저온 뇌손상 백서에서 아데노바이러스 매개 BDNF (Brain-Derived Neurotrophic Factor)와 골수 중간엽 줄기세포 이식이 신경학적 기능 회복에 미치는 영향

인간 중간엽 줄기세포는 다기능적이어서 골세포, 연골세포, 지방세포, 근육세포로 분화가 가능하다. 또한 줄기세포는 신경계 세포로 분화 가능하며, 뇌허혈 모델 백서에 인간 줄기세포를 주입하면 신경학적 기능이 호전된다고 보고되었다. 일부 보고자들은 뇌손상 백서에 골수 중간엽 줄기세포를 국소적으로 혹은 동맥, 정맥내에 투여하면 신경학적 기능을 향상시킬 수 있다고 보고하였다. 골수 중간엽 줄기세포는 신경영양인자를 분비, 유도하는 능력이 있다. 즉, BDNF (brain-derived neurotrophic factor), NT-3 (Neurotrophin-3), GDNF (Glial cell line-derived neurotrophic factor), NGF(nerve growth factor), VEGF(vascular endothelial growth factors), HGF(hepatocyte growth factor) 등과 같은 신경영양인자를 분비하는 것으로 보고되어왔다. 이런 신경영양인자들 중 BDNF는 신경세포의 성장 및 분화, 생존에 중요한 역할을 하는데, tyrosine kinase (Trk B) 수용체와 결합하여 신경계를 활성화시킴으로써 생물학적 효과를 나타낸다고 한다. 최근에는 신경영양인자 유전자를 줄기세포에 삽입하는 유전자 치료 방법이 개발되었다. 일부 저자들은 세포 내 cAMP를 증가시키는 조건하에서 인간 줄기세포가 초기 신경세포의 형태로 그 분화가 촉진된다고 보고하였다. 하지만, 이식된 줄기세포가 신경세포로 극히 일부만 분화하거나, 이식 받은 조직의 세포와 세포 융합, 면역 반응 등에 따른 세포 사멸로 장기간 생존하지 못한다는 결과가 최근 일부 연구자들에 의해 보고되어왔다.본 저자는 저온 뇌손상 모델 백서에 중간엽 줄기세포를 이식하면 뇌손상 모델의 신경학적 기능이 향상되고, 신경영양인자를 함께 이식하면 신경손상 회복이 더욱 향상될 것이라는 가설을 세웠다. 또한 아데노바이러스 이용한 유전자 기법으로 신경영양인자인 BDNF를 중간엽 줄기세포에 도입(transfection) 한 후 저온 뇌손상 백서에 이식하여, 신경학적 기능 회복에 미치는 영향을 조사하였고 BDNF 유전자 도입(transfection)이 효과적인 신경영양인자 투여 방법이며, 이식된 줄기세포가 신경세포로 분화되어 장기간 생존 가능한가에 초점을 맞추어 연구하였다.본 연구는 골수 중간엽 줄기세포만 이식한 군, 골수 중간엽 줄기세포에 forskolin를 처리한 군, 그리고 아데노바이러스 매개 BDNF를 도입(transfection) 한 골수 중간엽 줄기세포를 이식한 군으로 나누고 이들은 저온 뇌손상 후 7일 뒤 이식하였다. 신경학적 기능 검사는 rotarod 검사와 forepaw stepping 검사를 이용하였으며, 면역세포형광 염색을 시행하였다. 대조군에 비하여 골수 중간엽 줄기세포만 이식한 군, 골수 중간엽 줄기세포에 forskolin를 처리한 군, 그리고 아데노바이러스 매개 BDNF를 도입(transfection) 한 골수 중간엽 줄기세포를 이식한 군 모두 신경학적 기능이 향상되었다. 특히 아데노바이러스 매개 BDNF를 도입(transfection)한 중간엽 줄기세포 이식군에서 신경학적 기능 회복이 가장 좋은 결과를 보였다. 이식 10주 후, 면역세포 형광 염색에서 이식한 중간엽 줄기세포가 일부 신경계 세포로 분화되는 것을 관찰할 수 있었다. 이러한 결과들은 본 저자가 제시한 가설인 중간엽 줄기세포와 신경영양인자 BDNF를 뇌손상 모델 백서에 이식하면 신경학적 기능이 향상시킨다는 것을 확인하였으며, 아데노바이러스 매개체를 이용한 신경영양인자 투여방법이 효과적이다라는 것을 알았다. 또한 cAMP를 증가시켜 신경 전달계를 활성화시키는 forskolin를 중간엽 줄기세포에 처리 하면, 신경영양인자인 BDNF의 수용체 tyrosine kinase (TrkB)를 발현시켜 신경세포의 분화를 촉진시키는 중요한 역할을 한다는 것을 알았다. 이상의 결과로 보아 저온 뇌손상 백서에 골수 중간엽 줄기세포 이식, 신경영양인자 BDNF, 그리고 신경계 활성화 물질인 forskolin의 투여는 이식한 중간엽 줄기세포가 신경세포로 분화되는 것을 더욱 촉진했음을 알 수 있었다.결론적으로 아데노바이러스 매개 BDNF와 골수 중간엽 줄기세포의 이식이 저온 뇌손상 백서의 뇌기능을 향상시킨다고 말할 수 있다. 향후 적정한 조건을 제시하는 것이 필요하겠지만, 이 연구 결과는 앞으로 뇌손상 환자의 치료에 기여할 수 있으리라고 사료된다.

