HBs 항원과 항체 검출을 위한 국산 radioimmunoassay 시약의 평가

dc.description.abstract[한글] 동위원소 면역법에 의한 HBsgkddnjs 및 항체검출 시약으로 국내에서 현재 많이 쓰이고 있는 외국시약(Abbott, 미국)과 국산제품으로 개발중에 있는 녹십자(KGCC)시약을 비교하여 보았다. 1988년 10월, 11월에 걸쳐 임상검체를 재료로 HBsAg 및 anti-HBs검사를 시행하여 다음과 같은 결과를 얻었다. HBsAg 양성률은 KGCC시약으로 44.0 %, Abbott시약으로 45.4 %이었고, anti-HBs 양성률은 각각 69.9 %, 70.9 %으로 검출율의 차이가 없었다. 두 시약으로 검사한 결과가 일치하지 않는 검체를 제 3의 시약(Sorin, Italy)으로 검사하였을 때 두 시약간에 특이도 및 예민도 의 차이는 없었다. 또한 중화시험(neutralization)으로 확인검사를 실시하였는데 항원의 경우 KGCC시약이 Abbott시약에 비해 위양성이 높은 것으로 나타났으나 항체의 경우는 오히려 위양성이 낮은 것으로 나타났다, detection li mit와 cut-off value의 안정성은 KGCC시약이 Abbott시약보다 낮았다. 그러나 KGCC시약은 Abbott시약에 비해 예민도 및 특이도면에서 차이가 없으므로 임상적 사용이 가능할 것으로 사료되며 앞으로 그 검출 능력과 안정성 및 사용상의 편의성만 개선되면 크게 활용될 것으로 보인다. [영문] To evaluate a domestic RIA reagent(KGCC) for HBs antigen and antibody detection, it was compared to an imported reagent(Abbott, USA) which has been widely used in Korea. A total of 1556 samples were collected from in and outpatients of Yonsei University Medical Center(Seoul, Korea) during October and November 1988. Accordant results were 96.4 % and 90.8 % for antigen and antibody respectively. When the results of bath reagents were discordant, the third reasent(Sorin, Italy) was used, and if one of the former reagents showed the same result with the third reagent, the result was considered as a true result. In this aspect, there ere no significant differences in sensitivity and specificity between KGCC and Abbott. With a confirmatory test by neutralization, KGCC reagent for HBs antigen showed a higher false positive rate than Abbott reagent, but KGCC reagent for HBs antibody showed a lower false positive rate than Abbott reagent, KGCC reagent showed less satisfactory results than Abbott reagent in detection limit and stability of cut-off values in both HBs antigen and antibody tests. In conclusion, because there were no significant difference between the two reagents in sensitivity and specificity, the domestic product was considered as good as the imported product for clinical use, and the use of domestic product will be greatly promoted with improvement of the stability of the reagent and the simplicity of the method.-
