폐암 환자를 돌보는 가족원의 삶의 질에 영향을 미치는 요인

본 연구는 폐암 환자의 일차가족원, 이차가족원, 삼차가족원의 부담감과 삶의 질을 파악하고, 삶의 질에 영향을 미치는 요인을 찾아 폐암 환자 가족원의 부담감을 줄이고 삶의 질을 높일 수 있는 간호 중재를 위한 기초 자료를 마련하고자 시도된 상관관계 조사 연구이다.자료 수집은 2007년 11월 6일부터 2007년 11월 30일까지 서울 소재 3차 의료기관인 A병원 종양내과에서 폐암으로 진단 받은 환자를 돌보는 가족원을 대상으로 연구자가 가족원에게 질문지법을 통해 이루어졌다.연구도구는 폐암 환자의 일반적 특성과 질병적 특성, 가족원의 일반적 특성, 가족부담감 도구(서미혜, 오가실, 1993), Korean version of the Caregiver Quality of Life Index-Cancer(윤영호 외 9인, 2005)를 사용하였다. 수집된 자료는 SPSS Win 12.0 프로그램을 이용하여 전산처리하였으며, 기술통계, Pearson’s correlation, Stepwise multiple regression 등을 이용하여 분석하였으며 연구 결과는 다음과 같다.1. 대상자는 총 202명으로 남자 76명, 여자 126명이었다. 이 중 일차가족원은 115명, 이차가족원은 68명, 삼차가족원은 19명이었다.2. 대상자의 가족부담감은 5점 만점에 평균 2.69(±1.166)였고 최대 145점에서 평균 66.88(±15.195)으로 나타났다. 일차가족원의 가족부담감은 평균 70.40(±16.078)점이었고, 이차가족원은 평균 62.34(±12.917)점이었고, 삼차가족원은 평균 62.00 (±11.832)점으로 나타났다.3. 대상자의 삶의 질은 최대 140점 만점에 평균 70.46(±19.163)이었다. 일차가족원의 삶의 질은 평균 66.17(±19.030)이었고, 이차가족원의 삶의 질은 평균 75.90 (±18.179)이었으며, 삼차가족원의 삶의 질은 평균 78.38(±16.800)이었다.4. 대상자가 지각하는 가족부담감과 삶의 질의 관계는 강한 음의 상관관계(r=-0.737, p=.000)를 보였으며, 일차가족원(r=-0.752, p=.000), 이차가족원(r=-0.624, p=.000), 삼차가족원(r=-0.793, p=.000)이 모두 강한 음의 상관관계에 있었다.이상의 결과를 통하여 폐암 환자를 돌보는 일차, 이차, 삼차가족원 중에서 일차가족원의 가족부담감이 가장 높으며 삶의 질이 제일 낮은 것을 알 수 있었다. 특히 가족부담감은 가족원의 삶의 질에 영향을 미치는 중요한 요인으로 나타났다.임상실무에서 가족부담감을 줄일 수 있는 간호중재 프로그램의 개발로 가족원의 부담감을 낮추고 삶의 질을 향상시킬 수 있다면 가족원으로부터의 폐암 환자 간호의 질을 높일 수 있을 것으로 기대된다.

[영문]This is about investigating the burden and quality of life of primary, secondary, tertiary family caregivers of patients with lung cancer. It has been conducted, in order to provide the basic data for developing systematic and distinctive nursing intervention programmed for family caregivers of patients with lung cancer to reduce the burden and improve the quality of life.Data were collected from November 6 to November 30, 2007 by the researcher in university teaching hospital in Seoul.The measurement instruments are as follow :1) Family Caregiver Burden scale from Suh Mi Hae & Oh Ka Sil(1993).2) Korean version of the Caregiver Quality of Life Index-Cancer from YunYoung Ho et al.(2005).3) Demographic data of the patients and the caregivers are collected.The analysis for data were done by SPSS Win 12.0 programs, descriptive statistic, Pearson’s correlation coefficient, and Stepwise multiple regression are used. The results of this study were as follow :1. The subjects were consisted of 126 females and 76 males which enrolled 115 primary caregivers, 68 secondary caregivers and 19 tertiary caregivers.2. The mean score of family caregiver burden was 66.88. The mean score of primary caregivers was 70.40 and the mean score of secondary caregivers was 62.34 and the mean score of tertiary caregivers was 62.00.3. The mean score of family caregiver quality of life was 70.46. The mean score of primary caregiver quality of life was 66.17 and the mean score of secondary caregiver quality of life was 75.90 and the mean score of tertiary caregiver quality of life was 78.38.4. The data showed high negative correlations between quality of life of family caregivers and burden of family caregivers(r=-0.737).The results demonstrated that primary caregivers(r=-0.752) of patients with lung cancer experience much higher burden and much lower quality of life than the other caregivers(r=-0.624). It is imperative to develop nursing intervention which can reduce family caregiver's burden to enhance quality of life.
