뚜레장애환자에서의 지능검사 양상과 주의력결핍 및 과잉운동의 관련성

뚜레장애환자에 있어 대뇌인지기능 상태와, 동반된 주의력결핍 및 과잉운동의 연관성을 규명함으로서 뚜레장애와 주의력결핍 과잉운동장애(Attention-deficit Hyperactivity Disorder: 이하 ADHD로 약함)사이의 관계를 밝히기 위해 40명의 뚜레장애환자를 대상으로 하여 KWIS(Korean Wechsler Intelligence Scale)와 과잉운동의 행동적, 사회적 측면을 부모가 평점하는 질문지식 평가척도 CPRS(Conners' Parent Rating Scale), 주의력결핍의 인지적 측면을 객관적으로 측정하는 MFFT(Matching Familiar Figure Test)를 시행하였다.

연구방법은 첫째, KWIS를 시행하여 뚜레장애의 특정한 지능검사 양상이 있는지 알아보았고, 둘째, MFFT를 시행하여 주의력결핍 및 과잉운동의 인지적 측면이 뚜레장애환자에서도 나타나는지 조사하였으며, 셋째, 환자의 부모에게 CPRS를 평점하도록 하여 그 결과와 KWIS-소검사 및 MFFT 결과간에 상관성이 있는 지 분석함으로써 뚜레장애 환자에서의 주의력결핍 및 과잉운동이 ADHD와 동일한 것인가를 밝히고자 했으며, 넷째, 뚜레장애가 ADHD를 동반하는 환자군과 동반하지 않는 환자군으로 이루어진 이질집단인 지를 조사하기 위해 CPRS점수를 기준으로 두 집단으로 나누어서 인구학적 특성 및 KWIS, MFFT 결과를 차이검증하였다.

그 결과는 다음과 같다.

1) 뚜레장애 환자의 KWIS 양상은 보통이상의 지능지수(평균11 8.25)를 갖고 있고, 특히 공통성문제에서 높은 점수를 보였다. (P<.05). 그리고 ADHD의 특징적인 KWIS 양상과 공통되는 소견은 없었다.

2) KWIS-소검사중 주의집중력을 요구하는 산수문제, 바꿔쓰기, 빠진곳 찾기가 평균점수보다 저하되어 있었다.

3) MFFT결과에서 오답수 백분위는 낮고 (평균 36.90), 반응잠재시간 백분위는 높게(평균 77.38) 나타나, 주의력의 인지유형은 숙고형에 가까웠다.

4) CPRS 점수는 평균 12.68로서 높았으나 개인차가 많았다(범위 4점∼25점). CPRS점수와 KWIS-소검사 및 MFFT 결과간에는 유의한 상관성이 없었다.

5) CPRS점수가 18점 이상인 뚜레장애+ADHD환자군이 나머지내부 대조군보다 연령이 낮고, 남자의 비율이 더 높았으며(P<.05), 바꿔쓰기 소검사에서 유의하게 낮은 점수로 측정되었다(P<.05). 그러나 MFFT결과에서는 유의한 차이가 없었다.

이상의 결과로 부터 뚜레장애는 ADHD와는 상이한 지능검사 양상을 보이며, 연관된 주의력결핍 및 과잉운동증상은 ADHD와는 다른 증상임을 알 수 있었다. 그러나 CPRS점수가 매우 높은 일부 뚜레장애 환자군은 ADHD를 함께 가지고 있고 또 다른 신경정신과적상태의 동반에 따른 영향을 받고 있을 것으로 추정되었다.

This study had been conducted to investigate cognitive brain defects and it's relation to attention-deficit and hyperactivity in patients with Tourette disorders by which Tourette disorder -ADHD relationship would be tested. As assessment instruments, two neuropsychological tests and one behavior-rating scale were made.

The subjects were 40 patients with Tourette disorder whose parents complained about attention-deficit and hyperactivity of them. All subjects were administered KWIS and MFFT, and CPRS was made by their parents.

The results were as follows,

1) The KWIS pattern of Tourette disorder was high-average intelligence(mean 118.25) and significantly good performance in Similarity subtest (p<.05). There was no common feature with the characteristic KWIS pattern of ADHD.

2) The scares in Arithmatic, Coding and Picture Completion subtests of KWIS which required attention and concentration, were below the average and MFFT results were closer to a reflactory cognitive style rather than an impulsive style. But there was no statistical significance in these results.

3) All subjects had high mean score (12.68) with wade range (425) in CPRS, but there was no significant correlation between CPRS score and KWIS-subtests results, and MFFT results (p>.05).

4) The patient group with CPRS score above 17 points, designated the (Tourette disorder+ADHD) group, was younger and involved more male patients than the remainder designated the internal control group (p<.05). And the former was poorer in Coding sub-test (p<.05), but not in MFFT results.

IN summeary, these results suggest that Tourette disorder shows peculiar KWIS pattern different from that of ADHD, Tourette disorder associated with severe hyperactivity coexists with ADHD and a part of Tourette disorder shows less severe hyperactivity as a maladaptive behavioral reaction secondarily. And possibility of obsession with reflactory cognitive style associating with Tourette disorder should be tested in future study.
