피판과 무피판 임플란트 수술 후 임플란트 주위 점막의 혈관분포 비교

임플란트 주위 점막의 혈관분포는 임플란트 식립 후 주위 조직의 치유와 유지에 중요한 영향을 미친다. 이에 본 연구는 피판과 무피판 임플란트 수술 후 임플란트 주위 점막의 혈관분포를 비교 평가하였다. 성견 6마리에서 하악 좌우측 소구치를 발거하고 치조골을 평탄하게 하였다. 3개월의 치유기간이 경과한 후 하악 양측에 피판과 무피판 임플란트 수술을 이용하여 2개의 임플란트를 하악 양측에 각각 식립하였다. 임플란트 식립한 뒤 3개월 후 성견을 희생하여 조직시편을 제작하였다. 조직형태학적 분석을 이용하여 임플란트 주변 500㎛ 범위의 결합조직에서 혈관의 수와 단위면적당 혈관 단면적의 비율을 계산하였다. 임플란트 주위 결합조직에서 무피판 임플란트 수술을 이용하여 임플란트를 식립한 경우 임플란트 주위 점막에 혈관이 더 많이 분포하였다. 이러한 결과는 무피판 임플란트 수술 방법이 피판 임플란트 수술 방법과 비교시 임플란트 주위 연조직의 혈관분포를 증가시키는데 효과가 있음을 보여 준다.

Background: The vascularity of the peri-implant tissue is a very important parameter in establishment and maintenance of a healthy tissue after dental implant insertion.

Objective: The purpose of this study was to compare the vascularity of the peri-implant mucosa between flap and flapless implant surgeries by using a canine mandible model.

Study Design: In six mongrel dogs, bilateral, edentulated, flat alveolar ridges were created in the mandible. After 3 months of healing, two implants were placed in each side by either the flap or flapless procedures. After another healing period of 3 months, biopsies were obtained, prepared for light microscopy and exposed to morphometric measurements.

Results: The supracrestal connective tissue lateral to the implant was found to be more richly vascularized in the flapless group than in the flap group.

Conclusion: These results suggest that the flapless procedure may have an effect on increasing the vascularity of the peri-implant mucosa.
