과민성방광 진단에서의 전류인지역치 측정 검사의 유용성

Abstract
[한글]목적 : 요절박은 과민성방광 진단에 있어 매우 핵심적인 요소이다. 하지만 문진을 통해 환자가 호소하는 증세가 요절박이 맞는지, 그리고 맞다면 얼마나 정도가 심한지를 예측하는 것은 매우 애매하다. 하부요로의 감각신경을 통해 요절박이 인지되기에 과민성방광군과 비과민성방광군에서의 감각신경 기능의 차이를 측정하여 비교하였다.재료 및 방법 : 33명의 여자환자를 과민성방광군 22명과 비과민성방광군 11명으로 분류하였다. 모든 환자의 방광점막에 2000Hz, 250Hz, 5Hz의 신경선택적 전류자극을 주어 전류인지역치를 측정하였으며, 두 군 간에 전류인지역치 값을 비교하였다. 과민성방광군에서 5점 척도를 이용한 배뇨일기를 3일간 작성하게 하여, 각 3개의 주파수에서의 전류인지역치 값과 요절박 횟수/총 배뇨횟수, 요절박 호소시 배뇨량/기능적 방광용적 간의 상관관계를 확인하였다. 요역동학 검사를 실시하여 최초 배뇨요의, 강한 배뇨요의, 참지 못할 정도의 배뇨요의에서의 방광용적과 각 주파수에서의 전류인지역치 값과의 상관관계를 분석하였다.결과 : 과민성방광군과 비과민성방광군 사이에 나이의 차이는 없었다.(64.0±9.5 세 vs 67.4±11.4 세, p=0.36) 2000, 250, 5Hz에서의 전류인지역치 값은 과민성방광군에서는 각각 2.36±1.35 mA, 1.21±0.70 mA, 0.87±0.59 mA이었으며, 비과민성방광군에서는 각각 4.14±1.48 mA, 2.06±0.73 mA, 1.43±0.65 mA이었다. 모든 주파수에서의 전류인지역치 값은 과민성방광군에서 비과민성방광군에 비해 낮았다 (p=0.002, 0.004, 0.009). 요절박 횟수/총 배뇨횟수, 요절박 호소시 배뇨량/기능적 방광용적 값은 250Hz에서의 전류인지역치 값과 의미있는 상관관계를 보였다(γ=-0.432, p=0.045, γ=0.504, p=0.017). 두 군 간에 최초 배뇨요의와 강한 배뇨요의 시의 방광용적에는 차이가 없었으나, 참지 못할 정도의 배뇨요의 시 방광용적은 비과민성방광군(443.6±124.2mL)에 비해 과민성방광군(318.6±136.9mL)에서 의미있게 낮았다. 모든 주파수에서 전류인지역치 값은 정상 방광 감각과 상관관계가 없었다.결론 : 과민성방광군에서의 전류인지역치 값이 비과민성방광군에 비해 유의미하게 낮았다. 과민성방광 환자 중 250Hz에서 전류인지역치 값이 낮을수록 총 배뇨 중 요절박 횟수가 많았고 요절박시 기능적 방광용적에 비해 배뇨량이 적었다. 참지 못할 정도의 배뇨요의 시에만 두 군간에 방광용적의 차이가 있었다. 이는 과민성방광의 경우, 감각신경이 과민하다는 것을 의미하며 이는 진단에 있어 매우 중요한 요소가 된다.



[영문]PURPOSE: Urgency is very important in diagnosing overactive bladder (OAB). However, it is difficult to correctly identify whether the symptom that patients complain of is urgency and if so, the degree of severity, through patient interview or self-reported questionnaire. Because this symptom is perceived by sensory nerves of the lower urinary tract, we evaluated the differences in sensory nerve function between patients with and without OAB.

MATERAILS AND METHODS: Thirty-three female patients were divided into 2 groups: 22 with OAB and 11 without. Current perception threshold (CPT) values were measured from the bladder mucosa in all patients using neuroselective electrical stimulation at frequencies of 2000, 250, and 5 Hz; and differences in CPT values were evaluated between the 2 groups. In OAB patients, data from 3-day bladder diaries using a 5-point urgency rating scale were recorded, and the correlation between CPT values at all 3 frequencies and total number of urgency episodes / total number of voids and mean volume of urgency episodes / functional capacity was investigated. Conventional urodynamic test was done in all patients and we evaluated the association between CPT values at all 3 frequencies and bladder volumes at first desire to void (FDV), strong desire to void (SDV), and compelling desire to voids (CDV).

RESULTS: There was no age difference between OAB (mean, 64.0±9.5 years) and non-OAB (mean, 67.4±11.4 years, p=0.36) patients. CPT values at 2000, 250, and 5 Hz were 2.36±1.35 mA, 1.21±0.70 mA, and 0.87±0.59 mA in the OAB group and 4.14±1.48 mA, 2.06±0.73 mA, and 1.43±0.65 mA in the non-OAB group, respectively. All CPT values in OAB patients were significantly lower than those of non-OAB patients (p=0.002, 0.004, 0.009). Total number of urgency episodes / total number of voids and mean volume of urgency episodes / functional capacity from bladder diaries were correlated with CPT values at 250 Hz (γ= -0.432, p=0.045, γ=0.504, p=0.017). There were no differences in FDV and SDV between the OAB and non-OAB groups, but bladder volume at CDV (318.6±136.9 mL) was significantly lower in OAB patients than that of non-OAB patients (443.6±124.2 mL, p=0.02). Bladder volume at 3 bladder sensations was not correlated with CPT values at all frequencies. CONCLUSIONS: CPT values in OAB patients were significantly lower than those of non-OAB patients. The lower the CPT values at 250 Hz were, the more the patients with OAB felt urgency in total voids, and lower bladder volume at urgency was compared to functional capacity. There was no difference in bladder volume at FDV and SDV but a significant difference at CDV. This may be because that the sensory nerve for OAB is hypersensitive, which is an important criterion that characterizes the condition.
