전립선비대증 환자에서 전립선염 동반유무가 수술 결과에 미치는 영향

최근 전립선비대증에 동반된 전립선염의 임상적 중요성이 강조되면서 이에 대한 여러 연구가 보고되었는데, 그 중 두 질환이 동반된 경우 전립선비대증에 대한 약물치료 후 전립선염이 동반된 군과 동반되지 않은 군 간에 하부요로증상의 개선 효과에 대한 연구는 있으나, 경요도전립선절제술 후 전립선염의 동반이 하부요로증상의 개선에 어떤 영향을 주는지에 대한 연구는 보고되지 않았다. 이에 전립선비대증 환자에서 전립선염의 동반이 경요도전립선절제술 후 하부요로증상 개선에 어떠한 영향을 주는지를 알아보고자 하였다.1996년 3월부터 2006년 12월까지 경요도전립선절제술을 시행받고 추적관찰이 가능하였던 237명의 환자들을 대상으로 수술 후 전립선 조직의 병리결과에 따라 전립선염군과 비전립선염군으로 분류하였으며, 수술 전 후의 국제전립선증상점수, 삶의 질 지수, 요속의 변화, 합병증 유무를 조사 분석하였다.전립선염군과 비전립선염군의 경요도전립선절제술 후 최고요속 및 합병증 동반률은 통계학적으로 유의한 차이를 보이지 않았으나(p>0.05), 국제전립선증상점수 및 삶의 질 지수의 변화는 유의한 차이를 보였다(p<0.05).즉, 임상적으로 전립선염군은 비전립선염군에 비해 객관적 지표인 최고요속은 차이가 나지 않았으나, 주관적 지표인 국제전립선증상점수 및 삶의 질 지수가 유의하게 낮았다. 따라서 전립선염은 경요도전립선절제술의 하부요로증상 개선효과를 저해시키는 것으로 생각되며, 수술 전 전립선염이 진단된 경우에 전립선염에 대한 치료를 먼저 시행하고, 수술 후 조직검사에서 전립선염이 진단된 경우에도 전립선염에 대한 치료를 시행하면 환자의 술 후 만족도를 더욱 증가시킬 수 있을 것으로 판단된다.

Benign prostatic hyperplasia (BPH) and prostatitis may present similar clinical manifestations, although the severity of their symptoms is different.From march 1993 to december 2006, 237 patients with symptomatic BPH, who had undergone TURP by one surgeon, were enrolled in this study.Among total of 237 patients, 82 patients (34%) had prostatitis and 155 patients (66%) were free of prostatitis in pathologic diagnosis after TURP. We investigated the differences between BPH patients with prostatitis and those without prostatitis after TURP through International Prostate Symptom Score (IPSS), maximal urine flow rate (Qmax) and Quality of Life scale (QoL). Statistical analysis was performed using the independent t test and paired t test. Prostate volume, PSA, Qmax, complications have no differences between two groups (p>0.05), but IPSS and QoL have a significance differences between two groups (p<0.05).So BPH patients with prostatitis who are planning to transurethral prostate surgery should be treated before or after surgery in order to improve their satisfaction after surgery.
