병원정보시스템 관리상의 주요 이슈에 관한 연구

Abstract

[한글]병원정보화의 수준은 의료기관의 경영 성패를 좌우한다고 해도 과언이 아닐 정도로 비용 절감 및 서비스 향상 등의 효과를 가져오고 있다. 이에 따라 정보시스템을 관리하는 관리자는 기관의 목표 달성 및 경쟁력 강화를 위한 관리상의 주요 이슈는 무엇인지 예측하고 중요성을 인식하여 의사결정에 적용해야 하는데 이를 위해서는 주요 관리 이슈를 예측 할 수 있는 연구가 필요하다.본 연구는 미국 SIM survey의 경우와 같이 이미 정해진 이슈를 근간으로 이슈를 재평가하고 새로운 이슈를 찾는 연구로 병원정보시스템 관리자를 대상으로 향후 3~5년 관리상 주요 이슈를 파악하여 선행연구와 그 변화 정도를 비교 분석함으로써 향후 병원정보시스템의 관리 및 연구 방향을 제시하고자 하였다.델파이 기법을 통하여 2차례의 설문조사와 면담조사가 이루어 졌으며 연구 결과 향후 3-5년 병원정보시스템의 관리상 주요 이슈는 ‘최고 경영자의 지원’으로 나타났고 그 외 ‘사용자 부서와의 유기적 협조체제’, ‘PACS의 도입’, ‘재해복구 시스템의 구축’, ‘정보시스템의 전략적인 계획 수립’ 등의 순서로 나타났다.1999년의 선행 연구와 비교하여 꾸준히 10위 이내의 순위를 유지하는 이슈들은 ‘최고경영자의 지원’, ‘정보자원의 보안 및 통제’, ‘사용자 부서 와의 유기적 협조체제’, 등으로 병원정보시스템이 꾸준히 성장하는 단계에 나타날 수 있는 이슈들로 평가된다. 새롭게 등장한 상위 이슈로는 ‘재해복구 시스템 의 구축’과 ‘표준화 강화’로 환자기록의 유실을 예방하기 위한 시스템의 안정성, 의료기관간 진료정보교환을 위한 표준화의 필요성등이 대두되면서 등장한 이슈들로 평가된다. 탈락된 이슈들은 ‘정보통신망 운영•관리’, ‘인적자원의 고용 및 개발’ 등으로 병원정보시스템의 초기 도입 당시에 중요하게 생각되었던 이슈들로 평가된다.중요성과 지속성에 의한 이차원적 분류에서 핵심 이슈들은 ‘최고 경영자의 지원’, ‘정보자원의 보안 및 통제’ 등으로 나타났으며, McFarlan’s framework에 적용한 결과 전략적 이슈로는 ‘최고경영자의 지원’, ‘사용자 부서와의 유기적 협조체제’, ‘표준화 강화’, ‘전략적 계획수립’ 등으로 나타났다.이상의 결과를 토대로 한 향후 병원정보시스템의 발전 방향은 첫째, 시스템의 도입시에는 표준화나 보안, 재해복구 등을 고려해야 하며 최고경영자의 꾸준한 지원과 관심이 절실하게 필요하다. 또한 시스템의 효율성을 극대화 하기 위해서는 사용자 부서와의 유기적 협조가 이루어져야 한다. 둘째, 병원정보시스템의 발전 속도는 타 기업정보시스템 보다는 느리며 전문 인력 또한 부족한 상태로 대부분의 병원이 외주에 의존하고 있는 실정이다. 따라서 정보시스템을 관리하고 더 나은 시스템 개발을 위한 전문인력의 양성이 필요하다. 셋째, 병원정보시스템의 궁극적인 목표는 질적인 의료 서비스 제공 및 병원경영의 효율화일 것이다. 병원정보시스템 관리자 및 병원의 경영진들이 전략적인 계획을 잘 세울 수 있도록 하기 위해 병원정보시스템의 성공 요인에 대한 경제성 분석 등 객관적인 평가가 관리이슈에 대한 조사와 함께 주기적으로 수행되어야 한다.본 연구는 병원정보시스템의 관리상의 이슈를 재조사하여 1999년 선행 연구와 비교했다는 점과 연구 대상을 포괄적으로 선정 했다는 점, 중요성 및 지속성에 의한 분류를 통해 향후 병원정보시스템의 관리 및 발전 방향을 제시 했다는 데에 그 의의가 있다.

[영문]Information systems greatly affect performance of hospitals, especially in reducing costs and improving quality of services. Accordingly, managers for the information center should improve efficiency of their information systems by identifying key management issues in information systems that are important to achieving hospital objectives and strengthening its competitiveness. This study was conducted to identify 3~5 years future management issues in hospital information systems based on the sample survey of 50 managers from 28 hospital information centers. Two rounds of interview surveys were conducted by using the Delphi method.As a result, the following issues were identified in order: ‘top management support’, ‘close relationship with users’, ‘PACS’, ‘disaster recovery’, improving IS strategic planning’, ‘standardization’, ‘legalizing the electronic medical record’, ‘security and control’, ‘EMR’, and ‘facilitation of organizational learning’. Compared with the 1999 survey, the following issues were within the top 10 ranks for both periods: ‘top management support’, ‘security and control’, ‘close relationship with users’. New issues are ‘disaster recovery’ and ‘standardization’. Issues dropped from top 10 ranks were ‘network management’, ‘recruiting and developing IS human resource’ those are very important issues in the initial state for introducing the hospital information system.The issues classified by ‘importance’ and ‘persistence’ were: ‘top management support’, ‘security and control’ and etc. In addition, the strategic issues under the McFarlan’s framework were: ‘top management support’, ‘close relationship with users’, ‘standardization’, and ‘improving IS strategic planning’.Based on the above results, the following management strategies were recommended: obtaining continuous strong support from the top management, training system professionals, conducting a periodic economic analysis of systems to support strategic planning for hospitals, strengthening a collaboration with the user departments, and improving standardization and security.Significances of this study are: predicting future management issues based on the comparison of the current issues with the 1999 issues and recommending management strategies based on the classification of issues by importance and persistence.
