GIS를 이용한 입원환자의 지리적 접근성 분석

Abstract

[한글]본 연구는 특정 대학병원의 입원환자를 대상으로 지리정보시스템(Geographic Information System)을 이용하여 환자의 특성과 분포를 가시적으로 나타내어 의료이용의 공간적 접근성을 알아보았다. 거주지의 분포를 실제 접근하는 통행시간을 추정, 산출하기 위해 GIS tool의 하나인 TransCAD로 분석하였다.경기도에 위치한 허가병상 약 800병상 규모의 대학병원의 입원환자를 대상으로 하였으며, 2004년 10월 1일부터 2005년 9월 30일까지의 입원환자를 분석하였다.입원환자의 지리적 분포를 보면 83.1%정도가 대상병원에 30분 이내 접근 할 수 있는 지역에 거주하였으며, 지역별 구성도와 친화도 역시 인근지역에서 높게 나타났다. 입원환자의 특성별 통행시간을 기준으로 거주지 분포를 분석한 결과는 연령이나 질환군, 재원일수 등에 따라 분포가 다름을 알 수 있었다. 이런 지역별 분포를 통해 대상병원의 특성화의 필요성을 제시하고 마케팅을 위한 타겟 지역을 선정할 수 있으며, 더 나아가 gCRM의 일환으로 지역별 특성을 세분화하여 주민들이 선호하는 내용의 건강강좌나 무료검진 등을 계획하는데 반영하여 환자들이 필요로 하는 의료서비스를 제공할 수 있다.

[영문]In this study, the factors influencing the use of inpatient services were examined by using the special analysis with GIS(Geographic Information System). The time to come to hospital was estimated by the GIS tool, called TransCAD, and was used in the analysis.The subjects of the study were inpatients at the university hospital with about 800 permitted beds, located in Gyeonggi-do, and patients admitted from October 1, 2004 to September 30, 2005.As for the patient distribution by region, approximately 83.1% of patients resided in a region that they could reach the hospital within 30 minutes, and the regional composition and affinity were also higher in the neighboring regions. As a result of the analysis, influencing factors for the use of inpatient service were : age, disease group, and the length of stay. Through the distribution by each region, the specialization of the target hospital could be suggested and the target areas for marketing could also be selected; furthermore, the medical services needed by patients could be provided by subdividing regional characteristics as a part of gCRM and by reflecting them when planning health-related lectures or free medical examination preferred by residents.
