나선형 전산화단층촬영 조건이 얼굴 연조직 두께 계측에 미치는 영향

dc.description.abstract[한글] 본 연구는 임상적으로 사용되는 다양한 나선형 전산화단층촬영 조건이 3차원 전산화단층영상을 이용한 얼굴 연조직 두께 계측에 미치는 영향을 평가해 보고자 하였다.얼굴 연조직 부위에 특별한 결손부위나 손상이 없는 시신 3구를 대상으로 연구하였다. 각각의 시신은 다음의 8가지 조건하에서 전산화단층촬영을 시행하였다 :1) Protocol 1 : 3 mm 상층두께와 3 mm/sec table speed2) Protocol 2 : 3 mm 상층두께와 5 mm/sec table speed3) Protocol 3 : 5 mm 상층두께와 5 mm/sec table speed4) Protocol 4 : 5 mm 상층두께와 7 mm/sec table speed5) Protocol 5 : 5 mm 상층두께와 10 mm/sec table speed6) Protocol 6 : 7 mm 상층두께와 7 mm/sec table speed7) Protocol 7 : 7 mm 상층두께와 10 mm/sec table speed8) Protocol 8 : 7 mm 상층두께와 15 mm/sec table speed촬영된 데이터는 동일한 상층두께와 상층두께의 절반인 1.5, 2.5, 3.5 mm 영상 재구성 간격의 횡단면 영상으로 재구성하였고 개인용 컴퓨터로 전송하였다. 전송된 횡단면 영상으로 3차원 영상을 재구성하였고 연조직 계측 프로그램을 이용하여 각각의 시신에서 30개 부위의 얼굴 연조직 두께를 계측하였고 한명의 관찰자가 10회 반복 계측하였다. 계측한 부위와 동일한 부위를 직접 계측하기 위해서 시신을 절단하였고 절단면상에서 동일한 관찰자가 같은 부위를 10회 반복 계측하였다. 연조직 두께 계측치들은 repeated measure analysis of variance (ANOVA)를 이용하여 비교 분석한 결과 다음과 같은 결과를 얻었다.1. 10회의 시행동안 실제 얼굴 연조직 두께 계측치와 3차원 영상을 이용한 얼굴 연조직 두께 계측치 모두 관찰자내 차이는 통계적으로 유의할만한 차이가 존재하지 않았다(p>0.05).2. 각각의 시신에서 실제 연조직 두께 계측치와 8가지 3차원 전산화단층영상을 이용한 얼굴 연조직 두께 계측치를 비교 분석한 결과 protocol 8로 촬영하여 얻은 3차원 영상을 이용한 얼굴 실제 얼굴 연조직 두께 계측을 제외하고는 모두 통계학적으로 유의성 있는 차이는 없었다(p>0.05).3. 각각의 시신에서 실제 얼굴 연조직 두께 계측치와 3차원 전산화단층영상을 이용한 얼굴 연조직 두께 계측치와의 차이의 평균값이 0.5 mm 이상을 보이는 부위는 protocol 1, 2, 3, 4에서는 총 8부위 이하 protocol 5, 6, 7, 8에서는 총 20부위 이상에서 나타났다.결론적으로 정확한 얼굴 연조직 두께 계측을 위해서는 상층두께는 5 mm 이하, table speed는 7 mm/sec 이하로 촬영된 전산화단층영상이 유용하며 이런 촬영 조건은 임상에서 일반적으로 사용하고 있어 기존에 촬영된 3차원 전산화단층영상을 이용하여 얼굴 연조직 두께를 계측하는 것이 가능하리라 사료된다. [영문]The purpose of this study was to evaluate effect on measurement of facial soft tissue thickness using 3D computed tomographic images by various scanning protocols of spiral computed tomography.Three cadavers that had no particular loss or damages to their facial soft tissues was chosen for the study. The cadaver was scanned with a Helical CT under eight conditions :1) Protocol 1 : 3 mm slice thickness and 3 mm/sec table speed2) Protocol 2 : 3 mm slice thickness and 5 mm/sec table speed3) Protocol 3 : 5 mm slice thickness and 5 mm/sec table speed4) Protocol 4 : 5 mm slice thickness and 7 mm/sec table speed5) Protocol 5 : 5 mm slice thickness and 10 mm/sec table speed6) Protocol 6 : 7 mm slice thickness and 7 mm/sec table speed7) Protocol 7 : 7 mm slice thickness and 10 mm/sec table speed8) Protocol 8 : 7 mm slice thickness and 15 mm/sec table speedThe acquired data were reconstructed with 3, 5, 7 mm slice thickness and 1.5, 2.5, 3.5 mm reconstruction interval respectively and the images were transferred to a personal computer. Using a program developed to measure facial soft tissue thickness in 3D image, one observer measured the facial soft tissue thickness at the 30 sites a cadaver ten times. The cadavers were cut with bone cutter for direct measurement and then the same observer measured the facial soft tissue thickness ten times. Considering the ten-time repetition, a repeated measure analysis of variance (ANOVA) was adopted to compare and analyze the measurements. The results were as follows :1. There were no statistically significant intraobserver differences between the actual measurements of thickness of the facial soft tissues and the measurements using the 3D images (p>0.05).2. There were no statistically significant differences between the actual facial soft tissue thickness measurements and those using the 3D images except Protocol 8 in each cadaver (p>0.05).3. The points showing mean error over 0.5 mm between actual measurements and those using 3D images was 8 points or less in Protocol 1, 2, 3, 4 but 20 points or more in Protocol 5, 6, 7, 8.Conclusively, for more accurate measurement of faical soft tissue thickness, scanning protocols with slice thickness less than 5 mm, and table speed less than 7 mm/sec are recommended. And generally clinical spiral computed tomography is satisfied with this scanning protocol, so previous data acquired by the spiral computed tomography will be useful for measurement of facial soft tissue thickness.-
