의료보험용의약품 약가 보고 시스템 개발 연구

의약품 분야의 전산화는 다른 분야에 비해 다소 늦은감이 없지 않으나 최근 보건복지부에서 의약품 유통체계를 전산화하고, 의료보험의약품의 가격을 현행고시가 상환제도에서 실거래가격에 자동 연계토록 함으로서 시장 조정기능을 최대한 활용한 종합정보관리시스

템이 발표되면서 체계적이며 종합적으로 추진될 예정이다.

현행 약가제도는 고시가와 실거래가격간의 심한 약가차액이 발생되어 보험재정의 누수현상이 발생되고 있으며, 유통단계에서의 음성적인 뒷거래, 이면계약, 덤핑 등 불공정행위가 만연하고 과잉투약의 가능성 마저 있다. 또한 보건복지부에서는 유통되고 있는 의약품의 실거래가격을 파악하기 위하여 전국요양기관을 대상으로 극히 한정된 샘플을 선정하여 조사하고 있으므로, 이에 따른 오차의 발생뿐만 아니라 수작업으로 인한 오차, 장기간의 작업으로 인한 행정력의 낭비 등 여러 가지 문제점을 내포하고 있다.

유통되고 있는 의약품의 정확한 실 거래가격 파악은 보험재정의 누수현상을 방지할 수있으므로 매우 중요한 일이며, 때문에 유통되는 전 의약품 실 거래가격의 파악을 위해서는 전산화가 필수적이라 할 수 있다.

따라서 본 연구에서는 의료보험용 의약품의 실 거래가격을 파악하고, 약가를 관리하기위하여 시스템분석과 설계 과정을 통하여 약가관리와 연관된 주요 6개 기관을 보고 내용별로 각 개체를 연산화하였으며, 파악된 연산과 속성을 캡슐화하고 개체를 생성함으로서 개체-관계성도 (E-R Diagram)를 작성하였고, 업무와 업무간의 자료의 흐름을 자료흐름도(DFD)를 통하여 도형화하였을 뿐만 아니라, 계충적입출력모형 (HIPO)을 이용하여 시스템을 체계적으로 분할해 나가면서 도표를 사용하여 명세화 함으로서 시스템을 쉽게 이해할수 있도록 하였다. 또한 인터넷을 통한 실 거래자료 보고와 관리화면을 디자인하였고 국내외의 문헌고찰을 통한 현 의료보험 약가제도의 문제점을 분석하여 새로운 약가 관리시스템의 향후 실시방향을 제시하였을 뿐만 아니라 전자자료교환(EDI)망, 엑스트라넷 (Extranet), 디스켓을 이용한 보고방안 등에 대한 장단점을 비교 분석하였다.

부작용모니터링 시스템, 의약품 허가관리시스템, 처방전달시스템, 재고관리시스템 등 현재 의약품 분야의 전산화는 최근 들어 보건복지부, 연세대학교 보건대학원 등을 중심으로 활발히 진행중이거나, 현재 사용중인 시스템들 상호간의 연계성이 부족하여 통합 관리되지 못하고 있는 실정이다. 따라서 의약품 관리시스템이 성공하기 위해서는 이들 시스템간의 종합적이고 체계적인 통합이 이루어져야만 하며, 또한 사용자가 이용하기 쉬워야하고 이를 사용함으로서 경제적 이득과 편리성이 있어야 한다.

그러기 위해서 본 연구의 주제인 약가 보고시스템도 의약품 물류정보시스템, 의약품물류망, 금용결제시스템 등과 향후 연계되어 운영되어야 하며, 보안시스템의 구축, 관련제도의 보완, 전산교육의 시행 등 계획성 있는 사업이 계속되어야 할 것이다.

The identification of real selling prices for the reimbursed medicines is a key element to avoid the loss phenomenon from the National healthcare fund so that computerization can perform a mandatory and important task in identifying the real selling prices in the market. This study identifies the real selling prices for reimbursed medicines, and attempts to manage the medicinal prices through system analysis and formation monitoring, Also, data flow between the activity units is

visualized in DFD, and by utilizing HIPO, the system is analyzed systemically. In addition, the reporting system of real selling prices on the Internet and management documents was designed. Problems existing in the current reimbursement system was also analyzed based on the study performed in the publications cited in worldwide. Thereby the study gives the future perspectives taking control the prices and provide the comparative analytical modules on reporting system in the format of EDI network, Extranet and Diskettes.

Systems on adverse reaction monitoring, product marketing approval, prescription delivery and management of stock in medicines have been studied actively in the cooperative efforts in KMOHSA and The Granduate School of Health Science and Management in Yonsei University and Unfortunately, due to uncompatability between the computerized system integrated systems are not yet managed so far. For successful management in medicines control it is essential to integrate the information between the systems, to be utilized with ease and to also provide

economical benefits and users with convenience in use.

The medicines Price Reporting system focusing main subject in this report should be unilized in connection with computerized systems in supply, payment and products information in nationwide healthcare setting and further security system, improvement on the regulations and education are to be implemented properly in

plan.
