비용을 동반한 만성 부비동염 환자에서 배양균에 대한 항생제 감수성

만성 부비동염의 치료 원칙은 감염을 치료하고 비강의 생리상태를 정상적으로 유지시키는 것이다. 약물치료에 있어서 최근 항생제 남용과 그에 따른 내성 균주 증가로 인해 항생제 내성에 대한 정보의 중요성이 증가하고 있다. 일반적으로 만성 부비동염에서의 1차 항생제 치료는 광범위 항생제의 경험적 처방이었으나, 최근에는 만성 부비동염의 주요 원인 균주 대부분이 β-lactamase를 생성하게 되었고 3차 병원에서는 대부분의 환자가 항생제를 복용한 상태로 오기 때문에 만성 부비동염의 항생제 선택은 더욱 신중해야 하겠다.본 연구는 비용을 동반한 81명의 만성 부비동염 환자에서 중비도에서는 면봉 도말로 부비동에서는 흡인 천자로 검체를 채취 후, 호기성 및 혐기성 배양하여 분리된 균주에 대하여 항생제 감수성 검사를 시행하였다. Penicillin G, oxacillin, cefixime, cefdinir, cefditoren, cefpiramide, cefotetan, amikacin, clindamycin, levofloxacin, gatifloxacin, clarithromycin, azithromycin(BBL, Sparks, MD, USA)등 3차 의료기관에서 주로 사용되는 항생제를 대상으로 하여 최소 발육 저지농도와 항생제 감수성 정도를 평가하였다. 분리된 균주 가운데 Haemophilus influenzae는 46%가 ampicillin내성이었으며, Staphylococcus aureus의 14%가 oxacillin내성이었고, 이러한 MRSA는 대부분의 항생제에 내성을 보였다. Streptococcus pneumoniae는 cefa계열 항생제에 0-50%의 내성율을 보였으며 macrolide계열에도 75%의 내성을 보였다. Moraxella spp.는 penicillin을 제외한 모든 약제에 감수성을 보였으며, 혐기성 균주의 대부분은 cefixime을 제외한 cefa계열에 내성을 보이지 않아 항생제 내성율이 높지 않음을 알 수 있었다. 한편 , penicillin 내성율은 S. aureus는 93%, S. pneumoniae는 25%, Moraxella spp.는 100%를 나타내어 1차 약제로써 penicillin은 부적절함을 알 수 있었고, quinolone계열은 MRSA와 일부 Prevotella spp.균주에 대해서만 내성을 보였고, macrolide계열은 S.pneumoniae와 S. aureus 일부에서만 내성을 보여 대부분의 균주에 유효한 약제임을 알 수 있었다.결론적으로 비용이 동반된 만성 부비동염 환자에서 항생제 감수성 검사결과 ampicillin을 제외한 ampicillin/sulbactam 및 cefa계열, macrolide계열 항생제가 일차 약제로 투여 가능하고 환자의 증상 개선이 없을 때, 항생제 감수성 검사를 통해 이차 항생제를 선택해야 할 것으로 생각되며, 만성 부비동염에 대한 항생제 내성율 보고가 정기적으로 이루어 져야 할 것으로 생각된다.



[영문]The principles for the treatment of chronic sinusitis are to control the infection and preserve the normal physiology of the nasal cavity. Recently the overuse of antibiotics and the increase of resistant bacteria show the importance of a study regarding antibiotic resistance. The first-line antibiotics for chronic sinusitis were empirical broad spectrum antibiotics, but as most of the main causative bacteria produce β-lactamase, this needs to be reconsidered. Especially in a university hospital, the choice of the appropriate antibiotics is of great importance because of patients with previously prescribed antibiotics.In 81 patients with chronic sinusitis and nasal polyps, after obtaining the specimen from the middle meatus using a cotton swab and from the maxillary sinus by aspiration, both aerobic and anaerobic cultures were performed. The causative pathogens were identified through aerobic and anaerobic cultures and antimicrobial susceptibility tests were performed using the most commonly used antibiotics, for example, penicillin, oxacillin, cephalosporins, amikacin, clindamycin, levofloxacin, gatifloxacin, clarithromycin, and azithromycin.Ampicillin-resistant H. influenzae isolates were identified in 46% of the cases and only 14% of Staphylococcus aureus isolates were oxacillin-resistant. The resistance rates for S. pneumoniae were 0-50% and differed greatly among cephalosporins and 75% of S. pneumoniae was resistant for macrolide. Most of the anaerobic isolates did not show resistance for cephalosporins except for cefixime. The penicillin resistance rates were 93% for S. aureus, 25% for S. pneumoniae, and 100% for Moraxella spp., which suggest the inappropriateness of penicillin as a first-line drug. MRSA and some isolates of prevotella spp. were resistant for quinolones and some isolates of S.pneumoniae and S.aureus were resistant for macrolides, but macrolides and quinolones were still effective for most of other pathogens.Considering these results of antibiotic susceptibility tests in chronic sinusitis patients, it seems appropriate to use ampicillin/sulbactam, macrolides, and cephalosporins as the first-line antibiotics. In cases where there is no improvement of symptoms, cultures should be taken , followed by appropriate selection of second-line antibiotics according to the sensitivity test results.
