한국의 의료보험수가에서 간호수가 추정에 관한 연구

dc.description.abstract[한글] 현행 우리나라의 「의료보험 진료수가 기준액표」에는 의사의 "의료" 조산원의 "조산"에 관한 의료기술수가는 명시되어 있으나 불행하게도 전문직 간호원이 시행한 환자 간호행위에 대한 수가의 규정은 되어 있지 않으며 "간호"라는 용어조차 찾아볼 수 없다. '84년말 현재 전인구의 46.5%가 의료보장을 받고 있으나 정부는 복지국가 건설을 국정지표로 하고 있으며 1991년까지는 전국민 보험화정책을 추진하고 있는 상황에서, 이와같이 병원 의료서비스의 주요 생산자의 행위가 포함되어 있지 않은 현행 의료보험수가로 인해 병원 및 의료보험제도 운영에 따른 수가의 문제는 계속 확대될 것으로 본다. 따라서 본 연구는 현행 의료보험수가 중, 전문직 간호원의 간호행위가 차지하는 비중을 밝히고자 시도되었다. 연구대상은 서울 시내 1개 대학 종합병원에서 근무하고 있는 의료인력 중 간호원, 의사, 간호보조원, 용원이었다. 연구도구로는 「의료보험 진료수가 기준액표」(보사부고시 제85-11호)에 나타난 62개 항목으로 구성된 진료행위를 관찰하기 위하여 연구자가 현재 임상의 실정에 맞도록 수정·보완하여 개발한 진료행위 점검표를 사용하였고 연구 보조기구로는 의료인력(간호원, 의사, 간호보조원, 용원)의 진료행위 참여시간을 정확히 측정하기 위해 'Stop Watch'를 사용하였으며 각 진료행위는 ① 물품준비시간, ② 환자준비시간, ③ 시행시간, ④ 사(事)후처리시간, ⑤ 기록시간, ⑥ 추후 환자관찰시간의 6단계로 나누어 측정하였다. 자료수집기간은 1985년 8월 8일부터 1985년 9월 14일까지 공휴일을 포함한 38일간이었다. 수집된 자료는 62개 중 41개 항목으로 구성된 진료행위 각각에 소요된 의료인력(간호원, 의사, 간호보조원, 용원)의 참여시간을 산술평균하였고, 그 중 간호원, 간호보조원, 용원의 인건비를 고려하여 의료보험수가가 곧 의료원가를 나타낸다는 가정하에 의료원가를 산출하는 일정한 절차를 거쳐 간호수가를 추정하였다. 본 연구의 결과를 요약하면 다음과 같다. 1. 「의료보험 진료수가 기준액표」 중 41개 항목으로 구성된 40개의 진료행위에서 간호원의 참여가 각 진료행위당 의료인력의총참여시간의 80% 이상을 차지하는 진료행위는 21개였고, 50%이상 80% 미만은 16개, 30% 이상 50% 미만은 2개였으며, 30% 미만은 오직 1개였다. 이 행위 가운데 의사와 같이 참여하였을 때 15개의 진료행위에서 의사보다 더 많은 시간이 소요되었고, 의사가 간호원보다 더 많은 참여시간을 보인 진료행위는 2개에 불과했으나 간호원이 독자적으로 시행한 진료행위는 21개였으며, 간호보조원 및 용원의 진료행위 참여 양상은 비독자적인 것으로서 7개의 진료행위에서 부분적으로 나타났다. 2. 「의료보험 진료수가 기준액표」 중 41개 항목으로 구성된 40개의 진료행위 각각에 포함된 간호수가(간호원 인건비)를 추정하였고, 간호수가(간호원 인건비)가 각 진료행위당 인건비, 변동비 및 고정비의 50% 이상을 차지하는 진료행위는 18개였고, 30% 이상 50% 미만은 7개였으며, 30% 미만은 15개였다. 그리고 각 진료행위에 소요된 변동비는 12,907원부터 6원까지, 각 진료행위에 소요된 고정비는 14,342원부터 20원까지 다양하게 나타났다. 3. 본 연구에서 추정된 간호수가가 수가고시금액 및 진료보수에서 차지하는 비중은 상당한 것으로 나타났는데, 41개 항목으로 구성된 38개의 진료행위에서 간호수가(간호원 인건비)가 각 진료행위당 진료보수(수가고시금액+진료행위당 기본 진료료)의 101% 이상을 차지하는 진료행위는 7개였고, 50% 이상 100% 이하는 10개, 30% 이상 50% 미만은 6개였으며, 30% 미만은 15개였다. 그리고 본 연구에서 고려한 인건비(간호원, 간호보조원, 용원), 변동비 및 고정비는 진료보수(수가고시금액+진료행위당 기본 진료료)와의 차액을 잔여비용으로 제시하였는데, 이 잔여비용이 '-(Minus)'로 나온 진료행위는 41개 항목으로 구성된 38개의 진료행위중 20개로 -64,994원부터 -16원까지 다양하게 나타났다. 결론적으로 간호원이 진료행위에 참여하는 부분은 다른 의료인력보다 많음을 알 수 있고 「의료보험 진료수가 기준액표」에서 41개 항목으로 구성된 40개 진료행위 중 간호원이 독자적으로 시행한 진료행위는 21개로 나타났다. 따라서 이러한 간호행위에 대한 간호수가 책정은 매우 필요하다는 사실과 본 연구에서 추정된 간호수가는 수가고시금액 및 진료보수에서 차지하는 비중이 상당한 것으로 나타나 현행 우리나라 의료보험의 진료보수 지불방식에서 간호원의 전문직 간호행위에 대한 인식과 함께 적절한 보수체계가 도입되어야 하며 또한 도입이 되더라도 적정한 보상수준이 되어야 현행 의료보험의 확대로 발생할 수 있는 병원 운영의 위기를 미연에 방지할 수 있으며 환자를 위한 적정간호가 제공되어 환자회복 및 국민 건강관리에 기여할 수 있을 것으로 믿는다. [영문] The existing Medical Payment Insurance Rates in Korea stipulate charges for medical treatment by the doctor and maternity nurser but unfortunately neither specify those charges for nursing done by the professional nurse nor contain mention of the term 'nursing' at all. As of the end of 1984, 46.5 percent of the population in Korea are benefitted from the medical insurance system, and the government now promotes in an effort for constructing its welfare state, a policy for benefitting the entire people by medical insurance by 1991. Under the conditions it seems that the problems pertaining to operating the hospital and medical insurance system would be incessantly expanded in that no mention is made of medical charges rendered by the major medical service in the existing sysstem. For that reason, this study makes an attempt to clarify the importance the professional nursing puts on the existing Medical Payments Insurance Rates. The subjects of study include medical activities which are composed of 62 Items in the existing Metrical Payments Insurance Rates(Ministry of Health & Social Affairs Notice No. 85-11). The tool of study depends upon the Medical Activity Check List devised with the intention of being suitable for this case by the researcher by adding to it some corrections and supplements through consultation. And an additional tool of stuffy has recourse to a stop watch to accurately measure the length of time into which nurses, doctors, nurse's aides, and extra hands take part, and measurement of their activities is conducted by classifying them into six stages-(1) Supplies preparation time, (2) Patient preparation time, (3) Treating time, (4) Posterior treatment care time, (5) Recording time, and (6) Follow-up care time after