dc.description.abstract[한글] 수술간호란 환자가 수술을 경험하는 수술 전 중 후 기간 동안 전문직 간호사에 의해 수행되는 활동이다. 효과적으로 간호 인력에게 업무를 적절히 분배하여 비전문 간호업무를 줄이고 가치 있는 전문 수술간호업무를 합리적으로 수행 할 수 있게 할 뿐만 아니라 업무가 제대로 수행되지 않는 원인을 알고 가능한 처방책을 제시하기 위해서 수술간호업무 분석이 필요한데 업무는 행위 및 여러 가지 활동을 포함하는 상위개념이다.이 연구의 목적은 서울에 소재한 일 대학 부속병원 수술실 간호사들을 대상으로 일주일동안 수행한 1) 50개 수술간호 활동별 소요시간을 파악하고 2) 전문 수술간호활동과 비전문 수술간호활동을 분류하며 3) 대상자 특성별 수술간호 활동시간을 분석한 후 4) 비전문 간호활동을 위한 위임대상 인력을 산출하는 것이었다.자료는 윤선옥(1999)에 의해 개발되고 박광옥(2001)등에 의해 수정된 '수술실 간호활동 조사지'를 이용하여 2005년 3월 16일부터 3월 22일까지 254명의 수술실 간호사들로부터 수집하였다.자료는 Excel로 분석하였다.본 연구의 결과는 다음과 같다:1. 22개 수술실 가동을 위한 주중 전체 근무자(401명)중 58.1%인 233명이 조사에 참여했고, 주말은 전체 근무자(22명)중 95%인 21명이 조사에 참여했다.2. 50개 수술간호활동을 전문 수술간호활동(46개), 비전문 수술간호활동(3개), 식사 및 개인적 용무(1개)로 구분하였다.3. 전수조사가 이루어지지 않으므로 해서 업무 수행수준별 5개 그룹으로 분류한 후 그룹·근무번 별 조사 참여자의 각 활동시간 평균에 그룹근무자수를 곱한 후 모두 합하여 전수대비 예측 활동시간을 도출하였다.4. 예측된 주중 간호영역별 일일 소요시간을 보면 사정 및 평가업무는 평균 44분, 환자 준비업무는 15분, 기술적 업무는 339분, 감독 및 감시업무가 54분, 기타업무가 85분이었다.5. 주중 총 수술건수는 361건으로 일인당 수술건수는 0.9건이었다.6. 전체 근무자의 주중 수술 한 건당 예측된 거즈, 기계, 바늘 계수의 평균시간은 10.39분이었다. 이는 이중계수 원칙에 의하면 수술 1건당 약 5.2분정도 이루어짐을 의미한다.7. 주말의 수술관련 물품 만들기 항목이 2785.0분(25.79%), 수술실 청소 및 정리정돈이 1732.5분(16.05%)을 차지하였다.8. 비전문 간호활동으로 분류되어 예측된 수술 전 소독물품 기구준비항목, 기계세척, 포장, 소독업무(소독용기 수거소독포함)항목과 수술관련물품 만들기 항목의 주중 및 주말의 시간은 각각 7729.9분(3.60%), 14411.2분(6.71%), 1548.1(0.72%) 및 180.0(1.66%), 1152.5(10.67%), 2785분(25.79%)이었다.9. 주중 대상자 특성별 수술간호 활동시간을 보면 기계세척, 포장, 소독업무(소독용기 수거소독포함)항목은 주로 초 밤번(62%), 밤번(22%)근무 중에 수행된 것으로 나타났다.10. 비전문 간호활동으로 분류되어 예측된 수술 전 소독물품 기구준비항목, 기계세척, 포장, 소독업무(소독용기 수거소독포함)항목, 수술관련물품 만들기 항목의 주중과 주말의 비중은 9.86명(3.22명, 6.00명, 0.64명)분과 1.73명(0.09명, 0.48명, 1.16명)분이었고 이들의 합은 11.59명분 이었다.11. 주중 식사 및 개인적 용무 항목의 평균은 약 29분이며 총 89명의 간호사가 식사를 위해 소요한 예측된 시간은 11709.6분(4.9명)이었다.12. 주중 예측된 일평균 근무시간은 536.86분으로 56.86분의 초과근무가 발생하는 실정이었으며, 89명 간호사의 초과근무 시간 합은 23473.4분 이었다. 이것을 (60분x8시간x5일)로 나누면 9.8명분이었다.본 연구의 결과로부터 다음과 같은 결론이 도출 될 수 있다:비전문 간호활동의 위임과 함께, 초 밤번 간호사 근무 시작 시간이 좀 더 늦은 시간으로 변경되어야한다.비전문 간호활동이 간호보조 인력으로 위임되면 수술실은 11.59명 간호사와 간호보조 인력의 인건비 차만큼의 비용을 줄일 수 있다.식사교대를 위해 4.9명, 초과근무에 대한 9.8명, 총 14.7명의 증원이 요구된다.인력의 재분배로, 시간외 근로시간이 감소 할 것이고 수술실 간호사들은 수술환자간호를 위해 그들의 기대되는 고유한 더 많은 간호활동을 할 수 있을 것이다.비전문 간호활동의 위임 또는 물품 및 수술기구 공급체제 변경 이후, 간호활동의 변화 및 비전문 간호 활동의 잔류를 평가하는 추후연구가 필요하다. [영문]Operative nursing indicates all the activities of the professional nurse before, during and after any operations a patient receives in the hospital. For effective distribution of work, and in order to reduce nursing time on non-professional nursing jobs, to achieve rational professional operative nursing and to understand the reasons for work not being covered and suggest possible prescriptions, analysis of theoperative nursing job is needed. In this sense, nursing in the operating room is of a superordinate concept which includes nursing behavior and various activities.The main purpose of this study was to examine the nursing activities in an operating room of a university-affiliated hospital in Seoul, Korea, 1) to measure the time consumption of 50 operating room nursing activities, 2) to distinguish professional operating room nursing activities and non-professional activities and 3) to analyze time consumption over 1 week according to characteristics, then 4) to calculate the delegation of personnel for non-professional nursing activities.Data were collected using ''the operative nursing activities survey'' by Yoon(1999) modified by Park et al(2001) during the 1 week period from March 16 to March 22, 2005. The researcher collected data from 254 operating room nurses in a university-affiliated hospital.The data were analyzed with Excel.The results are as follows:1. For the 22 operation rooms, the total number of nurses on duty on weekdays was 401, of which 233 participated in the survey and on the weekend, the total was 22 and 21 participated in the survey.2. The 50 operative nursing