장의 발생은 다른 기관 계통과는 다른 다음과 같은 두 가지 특징을 갖는다. 첫째, 장은 발생 초에 잠시 탯줄 속으로 탈출되었다가 다시 복강 속으로 환원되며 회전을 하면서 성인의 배열을 갖는데, 언제 장이 탯줄 속으로 탈출되었다가 환원되는가, 언제 어느 정도의 회전이 일어나는가 등에 관한 정확한 발생 시간표가 제시되어 있지 않은 상태이다. 둘째, 장은 발생 중 상피의 증식에 의해 관강이 일시적으로 막혔다가 재형성된다고 설명되고 있다. 그러나, 실제로 발생 과정 중 관강이 완전히 막히는지는 아직 확실하지 않고, 관강이 막힌다면 막히는 부위, 정도 및 시기와 관강이 재형성되는 시기 등에 대해서 아직도 정설이 없다.

따라서 본 연구에서는 발생 2-12주 사이의 인간 배자 및 태아를 대상으로 수술현미경으로 미세 해부를 시행하고, 통상적인 조직 표본을 제작해 관찰함으로써 장이 처음 만들어지고 활발히 길어져 배꼽으로 탈출되었다가 복강으로 다시 들어가 최종의 위치에 놓이게

되는 과정과, 장 발생의 또 다른 특징으로 알려진 장상피에 의한 관강의 폐쇄가 실제 발생 과정 중에 일어나는지 등을 연구하여 다음과 같은 결과를 얻었다.

1. 장은 발생 5기에 처음 형성되어, 배자의 주름 형성과 함께 발생 10기에 원시장관의 세 부분이 처음 관찰되었고, 발생 12기에 정중선 상에 놓인 원시 장관이 형성되었다.

2. 장은 발생 15기에 시계 바늘 반대 방향으로 90도 회전하였고, 16기에 탈출되기 시작하였다. 발생 20기에는 다시 시계 바늘 반대 방향으로 90도 회전하였고, 발생 9주초에 빠르게 복강으로 환원될 때 맹장이 성인의 위치에 놓이면서 회전이 완료되었다.

3. 발생중인 배자에서 식도, 십이지장 및 장관의 다른 부분에서 상피의 증식은 인정할 수 있었으나, 관강이 완전히 폐쇄된 경우는 관찰하지 못하였다.

이 실험에서 제시한 장 발생의 시간표는 장의 탈출 및 환원 과정의 이상으로 발병되는 배꼽탈장이나 회전이상으로 초래되는 여러 종류의 선천성 질환 그리고 관강의 폐쇄 및 재형성과정에서 발병되는 장무공증이나 협착증 등의 발병기전을 이해하는데 도움이 될 것으로 생각되며, 다른 보고와 달리 발생 중 장관이 완전히 폐쇄되는 경우를 관찰하지 못 하여 보다 체계적인 연구가 이루어져야할 것으로 생각된다.

Unlike that of other organ systems, the development of intestine can be characterized by the following two traits. First, at the beginning of its development, the intestine is temporarily herniated into the umbilical cord, then reduced into the abdominal cavity, and finally reaches its adult location, rotating along the way. Nevertheless, no time table is yet available as to show, in a precise manner, when the intestine is herniated, when it is reduced, or to what extent the rotation occurs. Secondly, it is known that in the course of intestinal

development, the lumen is temporarily obstructed by epithelial proliferation and then undergoes the recanalization. However, it is not yet certain whether there is practically a complete luminal obstruction, even by supposing that there is a complete obstruction, no theory has yet been established to determine the time, location, or the extent of the obstruction, and the time of the recanalization.

In this study, 2-12 week old human embryos and fetuses have been used which were microdissected under a surgical microscope and were subsequently observed in an ordinary tissue slide. During this observation, two features of intestinal development have been perceived: first, the sequence from the initial intestinal formation to active growing followed by the herniation into the umbilical cord and finally the reduction into the abdominal cavity and into its ultimate location: second, the period, location and extent of the eventual obstruction of the lumen caused by proliferation of intestinal epithelium and the time of the recanalization. We have thus obtained the following results.

1. The intestine was first formed at Carnegie stage 5. Together with the folds of the embryos, three parts of the primitive gut were observed for the first time at stage 10. At stage 72, the primitive gut was located on the midline.

2. At stage 15, the intestine rotated to 90° counterclockwise, and at the next stage, it started herniation. There was another 90° counterclockwise rotation at stage 20, and at the beginning of the 9th week, with the rapid reduction into the abdominal cavity, the cecum was located in the proper adult position, and the rotation completed.

3. Although epithelial proliferation was recognizable in the esophagus, duodenum and other parts of the intestine of the developing embryo, we could not find any cases in which the lumen was completely obstructed.

It can be concluded that the results above may lead to a clarification of omphalocele caused by abnormality in the process of reduction on the one hand, and various congenital diseases brought about by abnormal rotation on the other hand.

They may as well give a better explanation as to the pathogenesis of intestinal atresia or of stenosis found in the course of luminal obstruction and recanalization.