treatment. Data collection term covers for 38 workdays firm August 8, through September 14, 1985. The arithmetical means were obtained for the participation time of nurses, doctors, nurse's aides, and extra hands sprint for each of treatment activities which are composed of 41 Items out of 62. An assumption is made of charges for nursing by means of due process for calculating medical cost on the premise that the calculation of medical cost is derived from Medical Payments Insurance Rates, in consideration of labour cost of nurses, nurse's aides, and extra hands. The findings are summarized as follows; 1. Nurses' participation into 40 medical activities which consist of 41 Items of the Medical Payments Insurance Rates reveals 21 activities that form more than 80% of all tine spent by medical human resources for medical treatment, 16 activities that form more than 50% and less than 80%, 2 activities that form more than 30% and less than 50%, only 1 activity occupying less than 30%. And nurses spend more time than doctors, in 15 medical activities when taking part in them together with a doctor, while the number of activities in which a doctor spends more time than a nurse is no more than 2 activities. In addition, 21 activities the nurse perform independently of the doctor. On the other hands nursers aides' and extra hands' participation into medical treatment is found not independent in performance, but partial paticipation in it is conducted 7 activities. 2. Nursing fees (labour cost for the nurse) assumed for 40 medical activities which are composed of 41 Items of the Medical Payments Insurance Rates include 18 activities forming more than 50%, 7 activities forming more than 30% and less than 50%, 15 activities forming less than 30% in all 'labour cost for the nurse, nurse's aide, and extra hands the variable cost and the fixed cost'. And the variable cost for each of medical activities ranges from 12,907 to 6 wons, the fixed cost for each of medical activities from 14,342 to 20 wonts. 3. It is shown that charges for nursing assumed in this study form considerable importance in the official fixed medical rates and medical remuneration. And for nursing fee per activity, 7 activities form more than 101%, 10 activities more than 50% and less than 100%, 6 activities more than 30% and less than 50%, 15 activities less than 30%, in the aggregate remuneration for 38 activities in 41 Items. And the labour cost for the nurse, nurse's aide and extra hands the variable coat, and the fixed cost, considered in this study, are calculated by suggesting a difference from medical remuneration(the official fixed medical rates plus the basic medical charges for each of medical activities) as the remaining expenses. The 38 activities which are composed of 41 Items in the remaining expenses comes out as a minus figure, ranging from -64,994 to -16 wons in 20 medical activities. In conclusion, it is found that nurses take part in more medical activities than other medical resources, the number of medical activities that forms more than 50% of all the time spent by medical human resources for medical treatment indicates 37 activities out of 40, and the number of medical activities in which nurses take part independently of the doctor indicates 21 of the 40 medical activities of being composed of the 41 Items listed on the Medical Payments Insurance Rates Tables. The fact that determining nursing fees is indispensably needed, and that the nursing fees assumed in this study, occupy considerable importance in the official fixed medical rates or medical remuneration leads to our understanding anew nurses' professional nursing activities, especially in paying medical remuneration in medical payments insurance in Korea, and introducing an appropriate system of medical remuneration. Furthermore, a system should be based on a reasonable level of remuneration if introduced so that the possible crisis of hospital operation from the expanded existing medical payments insurance could be prevented, nurses could offer nursing care of good quality, therby they could make a great contribution not merely to the convalescence of the patient but to the promotion of the people's health.-
