지역사회 거주 노인의 알코올 사용장애와 인지기능, 우울증의 연관에 관한 연구

지속적인 과다한 음주가 다양한 인지기능의 장애를 일으킨다는 것은 여러 연구에서 잘 알려져 있다. 노인에서 이러한 인지기능의 장애는 젊은 성인에 비해서 상대적으로 소량의 음주로도 문제가 초래될 수 있다. 또한 알코올의존과 우울증상은 밀접한 관련성이 있다. 따라서 노인의 알코올 사용장애에서는 전반적인 인지기능과 우울증상의 정도를 함께 평가해야 한다.본 연구는 238명의 지역사회 거주 노인을 대상으로 알코올 사용장애와 연관된 변인을 규명하고자 시도하였다. 본 연구에서는 알코올 사용장애 선별검사(AUDIT-K)에서 15점 이상을 알코올 사용장애로 정의하고, 간이정신상태검사(MMSE-KC)와 노인 우울척도 (GDS-K), 주관적 기억문제 호소 설문, 일상생활능력 척도 등을 함께 시행하였다.본 연구의 결과 낮은 연령, 남성, 저학력, 우울증상이 노인의 알코올 사용장애와 의미있는 연관이 있었으며(P<0.05), 인지기능, 일상생활능력, 주관적 기억문제 호소는 연관이 없었다. 향후 이 연구의 결과는 종적인 연구를 통해 확인이 필요할 것이다.

[영문]It is well known that persistent and large amount of alcohol intake can cause various cognitive impairment. In old age, relatively small amount of alcohol intake can induce cognitive impairment. It is also well known that alcohol use disorder(AUD) and depressive symptom is closely associated. Thus, evaluation of AUD in old age should include general cognitive function and depressive symptom.This study aimed to elucidate the associated variables with AUD in 238 community dwelling elderly subjects. AUD was defined by Korean version of Alcohol Use Identification Test (AUDIT-K) with 15 point as cut-off. Korean version of Geriatric Depression Scale(GDS-K), MMSE in the Korean version of CERAD assessment battery(MMSE-KC), subjective memory complaint questionnaire(SMCQ), activities of daily living scale were also administered.In the result of this study, young age, male sex, low education, depressive symptom showed significant association with AUD in old age(p<0.05), but cognitive function, activities of daily living function and subjective memory complaints showed no association. Result of this study should be confirmed by further longitudinal study.
