전산화 단층촬영을 이용한 정상성인의 안와크기 및 안구돌출에 관한 연구

안와의 형태는 기저부가 사각청인 원추형태의 서양배 모양이며, 안와의 가장 앞쫏인 안화연의 형태는 연령, 인종 그리고 개개인에 따라 많은 차이를 보이는 것으로 보고되고있다. Broca(1895)는 안와연의 형태를 안와의 높이와 폭과의 상관관게인 안와지수(orbital

index)로 표시하며 알아보았고, Cameron(1920)은 이 안와지수를 이용하여 인종간에 나타나는 안와형태의 차이를 비교적 둥근형인 Megaseme, 직사각형인 Microseme과 그 중간형인 Mesoseme의 3가지 형태로 분류하였다.

안와의 크기를 측정하는 방법으로는 단순 X-ray 촬영과 전산화단층촬영(computed tomography, CT)을 이용하는 방법이 있다. 이 중 단순 X-ray 촬영은 두개골의 여러구조가 중복되어 나타나기 때문에 측정하는 기준점의 식별이 어려우며 영상이 확대되어 보이는 단점이 있는 반면, CT는 안와의 골구조와 주의 연조직의 구조에 대한 선명한 영상을 얻을 수 있고 축소비율을 정확히 알수 있기 때문에 안와의 크기 및 구조에 대해 비교적 정확한 정보를 얻을 수 있는 장점이 있다.

저자는 이러한 CT를 이용한 우리나리 사람의 안와크기와 안구돌출정도의 측정은 안와질환이나 기형의 진단뿐 아니라 수술시 중요한 지표가 될 것으로 생각되어 1983년 1월 1988년 7월 까지 연세대학교 부속병원에서 시행한 안와 CT 사진중 안와 및 안구에 병변이 없는 20세 이상의 남자 68명, 여자 35명 총 103명 CT 사진을 대상으로 안와크기를 측정하여 다음과 같은 결과를 얻었다.

1. 안와의 폭

전 대상군의 평균치는 38.09±2.82 mm 였으며, 이중 남자의 평균치는 38.59±2.75 mm, 여자의 평균치는 37.11±2.73 mm이었다. 남·녀간의 외안와의 폭은 유의한 차이가 있었다. (P=0.01)

2. 안와의 높이

전 대상군의 평균치는 35.47±2.28 mm였으며, 이중 남자의 평균치는 35.75±2.14mm, 여자의 평균치는 34.95±2.48 mm이였다.

3. 안와지수

전 대상군의 평균치는 93.80±6.70 이였으며, 이중 남자의 평균치는 93.00±6.35, 여자의 평균치는 95.37±7.15이었다.

4. 안와의 깊이

전 대상군의 평균치는 42.67±3.56mm 였으며, 이중 남자의 평균치는 42.60±3.54mm, 여자의 평균치는 42.80±3.66mm이었다.

5. 내안와간 거리

전 대상군의 평균치는 24.97±2.25mm 였으며, 이중 남자의 평균치는 25.18±2.35mm, 여자의 평균치는 24.56±2.02mm이였다.

6. 외안와간 거리

전 대상군의 평균치는 97.38±5.14mm 였으며, 이중 남자의 평균치는 98.20±5.16mm, 여자의 평균치는 95.79±4.80mm 이었다. 남·녀간의 외안와간 거리는 유의한 차이가 있었다. (p=.0.02)

7. 안구돌출정도

전 대상군의 평균치는 14.28±3.49mm 였으며, 이중 남자의 평균치는 14.57±3.60mm, 여자의 평균치는 13.73±3.24mm 이었다.

[영문]

The orbit has roughly 7he shape of a quadrilateral pyramid whose base, directed forwards, lateralIy and slightIy do주wards, corresponds to the orbital margin, and whose apex is the optic foramen or, as some hold, the medial end of the superior orbital fissure: or the bar of bone between these two apertures.

Broca(1875) gave the relationship (between the orbital height and width) a mathematical value by expressing the proportion as the orbital index. According to the value of tis index, Cameron(1920) classified the orbit into 3 classes (Megaseme, Mesoseme and Microaeme).

The orbital dimensions can be measured by plain X-ray or computed tomogrphy. Plain X-ray of the orbit may be difficult for the nonradiologist to identify the measurement landmarks and interpret because of the superimposition of multiple shadows cast by overlapping bony structures, but whereas computed tomography allows more precise analysis by imaging soft tissue relationships to surrounding orbital bones. Measurement of the orbital dimensions and proptosis using CT does not only contribute to the diagnosis of specific pathologic processes, but also guide the

ophthalmologist in planning appropriate medical and surgical management.

One hundred and three orbital CT films of Korean adults taken by General Electric 9800 and Philips Tomoscan 310 from January, 1983 to July, 1888 were measured.

The results were as follows ;

1. The average orbital width was 38.09±2.82 mm in the whole group, 38.59±2.75mm in men and 37.l1±2.73 in women. There was a statistically significant difference between men and women (p=0.01).

2. The average orbital height was 35.47±2.28mm in the whole group, 35.75±2.14mm in men and 34.95±2.48mm in women.

3. The average orbital index was 93.80±6.70mm in the whole group, 93.00±6.35mm in men and 95.37±7.15 in women.

4. The average orbital depth was 42.67±3.56mm in the whole group, 42.60±3.54mm in men and 42.80±3.66mm in women.

5. The average interorbital distance was 24.97±2.25mm in the whole group, 25.18±2.35m in men and 24.56±2.03mm in women.

6. The average extraorbital distance was 97.38±5.14mm in the whole group, 98.20±5.16mm in men and 95.79±5.14mm in women. There was a statitically significant difference between men and women(p=0.02).

7. The average proptosis was 14.28±3.49mm in the whole group, 14.57±3.60mm in men nd 13.73±3.24mm in women.
