복부수술후 혈장 아미노산의 변화

수술후 나타나는 이화현상은 negative nitrogen balance, 체내 단백질의 분해 및 체중의 감소를 초래하게 되는데 이러한 변화로 수술후 혈장 아미노산의 변화도 초래된다.

복부수술을 받은 15명의 환자를 대상으로 하여 수술전, 수술후 4일 및 수술후 10일에 혈장 아미노산을 측정하였으며 혈청 알부민 및 체중변화를 측정하여 그 변화를 비교관찰하였다.

1. Alanine은 수술전에 비하여 수술후 4일 및 수술후 10일에 감소하고 있었다.

2. 수술후 10일에 있어 필수아미노산 및 glucogenic 아미노산들이 수술전에 비하여 의미있게 증가하고 있었다.

3. 체중의 감소는 수술후 4일 및 수술후 10일에 의미있게 감소하고 있었으나 혈청 알부민은 수술후 4일에는 감소하였으나 수술후 10일에는 증가하는 경향을 보이고 있었다.

Negative nitrogen balance, break-down of body protein and weight loss result from the postoperative catabolism in surgical patients. In addition, concentrations of plasma amino acids change in unique patterns postoperatively.

In 15 patients undergoing abdominal operations, concentrations of plasma amino acids, serum albumin and body weight are measured preoperatively, at postoperative day 4 and day 10,respectively.

The results are as follows :

1. The concentration of alanine decreased significantly at posto perative day 4 and day 10.

2. The concentrations of essential amino acids and glucogenic amino acids increased significantly at postoperative day 10

3. Body weight reduced significantly at postoperative day 4 and day 10. The level of serum albumin decreased at post operative day 4. but in comparison with the level of serum a1bumin at postoperative day 4, the level of sect albumin increased

at postoperative day 10.
