심장 수술을 시행할 때 저체온하에서 심장을 완전히 정지시키고 수술을 시행하는 것은 보편화된 방법이다. 그러나 정지된 상태의 심장은 산소의 공급이 중단된 상태로, 세포의 기본적인 대사와 구조의 유지를 위해 조직내의 ATP는 계속적으로 소모된다. 무산소 상태에서 세포는 해당작용을 통하여 ATP를 생산한다. 그러나 해당작용만을 통한 ATP의 생산으로는 심근이 필요로 하는 에너지를 다 충족시킬 수 없기 때문에, 허혈상태의 심근에서는 아미노산을 이용한 기질의 인산화를 통하여 부족한 ATP를 보충할 것이라는 여러 실험적인 증거들이 제시되었다. Glutamate와 aspartate는 여러 실험을 통하여 무산소 상태의 심근 및 허혈후 재관류 상태의 심근에서 심근에 필요한 에너지의 공급기질로 심근의 보호 및 심근의 기능 회복에 도움을 줄 수 있는 아미노산으로 추정되었다. 그러나 한편으로는 glutamate와 aspartate가 무산소 상태 및 허혈후 재관류 상태의 심근에서 대사에 이용되지 못하고 심근보호 효과도 일반적인 방법과 차이가 없다는 실험결과도 보고되고 있어 glutamate와 aspartate에 대한 이론이 정립되지 않은 상태이다. 저자는 aspartate와 glutamate가 무산소 상태의 심근에 대한 보호 효과가 있는지를 알아보기 위해 20마리의 쥐를 두 군으로 나누어 실험하였다. 대조군은 St. Thomas 심정지액으로 심정지를 유도하였고, 연구군은 St. Thomas 심정지액에 glutamate와 aspartate를 각각 13mM씩 섞은 심정지액으로 심정지를 유도한 후에 60분간의 허혈상태를 거쳐 심장의 회복을 관찰하였다. 실험결과 재관류후 60분이 지났을 때, 심정지전의 기준값에 비해 수축기 혈압은 대조군과 연구군에서 각각 86.2%와 94.8%로(p < 0.05), 심박동 수는 78.2%와 97.1%로(p < 0.001), 대동맥 과도 일출량은 74%와 97.9%로(p < 0.05), 관상동맥 관관류량은 69% 와 91.7%로(p < 0.05), 심박출량은 71.9%와 94.6%로(p < 0.001), 모두 연구 군에서 대조군에 비해 뛰어난 회복률을 나타냈다. 또한 실험후에 측정한 두 군의 수분 함유량 및 CPK, Lactic acid 유출량에는 차이가 없었으나, LDH 유출량은 연구군에서 유의하게 대조군에 비하여 감소 하였다. 이상의 결과로 glutamate와 aspartate는 결정성 냉심마비 용액에 첨가하여 사용될 때 우수한 심근보호 효과를 나타낸다고 생각된다. 그러나 glutamate와 aspartate를 심마비 용액에 섞어서 사용할 때에는 심마비용액의 pH가 산성이 되지 않도록 NaHC0^^3를 섞어주어야 하며, 첨가된 NaHC0^^3 에 의하여 Na+의 농도가 증가되지 않도록 심마비 용액을 조성할 때 NaCl의 양을 적절히 조절하는 것이 중요하리라 생각된다.



[영문]

Hypothermic cardiac arrest is an usual method in open heart surgery. Because the arrested heart is in anoxic state, ATP is depleted continuously to meet the need of basal metabolism of anoxic state of the heart. Myocardium in anoxic state produces ATPs through glycolysis. There have been some experimental evidences that substrate phosphorylation may play an important role in ATP production to meet the need of myocardial metabolism in anoxic state. Several experiments have also shown that glutamate and aspartate may be important in myocardial protection and myocardial recovery during anoxic state and reperfusion period after ischemia. On the other hand, other investigators proposed that glutamate and aspartate could not be utilized in ischemic myocardium so that they had no effect on the myocardial

protection. Isolated working rat heart model was used to compare the effect of aspartate and glutamate on recovery of the ischemic myocardium during hypothermic cardiac arrest. In control group(n=10), the hearts were arrested with St. Thomas' cardioplegic solution only, and those in study group(n=10) were arrested with L-glutamate and L-aspartate enriched St. Thomas' cardioplegic solution. After 60 minutes' arrest, hearts were reperfused, and subsequent 60 minutes' working period was followed. After 60 minutes' working period, the percentages of recovery rate were measured as following: systolic pressure; 94.8% in study group vs. 86.2% in control group (p < 0.05), heart rate; 97.1% vs. 69.0% (p<0.001), aortic over flow; 97.9% vs. 74.0% (p < 0.05), coronary perfusion flow; 91.7% vs. 69.0% (p < 0.05), cardiac output; 94.6% vs. 71.9% (p< 0.001). In conclusion, glutamate and aspartate may have good myocardial protective effect in ischemic myocardium during hypothermic cardiac arrest when used with cold crystalloid cardioplegic solution. However glutamate and aspartate are acid in its nature, additive use of NaHC0^^3 to adjust pH of cardioplegic solution seems to be important role in preservation of myocardium.
