직업성 질환 관리를 위한 CDSS 개발 : 소음성 난청을 중심으로

Abstract

[한글]2000년에서 2003년까지 노동부에서 조사한 직업성 질환의 발생빈도를 보면 진폐, 소음성난청, 중독 등 전통적인 직업병 이외에 뇌심혈관계 질환, 근골격계 질환 등 새로운 작업성 질환이 등장하고 그 비중도 급속하게 증가하고 있어 산업보건 분야에서 직업성질환관리는 향후 주요한 중심과제가 될 것이다.이 연구는 직업성 질환관리를 위한 의학적 의사결정지원시스템을 개발하기 위한 것으로 우선 대상 질환으로 직업성 질환 중 유병률이 높고, 순음청력검사를 통해 비교적 진단이 명확한 소음성 난청을 선정하였다.진단과 산업의학적 관리구분을 위한 지식획득은 산업의학 전문의와 이비인후과 전문의의 경험과 지식, 관련 문헌들을 분석하였고, 이를 통해 획득한 지식을 규칙으로 표현하였다. 시스템에 의한 진단 예측도는 요관찰자와 유소견자의 감별진단은 86.5%로 나타났고, 산업의학적 관리구분에 대한 예측도는 66.7%로 다소 낮게 나타났다. 관리구분에 대한 예측도가 낮은 것은 직업성질환으로 판정되는 경우 보상과 연관되어 있어 질환이 다소 저평가되는 판정에 대한 문제점으로 이미 다른 연구에서 제기한 것과 같은 양상을 보였다. 이는 추후 판정기준을 통합하여 표준화시켜 나간다면 보완이 가능할 것이다. 소음성난청 CDSS는 향후 다른 직업성 질환에도 적용이 가능하며 효율적인 보건관리를 지원하여 근로자의 건강증진 사업에 조력자가 될 것이다.

[영문]Ministry of Labor reported that the new type of occupational diseases like cardiovascular system, musculoskeletal system are appeared and increased its portion relative to traditional diseases like pneomoconiosis, noise-induced hearing loss during 2000 - 2003. It means the new occupational disease management in industrial healthcare area are necessary.This study has a purpose to develop a clinical decision support system on noise-induced hearing loss that have relatively high frequency and clear decision criteria. Knowledge acquisitions for evaluation and classification are based on field experience, knowledge of occupational medical specialist, otorhinolaryngologic medical specialist and literature. The obtained knowledges are expressed to rule with Visual Basic 6.0.Degree of evaluation prediction by developed system is 86.5% for a person who need observation and a person who have a symptome, and 66.7% in case of industrial medical management criteria. The reason of low prediction rate in case of industrial medical management criteria is related to compensation to patient, inducing under-evaluation. Noise-induced hearing loss CDSS is applicable to other occupational disease, and can support for efficient health case management.
