인체 난소 표피 및 기질세포에 관한 형태학적 연구

인체의 난소는 피질과 수질로 나뉘어지며 피질의 외표면에는 한층의 상피세포가 덮혀있다. 상이세포의 모양은 젊은 연령에서는 입방형이지만 연령이 증가하면 부분적으로 편평해진다. 피질의 결체조직 부분을 난소기질이라 하며 섬유아세포와 유사한 방추형의 세포들과 세포간물질로 구성되어 있다(Ham과 Cormack, 1979).

난소의 표면상피세로는 출생 후 잠재성을 잃고 변화하지 않는 것으로 알려져 있으며, 기질세포는 여러 종류의 세포 즉 지방세포, 평활근세포, 또는 탈락막세포 등으로 가역적 변화를 일으킬 수 있는 잠재성을 갖고 있다(Ham과 Cormack, 1979; Scully, 1979). 생식세포주위의 난포세포(과립막세포와 내로막세포)는 기질세포에서 분화된 것으로 난소주기에 따른 형태학적, 내분비학적 변화를 일으키며 난포호르몬과 황체호르몬을 분비한다는 것은 잘 알려져 있는 사실이나(Botticelli등, 1963; Richardson, 1966) 기질세포의 내분비학적 측면에 대해서는 전혀 밟혀져있지 않다. 포유류 및 인체의 난소의 외포막과 기질에서 비전형 또는 전형적 평활근세포가 관찰되었으나(Rocereto등, 1969; Osvaldo-Decima, 1970; 0'shea, 1970), 이 평활근세포의 본성 및 기능에 대하여는 확실하게 밟혀져 있지 않다(Okamura등, 1972).

따라서 인체 난소의 표면상피세포와 기질세포의 난소주기 및 연령에 따른 변화를 관찰하는 것은 난소의 형태학적 특징과 내분비 기능을 추적하는데 의의있다고 사료되어 본 연구에 착수하였다.

연구재료 및 방법

연구대상으로 난소기능이 정상으로 사료되었던 환자에서 적출된 정상난소 60예를 연령과 난소주기별로 나누어 관찰하였다.

각 난소조직은 두 편으로 나누어 한편은 광학현미경 다른 한편은 투과 및 주사전자현미경 관찰을 시행하였다. 투과전자현미경 검사는 난소의 표면상지세포와 기질세포를 보기 위해 시행하였는데, 표면상피세포는 난소표면의 함몰부위에서, 기질세포는 난소표면으로

부터 약 5mm하방 난포가 관찰되지 않는 피질 부위에서 조직편을 채취하여 관찰하였고, 주사전자현미경 검사는 표면 상지세포를 관찰하기 위하여 실시하였다.

연구성적 및 결론

1. 난소의 표면상지세포는 연령이 증가할수록 세포표면이 편평해지고 세포간 간격이 넓어지며 미세융모의 수와 길이가 감소하는 경향을 보였으며 세포의 핵이 불규칙해지고 염색질 응집이 현저하였다.

2. 기질세포는 황체기에서 핵의 농축, 핵주위의 공포형성, 세포질내의 지방적의 증가, myelin figure와 dense bodies의 출현이 관찰됨으로서 난포기와 상이한 소견을 보였다.

3. 연령이 증가할수록 평활근세포의 수가 증가됨을 볼 수 있었는데 이는 특히 폐경전후기에 현저하였다.

이상의 소견을 종합하여 보면 표면상피세포는 연령이 증가함에 따라 세포표면의 편평화와 세포간 간격의 확장 및 미세융모의 감소를 보였으나 난소주기에 따른 변화는 없었으며, 난소기질세포는 연령보다 난소주기에 따른 변화를 보여서 난포기에는 증식하는 양상을, 황체기에는 위측하는 양상을 보였고 기질내 평활근세포는 난소주기에 따른 변화는 없었으나 연령이 증가함에 따라 수적 증가를 보였다.

[영문]

The ovaries develop from the gonadal ridges bulging from the surface of the coelomic cavity.

The mesothelial cells on the surface of the developing ovary differentiate into a layer of epithelial cells covering the organs. The surface epithelium subsequently loses potentiality after birth, but an alternative source of the cells that

surround the germ cello could be stromal cells which, at that time could be expected to possess a great deal of potentiality (Ham and Cormack, 1979).

The stromal cells not only produce reticulum and collagen, which are most abunant in the outer cortex, but they also differentiate into theca cells at the edges of developing follicles luteinized stromal cells (stromal lutein cells; stromal theca cells), and enzymatically active stromal cells (EASC).

The ovarian stromal cells are able to undergo metaplasia into a variety of cell trees, including fat cells, smooth muscle cells, and decidual cells (Scully, 1979).

We have much obvious evidence of morphologic chanties associated with steroidogenesis in follicular cello dependent upon the ovarian cycle but no appearant observations in changes of surface epithelium and the original stromal cells during ovarian cycle and changes of age. This study attempts to find out

whether the human ovarian surface epithelial cells and stromal cells change morphologically and have any evidence of endocrinologic function associated with the ovarian cycle and age changes.

Material and Method

The material used for this study was 60 ovaries obtained from regularly menstruating and post-menopausal women who had oophorectomies in which the indication was not related to ovarian disease.

The age distribution as follows :

The distribution related to ovarian cycle as follows :

The specimens were divided into two pieces for the light microscope and electron micro scope (scanning and transmission electron microscope) stuffy and were fixed in 10% neutral formalin solution and 3% glutaraldehyde (pH 7.4).

The light microscopic examinations were performed with hematoxylin-eosin and Masson's trichrome stains after preparation in the usual manner.

The scanning electron microscopic observations were done with the Hitachi model S-450 scanning electron microscope after preparation or the critical point drier for the surface epithelial cells. The transmission electron microscopic examinations were done with the Hitachi model H-500 transmission electron microscope after routine preparations for the studs of surface epithelium and stromal cells.

Results and Summary

1. The ovarian surface epithelium has the tendency of flattening of cell surface, widening of intercellular apace and decrease of the number and the length of microvilli in age process.

2. The stromal cell showed nuclear clumping, perinuclear vacuolation, increased lipid droplets in cytoplasm and the appearance of myelin figures and dense bogies in the luteal phase rather than in the follicular phase.

3. The number of smooth muscle cells of the ovarian stroma was increased in the age progress, especially in the postmenopause.

In summary the human ovarian surface epithelium had the tendency of flattening of the cell surface, widening of the intercellular space and a decrease of the number and the length of microvilli in the aging process but no evidence of cyclic change.

The cortical stromal cells showed the appearant evidence of cyclic change : proliferative appearance in the follicular phase.

The stroml smooth muscle cells with ultrastructural characteristics were increased in number progressively in the advancement of age but had no remarkable changes in the ovarian cycle.
