Abstract
[한글]본 연구에서는 일산 소재의 한 종합병원 건강검진센터 수진자를 대상으로 백혈구 수와 대사증후군과의 관련성을 조사하는 것이다. 사회적 환경인자인 흡연, 음주, 운동, 교육수준, 월수입 등의 변인과 대사증후군과의 관련성, 백혈구 감별계산과 대사증후군과의 관련성 연구를 시행하여, 백혈구 수가 증가하고 감소함에 따라서 대사증후군 발생 위험도를 밝히고 대사증후군 발병을 예측하는데 도움을 줄 수 있는지 알아보고자 한다.연구대상은 2004년 9월 1일부터 2005년 12월 30일까지 일산 소재 종합병원 한 기관의 건강검진센터를 방문한 수진자 12,510명 중 30세 이상의 성인 8,517명(남성 5,117명, 여성 3,040명)을 대상으로 하였다. 신체 계측치(신장, 체중, 혈압), 혈액검사(중성지방, HDL-콜레스테롤, 공복시 혈당, 백혈구 수)의 수치가 누락되거나 설문지를 작성하지 않은 자, 급성 질환자는 연구에서 제외하였다. 혈압 약, 당뇨 약을 복용하는 고혈압, 당뇨자의 경우는 연구 대상자로 포함시켰다.연구방법은 설문지를 통하여 음주, 흡연, 운동, 교육수준, 월수입 등을 조사하였고, 혈액검사를 실시하여 중성지방, HDL-콜레스테롤, 공복시 혈당, 백혈구 수 등을 측정하여 대사증후군 관련 요인을 살폈다. 신체계측으로는 체질량지수, 신장, 체중, 혈압 등을 측정하였다.통계처리는 SAS 8.02를 이용하여 분석하였으며, 대사증후군 유병률(有病率, prevalence rate)은 Chi-sqaure검정을 시행하였고, 백혈구 수 및 백혈구 감별계산 과 대사증후군의 관련성은 나이, 흡연 등을 보정한 후 로지스틱(Logistics) 회귀분석을 시행하였다.연구결과는 다음과 같다.첫째, 대사증후군의 유병률은 남성이 24.55%, 여성은 11.38%로 나타났으며, 연령별 유병률은 남성은 50대에서, 여성은 60세 이상에서 가장 높게 나타났다. 그리고 백혈구 수가 증가할수록 남성과 여성 모두에게 대사증후군의 유병률이 높게 나타났다.둘째, 백혈구 수가 증가됨에 따른 대사증후군 관련 요소 분석결과는 남성의 경우 정상인의 위험도를 1.0으로 했을 때, 중성지방의 교차비가 1.977(1.770-2.210), 체질량지수 1.396(1.257-1.551), 고혈압 1.353(1.218-1.504), 공복시 혈당 1.196(1.017-1.408)의 순으로 나타났다 (P<0.05).여성의 경우는 남성의 경우와 다른 양상을 보였는데, 공복시 혈당의 교차비가 2.080(1.564-2.767), 중성지방 1.800(1.465-2.212), 체질량지수 1.538(1.317-1.798), HDL-콜레스테롤 1.413(1.185-1.686), 고혈압 1.272(1.085-1.491)의 순으로 나타났으며, 증가된 백혈구 수 (≧6.8×103/ul)와 공복시 혈당이 가장 높은 위험도를 보였다.(P<0.05)셋째, 대사증후군과 백혈구 수와의 로지스틱(Logistics) 회귀분석의 결과에서는 남성과 여성 모두 백혈구 수가 증가할수록 대사증후군의 위험도가 높았다.넷째, 대사증후군과 백혈구 감별계산과의 로지스틱(Logistics) 회귀분석의 결과 먼저, 연령과 흡연을 보정한 후의 결과는 남성은 호중구 (Segmented neutrophil), 림프구 (Lymphocyte), 단핵백혈구 (Monocyte), 호산성백혈구 (Eosinophil), 호염기성백혈구(Basophil) 수가 증가할수록 대사증후군과 유의한 관련성이 있는 것으로 나타났고, 여성의 경우는 호중구 (Segmented neutrophil), 림프구 (Lymphocyte)는 수가 증가할수록 대사증후군의 위험도가 증가하고, 단핵백혈구(Monocyte), 호산성백혈구 (Eosinophil), 호염기성백혈구 (Basophil)는 그 수가 증가할수록 대사증후군의 위험도가 낮아지는 것으로 나타났다. 백혈구 감별계산은 총 백혈구 수에 영향을 받으므로 연령과 흡연이외에 총 백혈구 수를 보정한 후 로지스틱 회귀분석한 결과를 보면, 남, 여 모두 림프구 (Lymphocyte) 수가 증가 할수록 대사증후군의 위험도가 증가하는 결과를 보였다.결론적으로 백혈구 수가 증가할수록 대사증후군의 위험도가 증가하였다.

[영문]Increased white blood cell counts have been associated with an inflammatory condition.Recently, many studies have shown that obesity and the metabolic syndrome is associated with systemic inflammation. This study was attempted to illustrate the relationship between white blood cell counts and the metabolic syndrome among korean.This study included 8,517 adults (5,117 males, 3,040 females) in their thirties were chosen out of 12,510 people who visited the medical checkup center placed in Ilsan, Kyunggido, from September 1, 2004 to December 30, 2005. Index of blood test and physical checkup were performed on the study such as triglyceride, HDL-cholesterol, fasting sugar and white blood cell counts. Logistic regression analysis was used to evaluate the association between White blood cell counts, White blood cell differential counts and metabolic syndrome. with an adjustment age and smoking status.The results of this study: first, there was a prevalence rate of metabolic syndrome, 24.55% among males and 11.38% among females, and was particularly high among males in their 50s. The increase in white blood cell counts led to high prevalence of metabolic syndrome for both males and females.Secondly, as white blood cell counts increased, the values of body mass index and cardiovascular risk factors were increased significantly. The odds ratio for elevated white blood cell counts increased significantly in the subjects with each components of the metabolic syndrome compared to the subjects without them. The lymphocyte counts in the White blood cell differential counts were higher in patients with metabolic syndrome than in those without.High level of white blood cell counts in normal range can be used as indicater in chronic inflammation.Increased white blood cell counts were significantly associated with metabolic syndrome among korean.
