[한글]

연구 목적: 지역사회에 거주하는 60세 이상 노인을 대상으로 머리둘레와 시공간능력의 연관성을 확인하고자 한다.연구 방법: 지역사회에 거주하는 60세 이상 노인 463명을 대상으로 사회인구학적 특성, 과거 및 현재 질병력, 약물력, 가족력, 혈액검사, 머리둘레를 비롯한 신체 측정, ApoE 유전자형 검사, 기초 인지 선별 검사 (K-MMSE, Korean version-Mini Mental State Examination) 및 신경심리검사(SNSB-D, Seoul Neuropsychological Screening Battery-dementia version)를 시행하였다.결과: 상관분석 결과 머리둘레가 증가할수록 주의 집중 능력, 언어 기능, 시공간적 지각 및 구성 능력, 기억력과 전두엽 기능의 소검사 점수가 전반적으로 유의하게 증가하는 경향을 보였다. 머리둘레를 4분위 집단으로 나누어 분산분석한 결과는 시공간 능력만 4분위 집단별 통계적으로 유의한 차이를 보였으며, 나이, 성별, 키, 교육수준, 흡연 및 음주 여부, ApoE 유전자형 등 인지기능에 영향을 줄 수 있는 혼란변수를 보정한 회귀분석 결과 시공간 능력 중 시공간 기억력 점수만 머리둘레가 작아질수록 낮아지는 경향을 보였다. (β=0.18, p=0.002, β=0.15, p=0.01) ApoE ε4 유전자형유무는 머리둘레와 통계적으로 유의한 관계가 없었으며, 인지 기능 저하와도 유의한 상관 관계를 보이지 않았다.결론: 뇌 예비력의 중요한 관련 변수인 머리 둘레가 커질수록 대부분 인지영역별 기능은 증가하는 경향을 보였으나, 나이, 성별, 키, 교육수준, 현재 흡연 및 음주 여부, ApoE ε4 유전자형 유무를 보정하고 나면 시공간 기억력만 머리둘레와 연관성을 보였다. 이는 시공간 기억력이 뇌 예비력의 차이를 민감하게 반영하는 인지기능 중 하나일 가능성을 제시한다고 할 수 있겠다.



[영문]Objectives: The aim of this study is to find out the correlation between head circumference and visuospatial function of the elderly in community.Methods: A total of 463 subjects (170 men and 293 women) aged 60-91 years were analyzed from preliminary data of GDEMCIS (Gwangju Dementia and MCI Study). The study questionnaire consisted of demographic characteristics, current and past illness history, drug history, K-MMSE, SNSB-D. We also measured head circumference and examined ApoE genotype.Results: On regression analysis, as head circumference become smaller, visuospatial memory scores(S-Rey immediate recall & delayed recall) decreased adjusted by age, height, sex, educational level, ApoE ε4 allele, current smoking and alcohol drinking (β=0.18, p=0.002, β=0.15, p=0.01). Presence of ApoE ε4 allele has no direct correlation with head circumference and cognitive decline.Conclusion: As head circumference become smaller, only visuospatial memory scores decreased significantly. These results suggest that visuospatial memory function might be one of the sensitive markers of brain reserve differences.
