전립선 비대증 환자에서 경요도 전립선 절제술 후 전립선 증상점수 변화율 적용의 임상적 유용성

Abstract

[한글]경요도 전립선 절제술(TURP)을 시행한 후 치료성적을 알아보기 위해 시행되는 검사로 전립선용적측정, 요속검사와 국제 전립선 증상점수(IPSS)가 있다. 요속검사는 객관적이라는 장점이 있으나 환자가 배뇨할 때까지 장시간 대기해야 하는 번거로움이 수반되며 국제 전립선 증상점수는 환자 개개인의 지적능력과 문맹여부에 따라 오차가 생길 수 있다. 이에 경요도 전립선 절제술 전 및 후의 요속검사와 국제전립선 증상점수 변화정도를 이용하여 단일회 시행된 국제 전립선 증상점수의 신빙성을 검증하는 동시 국제 전립선 증상점수 보정값의 유용성을 알아보고자 하였다. 1998년 7월부터 2003년 3월까지 전립선비대증을 진단 받고 경요도 전립선 절제술을 시행 받은 437명의 환자 중 경요도 전립선 절제술 후 국제 전립선 증상점수, 생활의 질 및 요속검사에 대해 추적관찰이 가능한 360명의 환자 (50세-84세, 평균 70±8.4세)를 대상으로 하였다. 요속검사 및 국제 전립선 증상점수의 변화정도는 다음과 같은 수식으로 규정하였고 이 값은 해당 검사의 보정값 으로 정하였다.요류검사변화정도 (변화율) =(수술 후 최대요속-수술 전 최대요속)/수술 전 최대요속증상점수변화정도 (변화율) =(수술 전 증상점수-수술 후 증상점수)/수술 전 증상점수수술 전 평균 전립선 용적은 46±24ml, 평균최대요속은 8.2±5.4ml/sec, 평균 국제 전립선 증상점수는 26±6.8점, 수술 후 평균 전립선 용적은 33±25ml, 평균 최대요속은 14.6±6.8ml/sec, 평균 국제 전립선 증상점수는 10±6.2점이었다. 수술 전 요속검사와 증상점수사이의 상관관계를 비교해보면 통계학적으로 유의할 만한 차이는 관찰되지 않았고, 수술 후 요속검사와 증상점수사이의 상관관계 역시 유의할 만한 차이가 관찰되지 않았다. 요속검사와 증상점수의 변화정도사이에는 유의한 상관관계가 관찰되었다. 회귀분석을 통해 요속검사와 증상점수의 변화정도사이의 함수관계는 y=0.687+1.419x로, 요속검사와 증상점수의 변화정도는 정비례로 관찰되었다.국제 전립선 증상 점수와 최대요속측정은 경요도 전립선 절제술 후 환자의 증상 호전 및 악화여부를 측정하는 좋은 지표이지만 국제 전립선 증상점수의 주관성 및 환자의 이해도에 따라 오차가 생기며, 보정된 국제 전립선 증상점수는 요속검사에 상응한 정확성을 얻을 수 있었다.

[영문]Purpose: Currently uroflowmetry (UFR) and international prostate symptom scores (IPSS) are most widely used studies to evaluate the results of transurethral resection of the prostate (TURP) for patients with benign prostate hyperplasia (BPH) . However, UFR is sometimes flawed by cumbersome task of waiting for patient to urinate and IPSS itself has some difficulties due to wide variation of intellectual ability and literacy level of each individual. Therefore, we performed this study to verify the accuracy of a single IPSS before and after TURP and analyze the clinical efficacy of the change rate of IPSS.Method: Of 437 patients who were diagnosed with BPH and underwent TURP from July 1998 to March 2003, 360 patients followed by traceable postoperative IPSS, quality of life, and UFR were selected. A correlation analysis was done to evaluate the relationship between preoperative UFR and IPSS and also between postoperative UFR and IPSS respectively. And regression analysis was done between the change rate of UFR and symptom scores. The changes in UFR and symptom scores were defined by the formulas as below.Change rate of UFR = (postoperative Qmax preoperative Qmax) /preoperative QmaxChange rate of symptom scores = (preoperative symptom scores - postoperative symptom scores) /preoperative symptom scoresResult: Average age of patients was 70±8.4, pre- and post-operative prostate volume was 46±24cc and 33±25cc respectively, and pre- and post-operative IPSS was 26±6.8 and 10±6.2 respectively. The correlation between preoperative UFR and symptom scores had no statistical significance and also there was no correlation between postoperative UFR and symptom scores (p>0.05) , but the correlation between the changes in UFR and symptom scores were statistically significant (p<0,05) . The correlation formulas of these two variables using regression analysis was y=0.687+1.419x, indicating a direct proportion between UFR and symptom scores (y = change rate of UFR, x = change rate of symptom scores) .Conclusion: This result suggests that IPSS can be a good evaluated method to assess the clinical result for patients with BPH after TURP when both pre- and post-operative symptom scores are used simultaneously although single symptom score itself has a low objectivity.
