새로운 의료기술의 확산과 치료결과에 대한 의료기술평가 : 액시머 레이저를 이용한 근시교정술을 중심으로

본 연구는 새로운 의료 기술인 엑시머 레이저가 국내에 도입되어 확산되는 추세와 엑시머 레이저를 이용한 근시교정술의 치료결과에 대한 의료기술 평가를 시행하였다. 그리하여 보사부 신고자료, 수입무역회사의 판매자료 및 국내 안과개설의료기관 현황자료와 서

울 시내에 위치한 모대학 부속병원의 안과에서 1991년 4월 7일부터 1991년 7월 3일까지 안과 외래에서 엑시머 레이저를 이용한 근시교정술을 받은 182명을 대상으로 의무기록을 조사하여 다음과 같은 결과를 얻었다.

1993년 5월 현재 국내에 도입된 엑시머 레이저 장비는 총 52대이며 1988년에 처음 도입되어 급속한 확산 추세를 보이고 있다. 연도별 도입현황을 보면, 91년에 18대가 도입되고, 92년에 24대가 도입되어, 전체의 80.8%에 해당되는 42대가 91년 이후에 도입되었다.

국내의 총 649개의 안과 개설의료기관 중 52개의 의료기관에서 장비를 보유하고 있는 것으로 나타나 8.0%의 장비보유율을 보였다.

병원규모별 보유현황을 보면, 의원급에 34대가 설치되어 있어, 전체의 65.4%를 차지하고 있으며, 병원급의 병원에는 11대가 도입되어 있어, 전체의 21.1%를 차지하고 있으며, 종합병원급(대학병원 포함)이상에는 7대가 도입되어있어 전체의 13.5%를 차지하고 있다.

지역별 보유 현황을 보면, 서울을 비롯한 수도권에 31대가 도입되어있어 전체의 59.7%를 차지하고 있으며, 40.3%가 나머지 시·도에 설치되어 있는 분포양상을 보였다.

치료결과를 보면, 수술전의 시력이 수술후에는 평균 0.687 만큼 더 향상되었고, 굴절력은 4.641만큼 더 감소됨으로서 시력향상 정도가 높은 것으로 나타났다. 또한 수술전의 시력이 나쁠수록, 연령이 적을 수록, 남성보다는 여성에서, 시력의 향상이 더 높게 나왔다.

재방문 행위는 통계적으로 유의하지 않은 것으로 나왔다.

결론적으로, 첨단의 새로운 치료기술인 엑시머레이저를 이용한 근시교정술은 빠른 확산추세를 보이고 있으며, 치료결과에 있어서는 시력향상의 정도가 높다는 결과를 얻었다.

본 연구는 새로운 의료기술에 대해 확산추세와 치료결과의 두가지 측면에서 의료기술 평가를 시행하였으나, 환자의 만족도와 의사의 특성에 따른 치료결과 분석 및 비용-편익 분석등의 분석은 자료의 제한 때문에 시도하지 못했다. 따라서 엄밀한 의미에서의 의료기술 평가라고 하기는 어려우나, 아직까지 국내에서 의료기술평가에 대한 연구가 이루어지지 알았던 바, 본 연구는 앞으로 이루어질 의료기술 평가의 연구에 기초자료가 되고자 한다.

The purpose of this study is to assess the diffusion of excimer laser as a new medical technology in Korea and medical outcomes.

One data about diffusion of excimer laser was collected from the report to the Ministry of Health and Social Affairs and sales report of the import trade company in Korea.

Another data about the medical outcomes was collected from the medical records of 182 patients who has visited to the opthalmology out dispensary for the phtorefractive keratectomy with excimer laser.

The following findings from this study were obtained:

1. The excimer laser equipment was introduced into Korea since 1988 and showed very rapid diffusion trend. Total 52 excimer laser equipments have been installed thus far in Korea.

2. Most of excimer laser equipment were installed in clinics rather than in general hospitals. Most of them were distributed around metropalitan areas including Seoul. Also there has been a rapid influx of excimer laser equipment since 1991.

3. The analysis of medical outcomes of photorefractive keratectomy with excimer laser revealed good improvement in visual acuity and refraction.

4. The results of multiple regression analysis showed that the initial visual acuity influenced the degree of improvement negatively. This means that those who had poor visual acuity improved much better than those with better visual acuity.

5. The results of logistic regression analysis showed that the odds of revisiting was not associated with patients characteristics such as age, sex.