activities were classified as professional, operative nursing activities (46), nonprofessional, nursing activities (3), and personal activities such as meals and private time. (1)3. As it was not possible for all of nurses working in the hospital to participate in the study, nurses were grouped into 5 groups according to levels of performance, and duties were classified into 3 groups, then the average time for activities according to level of performance and duties by the participants in the study were calculated. This average was multiplied by the total number of the nurses in each group, and averages were totaled in order to derive the time for total activities by the total number of nurses in the hospital.4. Estimated mean time of 44 minutes was used for activities in the nursing domain, assessment and evaluation, 15 minutes for patient preparation, 339 minutes for technical work, 54 minutes for supervision and observation, and 85 minutes for miscellaneous activities.5. The total number of operations was 361 with a mean number of 0.9 operations per nurse.6. Estimated mean time for counting gauze, instruments and needles for each operation was 10.39 minutes. According to the double count principal, the result indicates time consumption was about 5.2 min./operation.7. On the weekend, preparation of materials related to operations took 2,785.0 minutes (25.79%) and time for cleaning and arranging the operating room, 1,732.5 minutes (16.05%).8. On weekdays, time for pre operative preparation of medical supplies & instrument and for washing, wrapping, and sterilization of instruments and preparation of materials related to operations was 7,729.9 minutes (3.60%), 14,411.2 minutes (6.71%), 1,548.1 (0.72%) respectively and on weekends, 180.0 (1.66%), 1,152.5 (10.67%), 2,785 minutes (25.79%) respectively.9. On weekdays, regarding the time taken for operative nursing activities according to each of the characteristics of the participant, it was found that washing, wrapping, and sterilization of instruments (including sterilization of the receptacles) was generally performed by the evening shift (62%) or night shift (22%).10. Classified as non-professional nursing activities, which were estimated pre operative preparation time for medical supplies & instrument, time for washing, wrapping, and sterilization of instruments and preparation of material related to operations, took 9.86nurses (3.22, 6.00, 0.64 persons) on weekdays and 1.73 nurses (0.09, 0.48, 1.16 persons) on weekends for a total of 11.59 nurses.11. On weekdays, there was no extra staff to cover for meals and private time, which took a total of 11,709.6 minutes (4.9 persons to cover for staff of 89 nurses for meal time), a mean time of 29 minutes.12. On weekdays, estimated mean 1 day work time was 536.86 minutes as the nurses worked an average of 56.86 min./day overtime. The sum of overtime for 89 nurses on weekdays was 23,473.4 minutes. When it was divided by (60minx8hrx5day), it was found to be equivalent to 9.8 persons.From the results of this study the following conclusions can be drawn: With the delegation of non-professional operative nursing activities, the start of duty hours for the evening shift could be delayed.If operating nurses delegate their non-professional nursing activities to a non-professional, salary costs could be reduced as much as 10.4 nurses'' salaries minus the non-professional''s salary.A minimum of 4.9 additional persons to cover meal time and 9.8 persons for overtime for 89 nurses are needed, that is a total of 14.7 persons.With a supplement of personnel, operating room nurses could reduce their overtime and provide more nursing activities for nursing care.There is a need for follow up research regarding change of nursing activities and what non-professional nursing activities remain after delegation.-