[영문]Human mesenchymal stem cells (hMSCs) are multipotent and they can differentiate into osteocytes, chondrocytes, tenocytes, adipocytes, and smooth muscle cells. Human MSC could also differentiate into neuronal cell types. It has been reported that the transplantation of MSC into local ischemic regions of the rat brain with experimentally induced stroke, reduced the functional deficits of them. Human MSC shows the reduction of the functional deficits after traumatic brain injury in rats. Both local, intra-arterial injection, and systemic intravenous administration of MSC promoted functional improvement in these animals. MSCs also have the ability to secrete and induce the expression of growth factors in parenchymal cells. Neurotrophic factors such as brain-derived neurotrophic factor (BDNF), neurotrophin-3 (NT-3), glial cell line-derived neurotrophic factor (GDNF), nerve growth factor (NGF), vascular endothelial growth factors (vEGF), and hepatocyte growth factor (HGF) were all reported to be produced by MSC. BDNF among these neurotrophic factors significantly promotes the survival and differentiation of neuronal tissue by acting on receptor kinases (tyrosine kinase; Trk B).Strategies are currently being developed for introducing growth factor genes into MSC. Some authors reported that the human MSCs may have been differentiated into early progenitors of neural cells under conditions that increase the intracellular level of cAMP. Recent studies showed that MSCs transplanted to the brain are rejected by an inflammatory response. Also some researchers reported that the graft volume had retracted and could not survive for long time.We hypothesized that BDNF (brain-derived neurotrophic factor) contributes to the maintenance of the therapeutic benefits of mesenchymal stem cells, and we examined the efficacy of human mesenchymal stem cells transfected with a adenovirus vector containing a BDNF gene. We investigated whether transplanted human MSCs could survive for long time.Human MSCs, forskolin(chemicals of elevation of intracellular aAMP)-treated hMSCs, and adenovirus-mediated BDNF-hMSCs (Ad-BDNF-hMSCs) were transplanted into the rats 7 days after giving cold injured trauma to them. Functional outcome was assessed with the rotarod test and forepaw stepping test, and the brain sections were stained by immunohistocytochemistry until 10 weeks after traumatic brain injury.Compared with the control sham group, Both hMSCs and Ad-BDNF-hMSCs elicited functional improvement, and the most effective cases was in the adenovirus-mediated BDNF-hMSCs (Ad-BDNF-hMSCs) group.Neuron-like morphology was shown by immunohistocytochemical staining in some cells of transplanted hMSCs at 10 weeks. These data support our hypothesis that BDNF contributes to neuroprotection in cerebral trauma rat model and cellular delivery of BDNF can be achieved by adenovirus vector. Thus, our results revealed BDNF plays a key role in the process of acquisition of neural differentiation in response to increase Trk B receptors through elevation of cAMP. Transplanted hMSCs regeneration can be enhanced by all combination of BDNF, human MSCs, and forskolin for 10 weeks.In conclusion, we identified that adenovirus vector-mediated BDNF and hMSCs transplantation partially promoted the neuron regeneration in rat with cold injured trauma. Although we need a further study to establish the optimal conditions for functional recovery, our study can serve a stepping-stone for the treatment of brain injury.
