장루전문간호 업무표준 개발

Abstract

[한글]새로운 의료 환경에서 간호의 질 향상 활동과 간호의 전문화는 필수적이며 중요한 과제로 인식되고 있어 장루전문간호사 모임에서는 이러한 시대적 흐름에 많은 계획과 전략을 준비하고 있다. 장루, 상처 및 실금 간호를 필요로 하는 대상자에게 완전�고 질 높은 간호를 제공하기 위한 전문간호 제도가 정착되기 위해서 지속적인 질 향상, 장루전문간호(WOCN, Wound Ostomy Continence Nurse) 업무의 성과 및 전문간호의 필요성을 입증하여야 한다. 이를 위해서는 체계적이며 과학적인 장루전문간호 업무표준의 개발이 선행되어 간호의 질 향상과 장루전문간호사 제도의 확립이 이루어져야 한다.

본 연구의 목적은 실제로 간호업무에 적용 가능하며, 간호의 질 평가 도구로 사용할 수 있는 장루전문간호 업무표준을 개발하기 위함으로 문헌고찰, 연구자의 장루전문 간호업무의 경험에 의하여 표준, 기준 및 지표로 구성된 예비업무표준을 개발하였다. 개발된 예비업무표준은 공인된 장루전문간호사 4인, 대학병원에서 장루전문간호업무를 3년 이상 수행해 온 장루전문간호사 5인 등 총 9명으로 구성된 전문가 집단으로부터 타당도 검증을 받았다. 내용 타당도가 검증된 장루전문간호 업무표준은 실제 임상에서 업무를 수행하는데 적합한지를 확인하고, 업무를 수행하지 못하는 사유를 분석하여 우리나라에서 장루전문간호사로 활동하고 있는 8개 병원의 장루전문간호사 전수인 15명에게 장루전문간호 업무표준의 수행여부를 조사한 후 최종적으로 장루전문간호 업무표준을 개발하였으며 결과는 다음과 같다.

1. 개발된 장루전문간호 업무표준은 간호업무표준과 전문직 성과표준으로 구성되었다. 간호업무표준에는 자료수집, 간호진단, 간호계획, 수행, 평가 등 5개의 표준이, 전문직 성과표준에는 간호의 질, 교육, 자문, 전문직 연구, 윤리 및 자원의 이용 등 6개의 표준이 포함되어 있어 장루전문간호 업무표준은 총 11개의 표준과 62개의 기준 그리고 91개의 지표로 구성되었다. 개발된 장루전문간호 업무표준은 장루전문간호의 업무 지침서, 자가평가서 및 감사의 도구로 사용 가능하다.

2. 전문가 타당도 검증은 각 문항에 대하여 내용 타당도를 4점 척도를 사용하여 3점은 '타당하다', 4점은 '매우 타당하다'에 동의한 비율로 CVI 를 구하였다. 전체 항목의 CVI는 0.96으로 높게 나타났다.

3. 개발된 장루전문간호 업무표준을 이용하여 장루전문간호사의 업무표준 수행율을 조사한 결과, 전체 업무표준 수행율은 70%이며, 각 표준의 업무 수행율은 평가 81.7%, 윤리 77.0%, 자문 76.0%, 교육 74.3%, 자료수집 73.3%, 자원의 이용 68.3%, 간호계획 66.0%, 간호의 질 62.7%, 간호진단 56.7% 및 연구 56.7%으로 높게 조사되었다.

위와 같이 완성된 장루전문간호 업무표준은 앞으로 간호업무에 적용하여 수정, 보완 후 간호의 질 향상을 평가하는 도구로서 활용될 것이며, 우리나라에서 장루전문간호사 제도 정착을 위한 정책 및 규정 등을 위한 지침서로 활용될 수 있을 것이다.

[영문]The quality improvement and specialization of nursing have been recognized as important factors in recent health care system. In order to establish Wound Ostomy Continence Nurse(WOCN) system to improve the quality of nursing care, development of the practice standards for WOCN is essential.

The purpose of this study was to develop the practice standards for WOCN that could be applicable to the clinical settings and to measur the quality of nursing care. A preliminary set of standards, criteria, and indicators were first developed based on the review of the relevant literature, the researcher's own experience, and experts' review. The panel of 4 Certified Enterostomal Therapist Nurses(CETN) and 5 Enterostomal Therapists(ET) with more than 3 years experience examined the content validity of the preliminary set. After 15 ETs were selected to examine their performance by using it, the final version of the practice standards for WOCN was developed.

The findings of this study are as follows:

1. The practice standards for WOCN including the standards of nursing care and the standards of professional performance were composed of 11 standards, 62 criteria, and 91 indicators.

2. The index of Content Validity was utilized to test the content validity of the practice standards for WOCN. It was high as .96.

3. The practice performance rate of the total standards was 70%. In terms of the practice performance rate of the each standard, evaluation was noted the highest rate of 81.7%, intervention 78.7%, ethics 77.0%, consultation 76.0%, educatio 74.3%, data collection 73.3%, resource utilization 68.3%, planning 66.0%, quality of nursing care 62.7%, nursing diagnosis 56.7% in the order of high rate.

In conclusion, the developed practice standards for WOCN could be useful in the clinical settings and utilized to evaluate the quality improvement of nursing care. Also the practice standards for WOCN are expected to establish the policy and rules of WOCN system in Korea.
