병원정보시스템의 평가 지표 개발

Abstract

[한글]본 연구는 병원정보시스템을 평가하는데 필요한 지표들을 BSC 기법을 적용하여 병원의 실태에 맞게 개발하는데 그 목적을 두고 있다. 통합된 성과측정시스템의 하나인 Kaplan과 Norton의 균형성과표(BSC : Balanced Scorecard)에 대한 충분한 이해를 위해 BSC의 개념, BSC 모형의 측정치간의 인과관계, BSC와 관련된 이론적 연구, 실증적 연구 등의 선행연구를 바탕으로 이론적 검토를 시행하였고, 이를 통해 BSC 4가지 관점인 학습 및 성장관점, 내부업무 프로세스 관점, 고객 관점, 재무적 관점의 평가영역에 해당하는 지표들을 확인하였다. 우선 병원정보시스템과 관련된 선행연구의 검토를 통하여 병원정보시스템의 평가와 관련된 지표들을 파악하였고, BSC 모형을 통하여 네 가지 개념모형 속에 이 평가지표들을 구체화하였다. 그리고 이들 간의 관계에 대한 검토를 바탕으로 개념적 틀로 평가모형을 설정하였다.본 연구는 병원정보시스템을 중심으로 도출한 평가지표들로 구성된 기 조사된 설문내용을 바탕으로 분석하였다. 2005년 8월 1일부터 2005년 10월 31일까지 전국 병원의 병원정보시스템 관리자를 대상으로 실시한 설문조사는 종합전문병원 42개, 종합병원 73개, 병원 199개 등 총 314개 의료기관에서 응답하였다. 조사된 자료는 빈도분석, 신뢰성 분석, 확인적 요인분석, 상관관계분석 등의 실증분석을 하였다.본 연구의 결과로는 첫째, 기존의 연구들을 배경으로 병원정보시스템 평가를 위한 BSC 기법을 적용한 통합적 평가모형을 제시하였고, 이 모형을 바탕으로 우리나라 병원 환경에 맞는 병원정보시스템의 평가지표를 도출하였다. 이러한 평가모형은 병원정보시스템의 평가 측정을 재무적 성과에만 국한시키지 않고 비재무적 측정까지 포함시켜 정보화로 나타난 성과들을 다양하게 측정할 수 있게 하였다.둘째, 기존 연구에서 도출된 22개의 평가지표들은 확인적 요인분석을 통한 개념타당도 분석과 내적일관성 분석을 통한 신뢰도 분석을 통하여 타당성과 신뢰성을 입증하였다. 그 결과, 이론적 배경과 개념고찰을 토대로 평가영역 내에 있는 몇 가지 평가지표들이 재분류되었고, 총 3개의 지표가 제거됨으로써, 병원정보시스템 평가지표로서 최종 19개의 지표를 제시하였다.셋째, 최종 제시된 19개의 평가지표들을 대상으로 4가지 관점간의 상관관계를 분석한 결과, 각 관점간의 유의한 상관관계가 있는 것으로 나타났다. 성과기여 관점은 특히 고객만족도 관점과 유의한 상관관계를 보였고, 학습 및 성장 관점은 나머지 3가지 관점, 즉 내부 업무 프로세스, 고객만족도, 성과기여 관점 모두와 유의한 상관관계가 있는 것으로 나타났다.본 연구는 기존의 지표개발 연구와 달리, 통합된 성과측정시스템인 BSC 기법을 적용하여 병원 환경에 맞는 평가지표 개발을 시도하였다는 데에 연구의 의의가 있다. 따라서 본 연구의 제한점을 보완하여 향후 연구에서는 BSC 4가지 관점별로 좀 더 구체적이고 다양한 측정지표들을 도출하고, 평가영역의 개념에 적합한 실제 대상에게 평가를 한다면, 좀 더 객관적이고 실증적인 평가지표 제시가 가능할 것으로 기대한다.

[영문]The purpose of this study was to develop the performance indicators for evaluating information systems for the hospitals using the BSC framework. The performance indicators from four perspectives (financial, customer satisfaction, internal business process, and learning and growth) were initially derived from literature review. The final performance indicators were derived by using the frequency analysis, reliability analysis using Cronbach alpha, confirmatory factor analysis, and correlation analysis, based on the survey data from 314 hospitals (42 tertiary specialty hospitals, 73 general hospitals, and 199 hospitals) collected from August 1 to October 31 2005.The results were as follows:First, the BSC-based integrated model for the evaluation of hospital information systems was developed and the performance measurement indicators were developed for evaluating the performance of hospitals by using this model. Second, 19 performance indicators were derived by analyzing 22 indicators obtained from the previous study based on the confirmatory factor analysis and the internal consistency analysis. Third, these 19 indicators from four BSC perspectives were analyzed again using the correlation analysis. The performance contributing (financial) perspective was significantly correlated with the customer satisfaction perspective, and the learning and growth perspective was significantly correlated with the remaining three perspectives.This study may be the first attempt to develop the indicators for evaluating the performance of information systems for the hospitals under the BSC framework. In the future, these indicators should be further improved by adding more performance variables in the analysis using more hospital data.
