투석을 시행하는 퇴행성 요추질환 환자군에서의 수술적 치료의 임상결과

Abstract

[한글]목 적 : 투석 치료를 요하는 만성신부전 환자 중 요추부 퇴행성 질환으로 척추수술을 시행 받은 환자의 임상 및 방사선학적 결과에 대해 알아본다.대상 및 방법 : 1998년 8월부터 2008년 12월까지 투석 시행하던 요추부 퇴행성 환자 중 수술적 치료를 시행한 9명의 환자에 대해, 임상결과는 JOA score를 이용하여 조사하였으며, 방사선학적 결과는 추시시 시행한 전후방 및 측면 굴곡-신전 사진을 이용하여 유합여부를 확인하였다. 혈중 알칼리성 탈인산 가수분해효소(Alkaline phosphatase)를 측정하여, 술 후 임상증상 호전에 따른 활동도 변화에 대한 골 흡수-형성에 대해 간접 평가하였다. 결 과 : JOA score는 술전 평균 12.1, 술후 6개월 17.4, 최종 추시 15.6로 조사되었으며, 이는 통계학적 유의한 차이가 있었다. 방사선학적 평가 결과, 유합술을 시행한 5례 중 2례에서 유합을 보였다. 혈중 알칼리성 탈인산 가수분해효소는 JOA score가 증가함에 따라 감소하였다가, 마지막 추시시 증가하여 JOA score와 역상관 관계의 양상을 보였다. 감압술만 시행한 경우와 유합술을 시행한 경우의 두 그룹간 임상 호전 정도의 차이는 없었으며, 골 유합술 시행한 경우 골유합 획득 여부와 상관 없이 술후 임상증상은 호전되었다. 결 론 : 투석 시행 중인 환자의 요추부 퇴행성 질환의 수술 치료시 수술의 종류와 상관없이 결과는 호전되었다. 특히 유합술을 시행한 경우에는 골 유합 여부와 상관없이 임상결과가 호전되었다.

[영문]Purpose : To Evaluate the clinical and radiological outcome of the operative treatment for lumbar degenerative diseases in patients undergoing dialysis.

Materials and Methods : Nine cases of lumbar degenerative spondylopahty with CRF undergoing dialysis underwent the operative treatment from August 1998 to December 2008. Clinical and radiological outcomes were evaluated using JOA score, plain X-ray follow-up and serum alkaline phosphatase level.

Results : We had 1 case of postoperative mortality who was treated with peritoneal dialysis because of sepsis casused by postoperative panperitonitis and 2 cases of postoperative complications including postoperative discitis and aspiration pneumonia. The JOA scores improved after operative treatment but got worse as the follow-up went on. Only 2 of 5 cases that underwent fusion procedure had the radiological bony union during follow-up.

Serum alkaline phosphatase levels seemed to be decreased as the postoperative JOA scores increased and to increase with long-term follow-up as the follow-up JOA scores decreased, but they were not statistically significant.

Conclusion : Surgical treatments outcomes of the degenerative spine disease patients with dialysis were improved regardless of preoperative conditions. Alkaline phosphatase levels changed reversely compared with JOA scores but the changes were not statistically significant.
