좌심실 부전이 관상동맥 우회로 조성수술에 미치는 영향

좌심실 부전이 동반된 허혈성 심장질환은 적극적으로 치료하여도 환자의 심한 기능장애를 유발하고, 높은 사망률을 나타내는 질환이다. 관상동맥 우회로 조성수술이 생존률을 높이고 협심증과 심부전의 증상을 완화시키는 역할을 하는 것으로 보고되고 있으나, 아직도 높은 수술 사망률과 낮은 장기생존률로 인해, 좌심실 부전은 관상동맥 우회로 조성수술의 주요 위험인자로 인식되고 있다. 이에 심한 좌심실 부전이 관상동맥 우회로 조성수술 성적에 미치는 영향을 조사하였다.

1991년 1월부터 1996년 12월 사이에 연세의료원 심장혈관센터에서 관상동맥 우회로 조성수술을 시행받은 환자 중 좌심실 박출계수가 30% 이하인 33명(좌심실 부전군)과 좌심실 박출계수가 30%를 초과하는 환자군에서 좌심실 부전군과 같은 수의 환자(대조군)를 선정하여 수술전 특성과 수술 성적을 비교하였다. 판막수술 및 심근경색의 기계적 합병증에 대한 수술을 동반 시행한 경우는 제외하였다. 좌심실 부전군 환자중 21례(62.6%)가 수술전 울혈성 심부전을 동반하고 있었으며 이에 따라 이뇨제, 강심제, 후부하감소재 투여 등의 내과적 치료를 받고있었고, 3례(9.1%)에서 수술전 심부전으로 인하여 대동맥내 풍선펌프 보조가 필요하였다. 수술전 심근경색의 과거력(84.8% vs. 27.3%)과 좌주관상동맥질환(45.5% vs. 18.2%)의 빈도는 좌심실 부전군의 환자들이 대조군의 환자들에 비해 많았다.

수술시 대동맥차단시간과, 이식편수의 차이는 없었으나, 좌심실 부전군의 경우 체외순환시간이 길었고 (148.6± 34.9분 vs. 126.7 ± 34.3분, p = 0.01), 내유동맥 사용은 적었다 (33.3% vs. 70.0%, p = 0.003). 수술후 사망은 좌심실 부전군에서만 2례(6.1%)가 있었으나 사망률에 통계적으로 유의한 차이는 없었다. 수술후 발생한 합병증중 저심박출증(39.4% vs. 9.1%)과 심실 빈맥(18.2% vs, 3.0%)이 좌심실 부전군에서 많이 발생하였으나, 수술후 심근경색, 출혈, 감염, 신부전, 호흡기계 합병증, 신경계 합병증의 발생은 두 군간에 차이가 없었다.

수술후 각군 평균 34.3 ± 22.5개월, 25.4 ± 19.5개월간 추적 관찰하였으며, 좌심실 부전군에서 2례의 만기 심인성 사망과 2례의 비심인성 사망이 있었고, 대조군에서는 심인성 사망은 없고, 2례의 비심인성 사망이 있었다. 대조군 2례에서 협심증이 재발하여 경피적 관상동맥 풍선확장술을 시행하였다. 환자의 기능상태는 NYHA (New York Hearth Association) 기능 분류 Ⅲ에 해당되는 경우가 좌심실 부전군에서는 4례, 대조군에는 1례가 있었으며, 나머지 환자들은 기능분류 Ⅰ 또는 Ⅱ의 상태로 생활하고 있다. 추적 관찰한 좌심실 박출계수가 좌심실 부전군에서는 수술전에 비해 4.8% 증가하였으나 (p = 0.0001), 대조군에서는 수술전과 비교하여 차이가 없었다 (p = 0.31).

좌심실 박출계수 30%이하로 좌심실 부전이 심한 허혈성 심장질환 환자의 관상동맥 우회로 조성수술은 수술 사망률이 대조군에 비해 유의하게 높지 않으며, 환자의 협심증 증상을 효과적으로 완화시키고, 심부전 증상을 호전시키는 효과를 기대할 수 있으므로 좌심실 부전이 동반된 환자에서도 적극적으로 수술이 고려되어야 한다.

[영문]

Ischemic heart disease combined with left ventricular dysfunction impairs patient's activity and is very fatal in spite of aggressive management. Many studies have been reported to improve survival rate and relieve of angina and heart failure symptoms by coronary artery bypass grafting (CABG). However, left ventricular dysfunction is still regarded as a major risk factor because of high surgical mortality rate and low long-term survival rate. This study was planned for evaluation of the effect of CABG in the patients of left ventricular dysfunction.

Between January 1991 and December 1996, 33 patients with left ventricular ejection fraction (LVEF) equal to and below 30% (left ventriclular dysfunction group) underwent isolated CABG at Cardiovascular Center of Yonsei University. Same number of patients with LVEF above 30% were randomly selected (control group). Preoperative conditions, operative conditions and surgical results of two groups were compared. In the left ventricle (LV) dysfunction group, 21 patients (62.6%) suffered from congestive heart failure and reqiured medical managment such as diuretics, inotropics, and after load reducing agents. Three (9.1%) of the LV dysfunction group patients required LABP (intra-aortic balloon pump) support preoperatively. History of myocardial infarction (84.8% vs. 27.3%) and left main coronary disease(45.5% vs, 18.2%) were combined more frequently in the LV

dysfunction group. Cardiopulmonary bypass time was longer in the LV dysfunction group (148.6 ± 34.9 min vs. 126.7 ± 34.3 min, p = 0.01), but the aorta cross clamp time was not different. Left internal mammary artery graft was more frequently used in control group (33.3% vs. 70.0%, p = 0.003), but there was no difference in number of grafts. There were two surgical deaths (6.1%) in the LV dysfunction group only. However, the difference of mortality rate between two groups was not statistically significant. The low cardiac output syndrome (39.4% vs. 9.1%) and ventricular fibrillation (18.2% vs. 3.0%) were more frequent in the LV dysfunction group postoperatively. But there was no difference in incidences of postoperative myocardial infarction, bleeding, infection, renal failure, respiratory problem and neurologic complication. During the follow-up period of 34.3 ± 22.5 (LV dysfunction group) and 25.4 ± 19.5(control group) months, there

were two late cardiac deaths and two non-cardiac deaths in the LV dysfunctin group. There were only two non-cardiac deaths in the control group. The angina pectoris recured in two patients of the control group and these patients required PTCA(percutaneous transluminal coronary angioplasty). The follow-up functional status of the patients were NYHA (New York heart association) class Ⅰ or Ⅱ except in four of the LV dysfunction group and in one of the control group whose functional class was Ⅲ. The follow-up left ventricular ejectrion fraction was improved in the LV dysfunction group compared to the preoperative EF(p = 0.0001).

However, the follow-up EF in group Ⅱ did not differ from preoperative one.

In conclusion, CABG in ischemic heart disease combined with left ventricular dysfunction improves left ventricular function as well as angina symptom with acceptable surgical mortality. Therefore, CABG should be considered more in ischemic heart disese patient with poor left ventricular function.
