직화학적 방법을 이용하였다.

계측은 현미경 그리기 장치를 이용하여 1000배 하에 YAP 1.0 및 1.1 컴퓨터 프로그램을 이용하여 위장관의 각 부위에서 각각 200 내지 300세포를 직접 계측하고 통계처리하여다음과 같은 결과를 얻었다.

1. 소장과 맹장에서 신경세포는 윤상근층의 근섬유와 나란히 줄을 이루며 배열되어 있었고 위에서는 세포의 군을 이루고 있었다. 이러한 배열상태는 미주신경 절단으로 영향을 받지 않았다.

2. 신경세포는 소장의 장간막 부착부위의 반대쪽보다 장간막 부착부위에 더 많이 분포되어 있었다.

3. 미주신경 절단 60일후에 공장과 맹장에서 신경세포의 수가 감소되었다.

4. 근층간신경총의 신경세포는 대조군에 비해 미주신경절단 후 크기가 감소되는 경향을 보였고 제 3형에 비해 제 1형과 제 2형의 변화가 더 현저하였다.

[영문]

Bilateral truncal vagotomy was performed on newborn rats at the distal esophagus. In all experimenths each rat was anesthetized with ether.

At thirtieth and sixtieth day after vagotomy, rats in control and experimental group were killed with ether. The abdomen was opened and saline(at room temperature) was gently injected into the lumen of the stomach, small intestien, and ascending colon, distending the muscle coat for the better staining of myenteric plexus. After applying ligatures to maintain the distension, the

specimens were obtained from funds, pylorus, proximal jejunum, distal ileum, and cecum. The specimens were then frozen in liquid nitrogen, and, after slow thawing, were stained by a histochemical technique to detect NADH-tetrazolium reductase activity.

Under the dissecting microscope, the mucosa and submucosa were seperated from the muscle coat and the latter was mounted in glycerine jelly for light microscopic examination.

Direct measurements of about 200∼300 neurons from the myenteric plexus of each specimen were carried out by the use of microscopic drawing apparatus and computer program, YAP. 1.0 and 1.1, under the microscope of 1,000 magnification.

The results were summerized as follows:

1. The normal arrangements of neurons of myenteric plexus in gut were not changed 30 and 60 days after vagotomy.

2. Neurons were more numerous in the mesenteric zone of small bowels than at the anti-mesenteric zone of the gut.

3. The number of neurons in the proximal jejunum and cecum were decreased 60 days after vagotomy than control group.

4. The neurons in myenteric plexus were classified as type 1, type 2, and type 3 according to the size and shape of cell body. All the types of neurons in myenteric plexus had a tendency to decrease in size after vagotomy. This tendency was more remarkable in type 1 and type 2 than in type 3.
