dc.description.abstract[한글] 본 연구는 기혼 여성의 유방엑스선촬영술 수검의 개인적 요인(유방암 위험 평가 지표, 유방암에 대한 민감성)과 사회적 요인(사회연결망, 사회적 지지, 사회적 영향)을 조사하여 각 요인이 기혼 여성의 유방엑스선촬영술 수검 여부에 미치는 영향을 파악하고, 궁극적으로 여성의 유방엑스선촬영술 수검 영향 요인을 규명하기 위한 횡단적 조사연구이다. 본 연구는 서울과 경기지역에서 만 35세 이상의 기혼 여성을 대상으로 2008년 10월 24일부터 2008년 12월 4일까지 구조화된 설문지를 사용하여 자가 보고식으로 자료 수집하였다. 총 187명의 설문지가 최종분석에 이용되었고, 설문지 1부 당 평균 소요시간은 약 15분 정도였다. 개인적 요인은 Lee 등(2004)에 의해 개발된 유방암 평가 지표와 Champion(1999)이 개발한 도구를 김태경(2008)이 사용한 것을 수정 보완한 도구로 측정하였고, 사회적 요인은 미국 Center for Public Health and Health Disparities Project 중 사회연결망 설문을 수정하여(염유식, 2003) 천의영(2007)이 사용한 문항과 Allen 등(2007)의 사회연결망 특성 문항을 번역 수정한 도구로 측정하였다. 수집된 자료로 SPSS Win 15.0 program을 이용하여 기술통계와 χ2 test, independent t-test, one-way ANOVA 및 다중 로지스틱 회귀분석을 실시한 주요 결과는 다음과 같다. 1. 대상자의 지난 2년간 유방엑스선촬영술 수검률은 35.8%로 나타났다. 대상자 유방엑스선촬영술 수검 유무와 대상자의 일반적 특성을 포함한 개인적 요인 중 통계적으로 유의했던 변수는 종교 유무(x2=6.19, p<0.05), 가족월수입(t=-2.37, p<0.05), 정기적 추후관리(x2=23.10, p<0.01), 지난 2년간 종합건강검진 유무(x2=10.93, p<0.01), 유방질병력(x2=9.31, p<0.01)으로 나타났다. 2. 유방엑스선촬영술 수검 유무와 대상자의 사회적 요인 중 통계적으로 유의했던 변수는 사회적 영향과 관련된 것으로 사회적 규범(t=-5.07, p<0.01)과 사회연결망 구성원의 유방엑스선촬영술 필요성 인지(t=-4.03, p<0.01)로 나타났다. 3. 기혼 여성의 유방엑스선촬영술 수검에 영향을 미치는 요인을 규명하기 위해 상호간의 영향을 통제한 상태에서의 결과를 볼 수 있는 다중 로지스틱 회귀분석을 실시한 결과, 의료기관에서의 상담과 관리를 받은 군이 받지 않은 군에 비해 3.33배, 유방암 위험 평가 경계군이 정상군보다 6.19배, 사회적 규범이 1점 높을수록 2.54배, 사회연결망 구성원의 유방엑스선촬영술 필요 인지가 1점 높을수록 2.39배 더 유방엑스선촬영을 하는 것으로 나타났다. 본 연구결과에서 개인적 요인으로 정기적 추후관리와 유방암 위험 평가, 사회적 요인으로 사회적 규범과 주관적 신념은 유방엑스선촬영술 수검 여부에 유의한 영향을 미칠 뿐 아니라, 가장 강력한 영향 요인임을 확인하였다. 결론적으로 기혼여성의 유방암 조기발견을 위한 유방엑스선촬영술 수검률 향상을 위해, 개인적 요인으로 유방암 위험 지표를 사정한 맞춤형 중재가 적용되어야 할 것이며 개인적 요인 강화 전략에 더해 사회적 요인으로 사회연결망, 사회적 지지와 사회적 영향을 고려한 중재 프로그램을 개발하여 적용한다면, 유방암 조기 검진이 일회성으로 끝나기 보다는 연결망 구조를 통해 계속적이고 꾸준한 검진 행위를 이끌어 기혼 여성의 유방암 조기 발견율 향상에 기여하리라 생각된다. [영문] The purpose of this study was to identify the personal and social factors affecting mammography screening in married Korean women. Personal factors were breast cancer risk appraisal and perceived susceptibility, and social factors were social network, social support, and social influences. The data used in this study were collected from 187 married women aged 35 and above using public health centers, health promotion centers, cultural centers, obstetrics and gynecology hospitals, and women in the community. The interviews were conducted from October 24 to December 4, 2008, using a standardzed structured questionnaire. The data were analyzed using descriptive statistics, χ2 test, independent t-test, one-way ANOVA and multiple logistic regression analysis with the SPSS 15.0 program. The major findings are as follows; 1. The mammography screening rate was 35.8%. Personal factors which showed statistically significant differences between the screening group and non-screening group were; (1) religion (x2=6.19, p<0.05), (2) family income (t=-2.37, p<0.05), (3) regular medical-care (x2=23.10, p<0.01), (4) generalized health examination during past the 2 years (x2=10.93, p<0.01), and (5) personal history of breast disease (x2=9.31, p<0.01). 2. Social factors which showed statistically significant differences between screening and non-screening groups were; (1) social norm, i.e., belief about the proportion of age peers who undergo regular mammography screening (t=-5.07, p<0.01), and (2) subjective norms, defined as the extent to which social network members approved of mammography (t=-4.03, p<0.01). 3. Using multiple logistic regression analysis; regular medical-care, breast cancer risk appraisal, social norm, and subjective norms were highly predictive of subsequent mammography(that controlling for it left no variability to be explained by other variables). This study confirmed that personal factors such as regular medical-care and breast cancer risk appraisal, and social factors such as social norm and subjective norms, not only influence mammography screening, but are the most strong influencing factors. The results of this study suggest adapting a socially mediated program that activates tailored interventions, such as utilizing peer educators to reinforce and activate existent network structures, such an approach would integrate social network and social support systems, promoting early detection of breast cancer as a continuous activity rather than a one time event.-
