Extremely varied manifestation in Granular corneal dystrophy type II

Abstract

[한글]목적 : 제2형 과립형각막이상증 이형접합자의 표현형의 변이의

다양한 정도를 slit lamp 사진을 통해 보고 하기 위하여.

제2형 과립형각막이상증은 각막굴절교정수술 후 악화된다고 잘

알려져 있다. 그러나 20대 환자에서 제2형 과립형각막이상증의

진단은 그 표현형의 다양함으로 인해 어려울 수 있다. 최소한의

각막혼탁만을 보이는 증례들에 대한 사진을 통한 고찰은 제2형

과립형각막이상증의 진단의 정확도를 높일 수 있을 것이다.

방법 : 비슷한 연령대의 다른 환자들보다 매우 경하거나, 심한 정도의

혼탁을 보이는 14명의 제2형 과립형각막이상증 이형접합자 환자의 Slit lamp 사진을 분석하였다.

결과 : 비슷한 연령대의 환자 군에서도 매우 경한 각막혼탁부터

동공이 보이지 않을 정도의 심한 혼탁까지 표현형의 차이가 매우

크게 관찰 되었다.

결론 : 제2형 과립형각막이상증의 임상양상은 환자마다 극단적으로

차이를 보일 수 있다. 각막굴절교정수술을 고려하기 전에 이 질환의

각막 혼탁양상이 얼마나 다양할 수 있는지를 파악하는 것은 매우

중요하다.

[영문]PurposeTo report the various extent of phenotypic variation for heterozygote Granular corneal dystrophy type II using slit lamp photographs. GCDII is now well-known to be aggravated after refractive surgery. But in twenties, diagnosis of GCDII can be difficult due to variation of phenotype. Photographic review of minimal cases of GCDII will help ophthalmologists to raise their level of suspicion. MethodSlit-lamp photographs of 14 heterozygote GCDII patients either in extremely mild, middle, or severe condition were analyzed for the extent of corneal opacity. ResultAlthough patients were in similar age group, significant difference in their phenotypes was observed, ranging from very mild corneal opacity to very severe opacity, in which pupil was not visible. ConclusionThe manifestation of GCDII can be extremely varied among patients and it is important to know the extent of variation before considering refractive surgery. Clinicians should be careful to rule out GCDII in patients of twenties or thirties with corneal opacity before recommending refractive laser surgery.
