Abstract
[한글]

백내장 낭외 적출술의 가장 중요한 합병증인 수정체 후낭 혼탁은 수술 후 시력 감소의 가장 흔한 원인이 되고 있다. 수정체 후낭 혼탁은 수정체 낭의 섬유화에 기인하며 이는 수술 후 남아 있는 상피 세포의 증식, 이동, 섬유모세포로의 화생등에 의한다. 따라서 수정체 상피 세포를 수술 중에 제거하려는 노력이 있어 왔으나 효과적인 방법이 아직 개발되지 않고 있다. 따라서 저자는 안과에서 검사용으로 널리 쓰이는 rose bengal을 광 감작제로 이용한 광 역학치료로 백내장 수술 후 발생하는 수정체 후낭의 섬유화를 억제할 수 있는 가에 대하여 알아보고자 하였다. 백색 가토에서 낭간 내 수정체 유화술 및 인공 수

정체 삽입술을 시행할 때 전방에 점탄 물질을 채운 상태에서 sodium hyaluronate에 용해시킨 rose bengal을 0.1 mg/ml의 농도로 0.2 ml를 수정체 낭내에 주입하고 420∼500 nm의 가시 광선 60 J/cm**2 을 조사하여 광 역학 치료를 시행한 후 수술 후 3개월까지 추적 관찰하였다. 수정체 후낭 혼탁의 발생 억제 효과를 직상 검안경 검사 및 세극등 검사, optical powermeter를 이용한 가시 광선의 차단률 조사,조직 검사 등을 통하여 관찰하였으며 rose bengal의 안 조직에 대한 독성을 알아보고 다음과 같은 결과를 얻었다.

1. 세극등 검사와 직상 검안경 검사에 의한 수정체 후낭 혼탁의 발생에 대한 광 역학 치료의 억제 효과를 관찰한 결과, 수술 후 3개월에 대조군, 가시 광선만을 조사한 제 1 실험군, rose bengal만을 투여한 제 2실험군, rose bengal과 가시 광선 조사를 하여 광 역학 치료를 한 제 3 실험군에서 수정체 후낭 혼탁의 평균 등급은 각각 2.9, 2.7, 1.7, 1.3이었으며 대조군과 제 1 실험군에 비하여 제 2실험군과 제 3 실험군에서 통계적으로 의의 있는 수정체 후낭 혼탁의 감소를 보였다.

2. Optical powermeter를 이용한 가시 광선의 차단률은 대조군이 30.6%, 제 1 실험군, 제 2실험군, 제 3 실험군이 각각 28.3%, 19.3%, 14.3% 이었으며 제 3 실험군이 대조군과 제 1 실험군에 비하여 통계적으로 의의 있는 수정체 후낭 혼탁의 감소를 보였다.

3. 수정체 후낭 혼탁의 조직 검사를 시행한 결과, 대조군 및 제 1 실험군에서는 대부분에서 수정체 전낭과 후낭 사이의 공간에 수정체 상피 세포의 증식과 많은 양의 재생된 수정체 피질이 관찰되었다. Rose bengal만을 투여한 제 2 실험군에서는 수정체 전낭 하 상피 세포의 부분적인 소실이 관찰되었고 수정체 피질의 재생도 대조군과 제 1 실험군에 비해 적음이 관찰되었다. 광 역학 치료를 시행한 제 3 실험군에서는 전낭 하 상피 세포의 뚜렷한 소실이 관찰되고 수정체 피질의 재생 역시 다른 군에 비하여 현격하게 적음이 관찰되었다.

4. 대조군 제 1 실험군, 제 2실험군, 제 3 실험군 모두에서 광학 현미경 소견상 각막, 모양 소대, 홍채, 모양체, 맥락막, 망막 등의 안 조직의 이상은 발견되지 않았다.

이상의 결과로 미루어 보아 백내장 낭외 적출술을 시행할 때 낭간 내 수정체 유화술을 이용하여 수정체 낭내에 sodium hyaluronate에 용해시킨 0.1 mg/ml의 rose bengal 0.2 ml를 주입하고 가시 광선을 조사하여 광 역학 치료를 시행함으로서 안 조직의 독성 없이 수정체 후낭 혼탁, 즉 수정체 후낭의 섬유화를 효과적으로 억제할 수 있다는 결론을 얻었다.

[영문]

One of the most important complication after extracapsular cataract extraction with posterior chamber intraocular lens implantation is posterior lens capsule opacification. This complication is the most common cause of decreased visual acuity after surgery. Posterior lens capsule opacification is caused by proliferation, migration and metaplasia to fibroblasts of the residual lens epithelial cells during surgery. So, the efforts were made to eliminate lens epithelial cells during surgery but none of them was proved to be effective.

In this study, the effect of the photodynamic therapy using rose bengal as photosensitizer on posterior lens capsule opacification after cataract surgery was investigated.

In white rabbit eyes, endocapsular phacoemulsification was performed. Rose bengal dissolved in sodium hyaluronate(0.2 ml in 0.1 mg/ml) injected into capsular bag and treated with visible ray 60 J/cm**2 of 420-500 nm. The posterior lens capsule opacification and the changes of ocular tissue were evaluated until 3 months after surgery. The results were as fallows

1. When evaluating the inhibition effect of photodynamic therapy on posterior lens capsule opacification by slit lamp and direct ophthalmoscopy at postoperative 3 months, the grades of control, first experimental group which was treated with visible ray only, second experimental group which was treated with rose bengal only, third experimental group(photodynamic therapy) were 2.9, 2.7, 1.7 and 1.3 respectively. The second and third experimental group showed statistically siglificant decrease in posterior capsular opacification when compared to control and the first experimental group(p<O.05).

2. The average obstruction rate of visible light, measured by optical powermeter was 30.6% in the control group,28.3% in the first experimental group, 19.3% in the second experimental group, and 14.3% in the third experimental group respectively. The third experimental group(photodynamic therapy) showed statistically significant decrease in posterior capsular opacification when compared to control and the first experimental group(P<0.05).

3. The histologic study of the posterior lens capsule opacification by light microscopy revealed that marked proliferation of lens epithelial cells and regenerated cortical lens material were observed in the capsular bag of the control and the first experimental group. These findings also ware observed in the second experimental group but the amount of these material was much less than that of those groups. In the capsular bag of the photodynamic therapy group ( the third experimental group), there were almost no lens epithelial cells and regenerated lens cortical material.

4. The histologic study for ocular toxicity by light microscopy that there was no abnormal findings in ocular tissue of all groups including the cornea. zonules, iris, ciliary body, choroid and retina.

From the above results, it may be concluded that the photodynamic therapy- the application of rose bengal dissolved in sodium hyaluronate ( 0.2 ml in 0.1 mg/ml) injacted into capsular bag and treated with visible ray 60 J/cm**2 of 420∼500 nm - during cataract surgery could effectively decrease the posterior lens capsule opacification without ocular toxicity.
