[한글]

연구 목적 강박장애의 생물학적 원인에 대한 연구가 수행되고 있으나 일관된 결과를 보여주지 못하고 있는데, 그 원인 중의 하나가 강박장애 환자들이 서로 다른 생물학적 특성을 가지고 있는 이질적 집단이기 때문인 것으로 생각되고 있다. 본 연구는 최초로 강박장애 환자들을 2년 간 치료 한 후 이들의 임상 증상, 신경인지기능, 뇌회백질의 변화를 정상대조군과 비교 분석하여 치료반응에 따른 생물학적 인자들의 차이를 규명하고자 하였다.연구 방법 17 명의 강박장애 환자와 14명의 정상대조군을 선정하여 연구시작 시점에 예일-브라운 강박장애 척도, 기질성격척도, 베크의 우울척도 및 불안 척도, 복잡한 도형 검사, 통제된 구어 연상 검사, 길 만들기 검사, 위스콘신 카드 검사, 뇌자기공명영상을 시행하였고 2년간 치료 후, 동일한 검사를 시행하였다. 치료 후 임상증상의 호전 정도에 따라 강박장애 환자군을 치료반응군(6명)과 치료미반응군(11명)으로 분류하였으며 이들간의 임상적 변인, 신경인지기능의 차이를 반복분산분석으로 비교하였다. 뇌회백질 농도의 차이는 뇌의 회백질 전체를 대상으로 하여 SPM2 프로그램을 이용한 voxel-based morphometry 방법을 이용하여 조사하였다.연구 결과 강박장애 치료반응군은 정상대조군과 유의한 신경인지기능의 차이를 보이지 않았다. 치료미반응군은 복잡한 도형 검사에서 즉각회상점수와 지연회상점수가 정상대조군보다 유의하게 낮았다. 또한 통제된 구어 연상 검사에서 범주 유창성 항목에서 치료 미반응군이 정상대조군보다 연상의 수가 적은 경향성을 보였다. 길 만들기 A형 검사에서는 치료 반응군과 치료 미반응군 모두 정상대조군보다 수행시간이 유의하게 길었다. 연구시작 시점에서 치료반응군은 정상대조군에 비해 우측 중간후두이랑 부위가 회백질농도가 유의하게 높았으며 치료미반응군은 정상대조군에 비해 우측 상전두이랑 부위의 회백질 농도가 유의하게 높았고 우측 하전두이랑 부위에서 정상대조군보다 유의하게 회백질의 농도가 낮게 나타났다. 치료 미반응군과 치료 반응군의 비교에서는 우측의 안와이랑, 뇌섬 부위가 유의하게 회백질 농도가 낮았다. 2년 치료 후의 뇌자기공명영상 결과를 포함한 비교에서는 치료 반응군의 우측 소뇌 부위가 정상대조군과 치료 미반응군보다 높게 나타나는 집단간 차이를 보였으며 치료미반응군은 좌측 뇌섬 부위가 정상대조군보다 회백질 농도가 높아진 것으로 나타났다.결 론 강박장애 환자는 치료 반응에 따라 신경인지기능의 손상 정도가 차이가 나며 정상군과의 뇌회백질 이상을 보이는 부위도 서로 달랐다. 이는 치료반응에 따라 강박장애 환자들이 이상을 가지고 있는 신경회로가 서로 다를 가능성을 시사한다.



[영문]Objective: Many studies performed about the biologic etiologies of obsessive compulsive disorder have not showed an obvious result. The differences in biologic characteristics among obsessive compulsive disorder patients are considered as one of possible etiologies. This is the first study to prove differences in biologic characteristics between good treatment response and poor response obsessive compulsive disorder patients by comparing with normal control group after two years of treatment by analyzing neurocognitive functions and change in gray matter.Method : Yale-Brown Obsessive Compulsive Symptoms scale(YBOCS), Temperament and Character inventory(TCI), Beck Depression Inventory(BDI), Beck Anxiety Inventory(BAI), Rey-Osterrieth Complex Figure Test(RCFT), Controlled Oral Word Association Test(COWA), Trail Making Test(TMT), Wisconsin Card Sorting Test(WCST) and brain magnetic resonance image(MRI) scanning were performed at the beginning of the study and after two years of treatment on 17 OCD patients and 14 controls. After treatments, OCD patients group was divided into good responder group(6 patients) and poor responder group(11 patients) and clinical variables and neurocognitive function were analyzed by repeated measured ANOVA. The difference in gray matter concentration was performed with SPM2 program by voxel-based morphometry methods.Results: Analysis of neurocognitive function didn't show significant statistical difference between good responder OCD and control group. Poor responder group showed significantly lower performance in immediate recall and delayed recall of RCFT. In addition, category fluency of poor responder OCD group was relatively impaired than normal control. Both groups of OCD patients took longer time in completing TMT part A. The gray matter concentration in right middle occipital gyrus of good responder group was increased than normal control at baseline of the study. The poor responder group showed increased gray matter concentration in right superior frontal gyrus and showed decreased gray matter concentration in right inferior frontal gyrus. Compared with good responder OCD group, the poor responder showed decreased gray matter concentration in areas such as right orbital gyrus, insula. After 2 year treatment, good responder OCD group showed increased gray matter concentration than normal control in right cerebellum. The poor responder group showed increase in left insula comparing with normal controls.Conclusion: Treatment poor responder OCD patients were more impaired in neurocognitive functions comparing with normal controls than good responder. The areas which showed abnormalities in gray matter concentration were different between two OCD groups. These findings suggested they are affected by different neural circuit.
