병원 정보간호사의 직무 표준 개발을 위한 직무 수행 및 직무 중요도에 관한 연구

본 연구는 국내 병원 정보간호사의 직무 표준 개발을 위하여 병원 정보간호사로 업무를 수행하고 있는 간호사의 특성, 직무 수행 정도 및 직무 중요도를 파악하기 위한 조사연구이다. 병원 정보간호사 운영현황과 전산부서 부서장, 간호부서 부서장, 정보간호사, 전문가집단이 인식하는 정보간호사 직무 중요도를 조사하였고, 정보간호사를 대상으로 직무 수행 정도를 조사하였다. 전국 300병상이상 병원을 대상으로 200부의 설문지를 우편 발송하였고(회수율 17.5%), 자료 수집은 2007년 5월 25일부터 6월 20일까지 실시하였다. 자료 분석은 SPSS 통계프로그램을 이용하였다.외국 문헌과 전문가집단의 타당도 검증을 통해 정보간호사의 직무를 관리영역, 분석영역, 정책이행영역, 자문, 조정, 촉진, 통합영역, 필요역량개발영역, 교육영역, 연구영역 등 총 7개 영역 49개 항목으로 확정하였다.본 연구결과를 요약하면 다음과 같다.첫째로, 설문에 응답한 35개 병원 중 18개의 병원에서 정보간호사 제도를 운영하고 있었으며, 전임으로 운영하는 병원이 10개 병원, 겸임으로 운영하는 병원이 8개 병원이었다. 병원의 특징 및 규모와 정보간호사 운영 유무와의 관련성은 통계적으로 유의하였다. 둘째로, EMR을 사용하는 병원과 CPOE만 사용하는 병원사이의 정보간호사의 직무수행정도의 차이는 통계적으로 유의하였다. 또한, 정보간호사의 운영 형태와 정보간호사의 직무 수행 정도와의 관련성은 통계적으로 유의하였다. 셋째로, 정보간호사의 간호정보관련 인지도는 35.2%, 지식 습득도는 26.0%, 교육 요구도는 68.8%이고, 특히 정보시스템 영역의 교육요구도가 높았다. 넷째로, 정보간호사의 직무 중요도 점수 평균은 4.04±0.57점으로 7개 영역 모두 높은 점수를 나타냈는데 자문, 조정, 촉진, 통합영역 4.17±0.60점, 관리영역 4.09±0.59점, 분석영역 4.04±0.72점 순이었다. 다섯째, 연구영역을 제외한 6개 직무영역에서 정보간호사의 직무 수행 정도와 직무 중요도와는 양의 상관관계를 보였다. 마지막으로 정보간호사의 직무 중 가장 중요도가 높은 영역은 자문, 조정, 촉진, 통합영역이었으며, 실제로 정보간호사의 직무 수행 정도가 높은 영역도 자문, 조정, 촉진, 통합영역의 업무였다. 조사된 정보간호사의 직무 수행 정도와 직무 중요도를 토대로 국내 병원 정보간호사의 직무 표준 개발이 필요하리라 본다.또한 정보간호사의 직무를 수행하기 위한 교육 요구도는 높게 나타났으나 현재 국내에 개발되어 운영되는 간호정보 전문교육 과정이 없으므로 간호협회나 병원간호사회 차원의 간호정보 전문교육 과정이 필요하리라 본다. 이 연구의 제한점은 낮은 회수율로 일반화 하는데 어려움이 있으므로 향후 더 많은 병원을 대상으로 하는 연구가 필요하리라 본다.

[영문]The purpose of this study is to development of job standards of Hospital Informatics Nurses in Korea. This study was conducted to analyzed the general characteristics and roles of Informatics Nurse. Survey of the current status of nursing information systems in hospitals and the importance of Hospital Informatics nurses' roles as well as their current roles were conducted for the managers of information centers, managers of nursing departments, Informatics nurses, and the nursing informatics specialist group. Data were collected from 35 out of 200 hospitals with over 300 beds (response rate of 17.5%) from May 25, 2007 to June 20, 2007. They were analyzed by using SPSS program.Total of 49 nursing roles were categorized into seven domains based on the literature and validated by the specialist group. They were: administration, analysis, compliance and integrity management, coordination, consultation, facilitation and integration, development, educational and professional development, and research.The results were as follows;First, only 18 out of 35 hospitals were implementing nursing information systems. Roles of Informatics Nurse were exclusively nursing informatics for 10 hospitals and not exclusive for the remaining 8 hospitals. These were significantly related with the size of hospitals. Second, the degree of informatics work performance for Informatics Nurses were significantly better for hospitals with EMR (electronic medical records) than those with CPOE(computerized physician entry system) only. Third, degree of nurse’s perception of nursing informatics was 35.2%, acquisition of knowledge was 26%, and educational demand was 68.8%. Especially, educational demand on the informatics was very high. Fourth, average score of the importance of nursing informatics role was 4.04±0.59, the domain of coordination, consultation, facilitation and integration was 4.17±0.60, administration was 4.09±0.59, and anaylsis was 4.04±0.72. Fifth, the degree of work performance and these importance scores were positively correlated for all domains except research. Finally, important role domains were consultation, coordination, facilitation and integration. And these were also areas of high performance for informatics nurses.We also found that demand for better education was high. However, professional education program about nursing informatics is inadequate and therefore there is a need for the professional education program organized by the Korean Nurses Association or Hospital Nurses Association. The findings of this study is limited by a low response rate and therefore the future study include more hospitals.
