국내병원과 국내주둔 미군병원에서 분리한 Methicillin 저항성 Staphylococcus aureus의 항생물질 내성형과 mecA 유전자의 검출

2004년 1월에서 3월까지 서울에 주재하고 있는 미군병원에서 분리된 methicillin resistant Staphylococcus aureus(MRSA) 30균주와 국내의 서울 지역병원에서 분리한 MRSA 17균주, 부산지역에서 분리한 25균주 그리고 1999년 강원도 원주시에 소재하고 있는 원주기독병원에서 분리한 MRSA 36균주를 실험대상으로 하여 VITEK system과 E-test를 통하여 항생제에 대한 minimum inhibitory concentration(MIC)을 분석하였다. 그리고 E-test를 통하여 oxacillin과 cephalothin에 대한 감수성을 집단별로 비교 분석하였다.분자생물학적인 방법인 PCR을 통하여 methicillin에 내성을 유발하는 mecA gene의 검출을 확인하였으며, SCCmec(Staphylococcal chromosomal cassette) 유전자의 위치를 알아보기 위한 SCCmec primer들을 이용한 multiplex PCR을 시행하여 각각의 다른 유전자의 종류를 포함하고 있음을 확인하였다. 3개의 집단 에서 분류된 MRSA 임상분리균주를 대상으로 VITEK system과 E-test를 통하여 시행한 항생제에 대한 MIC 결과는 121st General Hospital에서 분리한 MRSA 임상분리균주들과 국내 병원들에서 분리한 MRSA 임상분리균주들과의 항생제들 간의 내성률의 큰 차이점을 확인하였으며 국내에서 분리한 MRSA 임상분리 균주 들은 121st General Hospital에서 분리된 균주들보다 항생물질에 대한 내성률이 매우 높은 것으로 나타났으며, 국내에 주둔하고 있는 121st General Hospital과 국내병원에서 분리한 MRSA 임상분리 균주들을 대상으로 다중중합 효소연쇄 반응을 통한 SCCmac type 검사결과 108개의 모든 균주는 SCCmec type 1-IV 중 하나의 type 혹은 2, 3개의 type으로 분류되었다.

[영문]A total of 108 strains of MRSA clinical isolates was collected from 121st general hospital(U.S. military hospital), Korean healthcare facility from January to March in 2005 and WonJu Christian hospital in 1999. To analyze and determine the type of resistance to antimicrobial agents, antimicrobial susceptibility test by Vitek System and MIC test using oxacillin and cephalothin stripes by E-Test.PCR based detection of mecA gene were performed and the location of SCCmec were tested on the all of MRSA clinical isolates by multiplex PCR and found the location of PCR products.In conclusion, MRSA clinical isolates was characterized with the results from antimicrobial agents resistance patterns, PCR based detection of mecA gene and validation of the multiplex PCR strategy of SCCmec among 121st general hospital, KHF, and Wonju clinical isolates.
