근로자 고혈압 관리를 위한 전문가 시스템 개발에 관한 연구

건강에 대한 근로자들의 요구는 날로 증가하고 있고 건강진단 판정 결과 직업성질환보다 일반질환의 유병율이 높으며 이 가운데 고혈압과 관련이 있는 순환기질환의 비중이 높아 근로자 고혈압 관리의 필요성이 대두되었다. 그러나, 보건관리대행기관에서 대부분의 보건관리업무는 간호사에 의해 수행되어 고혈압에 관한 전문지식이 부족한 실정이므로 고혈압의 체계적인 관리를 위해 전문의료인력을 보조할 수 있는 효과적인 정보시스템이 더욱 요구되고 있다. 이에, 근로자 고혈압 관리를 위한 전문가 시스템을 개발하였다.

심혈관 전문의와 산업보건 전문의로부터 지식을 습득하고 Multiple Classification Ripple Down Rules 방법을 이용하여 지식간의 연관성을 규명하고, 이를 규칙으로 표현하였다. 실제 고혈압 환자 데이터를 이용하여 구축된 규칙 베이스의 타당성 검증을 실시하였다. 개발된 고혈압 관리 전문가 시스템과 도메인 전문가와의 검진 결과를 비교하여 고혈압 그룹별 진단에서는 82.49%, 고혈압 관리요법에 따른 분류에서는 97.35%의 진단 예측력을 나타내었다.

고혈압 그룹별 진단의 자동화를 위한 고혈압 관리 전문가 시스템을 개발함으로써 고혈압에 관한 전문지식이 부족한 산업보건기관 보건관리자(간호사)에게 전문지식을 제공하고, 근로자 건강진단이나 보건관리대행에 있어서 효율적인 근로자 고혈압 관리가 이루어지도록 하였다. 본 시스템을 통하여 보건관리자에 의해 근로자의 고혈압 관리가 이루어짐에 따라 사업주 및 근로자의 의료비용의 감소와 산업보건의료서비스의 질적 향상의 계기가 될 것으로 기대된다.

Demand for better health care for workers is increased in recent years. The results of health examination showed that prevalence of non-occupational diseases is higher than occupational disease. Since the cardiovascular disease which is directly related with hypertension takes a big part of the general disease, there

is a need for hypertension management at worksite. At present, most hypertension management at worksite has been carried out by the nurses from the Group Health Care Centers. Since most of them don't have a specialized knowledge on hypertension, an expert system was developed to provide them with such information

and to support their hypertension management activity.

Knowledge acquired from cardiovascular specialist and occupational medicine specialist were represented in the form of rules using the Multiple Classification Ripple Down Rules(MCRDR). The rules were validated by comparing with the actual hypertension data. The predictive power for the diagnosis of hypertension was 82.49% ; whereas the predictive power of treatment was 97.35%.

Expected benefits from the system are : improving a quality of hypertension management, and improving an efficiency of patient management by automating a diagnostic process.
