소구치 발치를 동반한 교정치료가 하악 제3대구치 맹출에 미치는 영향

Abstract

[한글]소구치 발치를 동반한 교정치료가 발육중인 하악 제3대구치의 치축경사도(angulation)와 맹출에 어떠한 영향을 주는지 살펴보기 위해 소구치 발치 환자집단과 비발치 환자집단을 비교하였다. 소구치 발치 집단은 교정치료 후 대구치 전방이동량에 따라 두 집단으로 나누어 비발치 집단과 비교하였다. 세 집단 간의 교정 치료전후 하악 제3대구치 치축경사도의 차이를 비교해 보고, 유지기간 후 세 집단의 하악 제3대구치 맹출 여부를 비교하고 매복이 일어난 치아와 맹출이 일어난 치아의 비교를 통해 다음과 같은 결과를 얻었다.1. 비발치 집단에 비해 소구치 발치 집단은 발육중인 하악 제3대구치의 치축경사도의 개선량이 더 컸으며 유의성 있는 차이가 있었다(p<0.05,ANOVA). 그 차이는 두 발치 집단 중 대구치의 근심 이동량이 많은 집단과 비발치 집단 간에 유의성 있게 존재하였다(p<0.05, Scheffe's test).2. 세 집단의 유지 후 하악 제3대구치의 맹출 및 매복 여부 조사에서 세 집단 간에 매복률의 차이가 있었으며, 소구치 발치 집단에서 매복률이 유의성 있게 낮았다(p<0.05, Chi-square test).3. 매복치아와 맹출치아 간의 치료 전후 하악 제3대구치의 치축경사도와 교정치료 후 맹출 공간(eruption space)의 비교시 매복치아 집단에 비해 맹출치아 집단에서 치료 전 하악 제3대구치의 치축경사는 더 직립된 경향을 보였고 더 큰 맹출 공간이 존재하였다(p<0.05, t-test).결론적으로 소구치 발치를 동반한 교정치료는 하악 제3대구치의 발육 과정에서 치축경사도의 유의성 있는 개선을 일으킬 수 있으며 맹출 확률을 높여줌을 알 수 있었고, 하악 제3대구치의 매복에는 초기 치축경사도와 맹출 공간의 크기가 영향을 줌을 알 수 있었다. 치료계획 수립 시 이러한 부분을 고려한다면 교정치료 후 제3대구치의 매복이나 맹출 예측에 도움을 줄 수 있을 것이다.

[영문]The purpose of this study was to investigate the effect of premolar extraction therapy on mandibular third molar angulation and subsequent eruption, and to compare with a nonextraction therapy group.The following conclusions were reached:1. Changes in third molar angulation from pretreatment to posttreatment for three groups were compared for statistical differences using a ANOVA(p<0.05). Statistical analysis revealed premolar extractions therapy improved mandibular third molar angulation.2. In premolar extraction groups, there were statistically significant reductions in the rate of impaction of mandibular third molars compared with nonextraction group(p<0.05, Chi-Square test).3. Erupted teeth group had more favorable mandibular third molar angulation at pretreatment stage and larger eruption space than impacted teeth group(p<0.05, t-test).From above study, it is concluded that orthodontic treatment involving premolar extractions improves mandibular third molar angulations and decreases risk of impaction, and that early angulation of mandibular third molar and eruption space has significant effect in the eruption of it.
