지능형 진단검사의학과 종합검증 판독 자동화 시스템 개발 및 성과분석

Abstract

[한글]입원환자의 조속한 진단 및 치료에 도움을 주고 환자 진단 치료시기를 단축하고자 검사실 인증기관에 한하여 진단검사의학과로부터 발생되는 환자의 검사결과 및 진료 기록 등을 기초로 하여 검사결과의 적절성 검토를 한 후, 진단검사의학과 전문의의 전문지식을 이용하여 해당 환자의 주치의에게 종합검증 판독 소견서를 작성 보고한다. 해당 주치의는 이를 바탕으로 최종 진단과 치료에 보다 정확하고 신속한 결정을 내릴 수 있어 환자는 입원 후 빠른 쾌유와 입원 기간을 최대한 줄일 수 있는 것으로 사료되었다. 그러나, 대부분의 병원에서 급증하는 환자와 다양한 검사로 인하여 종합검증 판독 보고서 제작에 시간과 인력이 많이 소요된다. 환자의 진료정보, 입원정보 등의 통합된 정보 조회가 이루어지지 않아 환자관리 차원에서 어려움이 있어 이들 정보를 신석 정확하게 분석하여 효과적인 정보 제공을 하고자 지능형 종합검증 판독 자동화 시스템을 개발하게 되었다.진단검사의학과 전문의와 각 과 전문의의 지식을 습득하고 객체지향 기법을 이용하여 지식간의 연관성을 규명하고, 이를 규칙으로 표현하였다. 실제 입원환자를 대상으로 입원기록과 임상병리 검사 결과를 이용하여 구축된 규칙베이스의 타당성 검증을 실시하였다. 개발된 지능형 진단검사의학과 종합검증 판독 자동화 시스템과 진단검사의학과 전문의가 판독한 소견 결과를 비교하여 93.0%의 소견 예측력을 나타내었다.검사결과 해석의 자동화를 위한 지능형 진단검사의학과 종합검증 판독 자동화 시스템을 개발함으로서 병원에서 검사정보의 신뢰성이 향상되고 입원환자의 종합검증 판독 소견서 제출에 걸리는 시간이 현격하게 감소되었다. 진단검사의학과 전문의의 전문 지식을 제공함으로서 효율적인 진단이 이루어져 환자 만족도와 신뢰도가 향상되었으며, 다른 업무와의 병행이 가능하여져 환자의 건강 증진과 진료의 질적 수준 향상이 이루어 질 것으로 사료된다.

[영문]Laboratory final report are prepared and send to the physician in charge after test results are verified based on medical records for the accredited laboratories. However, it often takes long time to prepare the final reports because information on treatment and test results are not well prepared in time for many hospitals. In this study, the intelligent interpretation system for laboratory results was developed in order to provide test results to the physician in charge in a timely manner.In a knowledge acquisition process, knowledge of specialists in the laboratory medicine and other specialists were obtained first, and then their relationships were identified and represented in a form of rules. When the system outputs were compared with the actual readings by the laboratory specialist, it showed 93.0% accuracy. By automating interpretation of laboratory results, time for preparing the final laboratory results can be significantly reduced. In addition, patient satisfaction can also be increased since reliability of diagnosis is likely to be improved as doctors are provided with variety of relevant information in time.
