Thiopental sodium 정주 하에 rocuronium의 작용 발현 시간의 단축을 시도할 때 천장 효과가 나타나는 용량

Rocuronium은 작용 발현시간이 빨라 신속한 기관 내 삽관을 위해 사용할 수 있는 근이완제이다. 그러나 succinylcholine에 미치지 못하여 더 빠른 작용 발현시간을 얻고자 고용량의 투여가 시도 된다. 하지만 용량을 증가시킴에 따라 작용 발현시간이 단축되지만 한계가 있어 더 이상의 작용 발현시간 단축을 보이지 않는 천장효과가 나타난다. 여러 연구에서 작용 발현시간에 대한 천장효과가 관찰되었지만 천장효과를 유발하는 용량에 대해서 정확히 밝혀진 것은 없다. 이에 본 연구에서는 rocuronium 투여 용량을 0.1 mg/kg 간격으로 하여 발현 시간의 천장효과를 유발하는 용량을 구하고자 하였다.전신마취 하 수술 받는 140명의 젊은 남자 환자를 rocuronium 용량 0.6 mg/kg부터 1.2 mg/kg까지 7개 용량으로 20명씩 나누고 thiopental sodium을 정주한 뒤 근가속기로 모지내전근을 감시하였다. 정주 완료 후 단일 연축이 0% 될 때까지를 작용 발현시간으로, 단일 연축이 10% 회복할 때까지의 시간을 회복시간으로 정의하였다.본 연구에서 발현시간은 1.0 mg/kg 이상을 투여할 때 더 이상의 단축을 보이지 않았고 회복 시간은 투여 용량에 정비례하여 연장되는 양상을 보였다. 그러나 각 용량간의 혈역학적 차이는 없었다.결론적으로 젊은 남자 환자에서 thiopental sodium의 정주 하에 rocuronium의 작용 발현시간 단축을 시도할 때 1.0 mg/kg에서 천장 효과가 나타나며 발현시간은 평균 67.9초까지 단축되었다. 선택한 용량 범위에서 회복시간은 정비례로 연장되었으나 심혈관계 영향은 차이가 없었다.

Background: Rocuronium is considered a good candidate for rapid-sequence induction of anesthesia. Increased dose of rocuronium shortens the onset time but prolongs the duration of action. However, the ceiling effect of onset time appears when larger doses are used. Clinical trials have not shown the exact dose of ceiling effect. We performed this study to find dose producing the ceiling effect of onset time.Methods: One hundred forty young male adults were randomised to one of seven doses of rocuronium: 0.6, 0.7, 0.8, 0.9, 1.0, 1.1 or 1.2 mg/kg. Anesthesia was induced with thiopental sodium and maintained with sevoflurane after tracheal intubation. Neuromuscular block was monitored with acceleromyography using single twitch. The onset time, 10% recovery time and also the changes in hemodynamics were checked.Results: A dose of up to 1.0 mg/kg shortens the onset time but no further decrement was seen with doses larger than 1.0 mg/kg. The recovery time was prolonged as doses were increased but there were no differences in the hemodynamics.Conclusion: This study showed that in young male adults under thiopental sodium anesthesia, the ceiling effect of onset time appeared with rocuronium doses in excess of 1.0 mg/kg.
