골격성 I급 부정교합자의 수완부 골성숙도와 신장에 관한 누년적 연구

성장기 환자의 교정치료에서 각 개인의 성장을 평가하는 것은 매우 중요하며 이를 위해 발육연령을 이용하는 것이 바람직하다. 발육연령을 평가하는 방법에는 성적 성숙도, 치아의 성숙도, 신체의 성숙도, 골성숙도를 이용하는 것이 있는데 이 중 신체의 성숙도와 골성숙도는 교정 영역에서 널리 이용되는 방법이다. 국내에서의 성장에 관한 연구는 골성숙도에 대한 연구가 주를 이루고 있어 임상적으로 성장 단계에 대한 평가는 가능하였으나 잔여성장량에 대한 평가는 불가능하였다. 이에 본 연구에서는 수완부 골성숙 단계에 따른 신장의 계측치를 평가하여 특정 시기의 성장 속도와 잔여성장량에 대한 평가를 가능하게 하고자 한다.연세대학교 영동세브란스치과병원 치과 교정과에 내원한 I급 골격관계를 갖는 대상자 중 약 6개월 간격으로 수완부골방사선사진을 촬영하고 신장을 측정한 남자 22명, 여자 22명의 수완부 골성숙도와 신장에 관한 누년적 연구를 시행하였다. 수완부 골성숙도는 Fishman (1982)의 Skeletal Maturity Indicators(SMI)를 이용하여 평가하였으며 이에 따른 신장의 계측치에 대한 평가를 시행하여 다음과 같은 결론을 얻었다.1. SMI 단계별 남, 녀의 평균 신장을 구하였으며 성장완료 시점인 SMI 11 단계에서의 평균 신장은 남자는 174.3±6.6cm, 여자는 162.0±5.9cm로 나타났다.2. 남, 녀에서 신장의 SMI 단계별 성장완료율과 잔여성장량의 평균과 표준편차를 구하였다.3. 남, 녀에서 신장의 SMI 단계별 성장량의 평균과 표준편차를 구하였다.4. 신장의 SMI 단계별 성장 속도(growth velocity)는 남, 녀 모두에서 SMI 4-7 단계에서 큰 성장 속도를 보였다.본 연구 결과 수완부 골성숙도를 이용한 신장의 성장완료율과 잔여성장량의 평가가 가능하였다.

[영문]Growth evaluation is of much importance in patients receiving orthodontic treatment and evaluation of developmental age is the preferred method. Developmental age is evaluated by methods of sexual, dental, physical, and skeletal maturity. Out of these methods, physical and skeletal maturity is widely used in orthodontics. Skeletal maturity has been mainly used in domestic studies and clinical evaluation of growth stages has been possible. However, evaluation of remaining growth has been unachievable. This study evaluates height measurement according to skeletal maturity stage of hand- wrist and enables the evaluation of growth rate and remaining growth per specific period.22 male and 22 female Skeletal Class I patients of Department of Orthodontics, Yongdong Severance Dental Hospital, Yonsei University were evaluated longitudinally by height measurements and radiographs of hand-wrist taken by a 6 month interval. Skeletal maturation of hand-wrist were evaluated by Fishman's Skeletal Maturity Indicators(SMI) and height measurements were evaluated accordingly. The following results were observed.1. Average height of male and female for each SMI stage was obtained and it was 174.3±6.6 cm for males and 162.0±5.9 cm for females at stage 11 which is the end of growth.2. Average and standard deviation of growth completion rate and remaining growth amount of height for both sexes were evaluated per each SMI stage.3. Average and standard deviation of growth amount of height for both sexes were evaluated per each SMI stage.4. Growth velocity of height per SMI stage was high for both sexes in stages 4-7.This research enables the evaluation of growth completion rate and remaining growth amount of height by evaluating skeletal maturity of hand-wrist radiographs.
