Abstract
[한글]심폐우회로를 채우는 충진액(priming solution)은 일반적으로 혈액 대신 수액, 특히 교질액을 첨가하여 만든다. 교질액 중 에서도 알부민은, 다른 교질액에 비하여 혈액응고 장애 등의 부작용도 적으면서, 우수한 삼투압 유지 및 혈장 증량 효과를 보이고, 항 염증작용도 기대할 수 있어 흔히 사용되는데, 고가의 약품이고, 질병 전염 가능성과 같은 부작용 등이 문제가 될 수 있다. Hydroxyethyl starch(HES)는 우수한 혈장 증량 효과를 갖고 있지만, 혈액 응고 장애를 유발하여 출혈량과 수혈량을 증가 시키는 단점이 있었다. 가장 최근에 개발된 HES 130/0.4(분자량/ 몰치환율) 용액은 분자량 및 몰치환율이 낮아, 반감기가 짧고, 삼투압을 증가 시키는 작용이 탁월하고, 지혈 기전에 미치는 장애가 적으며, 혈장에서 보다 빠르게 제거 된다는 점 등을 고려할 때, 충진액에 포함되는 기존의 고가의 알부민을 대체 할 수 있는 적합한 교질액이라 판단된다. 이에 본 연구에서는 심폐우회로를 이용한 승모판수술이 예정된 성인 환자 36명을 대상으로, 심폐우회로 충진액에 알부민을 첨가하는 군과, HES 130/0.4를 첨가하는 군에 무작위로 배정하여 두 교질액이 혈액응고 체계, 출혈량 및 수혈량과 염증반응에 미치는 영향을 살펴 보았다. 두 군 모두에서 수술 후 사망하거나 재수술 한 경우는 없었으며, 두 군간 혈액응고 수치(혈소판 수치, 트롬보플라스틴시간, 활성화 부분트롬보플라스틴시간, 혈전탄성묘사도값)는 통계적으로 유의한 차이가 없었다. 염증 수치(백혈구수치, interleukin(IL)-6, interleukin(IL)-8, tumor necrosis factor-α(TNF-α)), 출혈량 및 수혈량, 호흡기 적용기간, 중환자실 재원기간, 병원 재원기간도 두 군간에 유사하였다. 본 연구 결과 HES 130/0.4는 알부민에 비하여 혈액응고 장애와 수혈량을 증가시키지 않았으며, 염증반응에 미치는 영향도 유사한 것으로 관찰되었다. 이는 승모판수술에서 HES 130/0.4이 고가이고, 질병 전염의 가능성이 있는 알부민을 대체하여, 충진액으로 안전하게 사용될 수 있음을 보여주는 결과라고 생각한다.



[영문]Albumin has very rare serious-adverse side effects, excellence in plasma expanding effects and anti-inflammatory responses, and thus is a preferable colloid solution as a component of priming solution for cardiopulmonary circuit in cardiac surgery, although

it is expensive and has a possibility to transmit disease. Hydroxyethyl starch (HES) is a widely used inexpensive alternative to albumin for correcting hypovolemia but having

concerns about altered coagulation. A new HES 130/0.4 has several advantages compared to former HES, which include fewer effects on hemostasis while maintaining similar plasma expanding effect and infrequent incidence of allergic reactions. We compared the effects of HES 130/0.4 on coagulation and pro-inflammatory reaction to albumin used as a component of priming solution in patients undergoing mitral valve surgery.

In a prospective, randomized, double-blind study, 36 low risk patients undergoing mitral valular surgery using cardiopulmonary bypass (CPB) were randomly allocated to albumin (n = 18) or HES 130/0.4 group (n = 18), in which 500 ml of 4% albumin or 6%

HES 130/0.4 solution was mixed to the priming solution of CPB circuit, respectively. Amount of fluid balance, blood loss and transfusion, coagulation variables including thromboelastography (TEG®) parameters, inflammatory markers including white blood

cell count, interlukin (IL)-6, IL-8 and tumor necrotic factor (TNF)-α and short term clinical outcome including duration of ventilatory care, intensive care unit stay and hospital stay were compared between two groups. There were no significant differences in amount of blood loss and transfusion, changes in coagulation variables. Changes in inflammatory markers and clinical outcomes were similar in both groups.starch HES 130/0.4 compared with HES 200/0.5 during and after extended acute normovolemic haemodilution. In conclusion, HES 130/0.4 solution as a component of priming solution for nonbiocompatible CPB circuit demonstrated similar efficacy in terms of coagulation and

pro-inflammatory reaction compared to albumin in low risk patients undergoing mitral valvular heart surgery.
