다운증후군형제자매가 있는 청소년의 경험

본 연구는 다운증후군아형제자매가 있는 청소년들이 생활하면서 가족과 학교등 사회에서 겪는 경험들에 대하여 알아보기 위해 실시된 서술적 탐색 연구이다.

본 연구의 참여자는 여학생이 3명, 남학생이 2명으로 총 5명이었고 자료수집은 반구조화된 질문지를 사용하여 심층면접을 실시하였다. 참여자별 1회 평균 면접시간은 100분이었고 총 면접 횟수는 일인당 3회에서 4회였으며 자료수집기간은 2004년 10월부터 2004년 12월까지였다. 녹음된 면담자료는 주제 분석법에 의해 분석하였다.

본 연구의 결과 다운증후군 형제자매가 있는 청소년의 경험에 관해 4개의 범주와 17개의 주제가 도출되었으며 범주는 다음과 같다 : 충격과 스트레스 속에서 자기 성장을 준비함, 염려(걱정)속에서 다운 증후군아동 돌보기, 부모와의 갈등 속에서 가족의 일원이 되어감, 장애아동의 형제자매로서 친구 사귀기, 정상적인 청소년의 발달기적 갈등과 어려움/다운증후군형제자매가 있는 청소년의 경험에 대해 17개로 도출된 주제의 총체적 글쓰기는 다음과 같다 ;

어머니의 임신과 장애형제자매 출생으로 큰 충격을 받고 당황하지만 성장하면서 장애형제자매의 입장을 이해하고 익숙하게 되어 일상화 된다. 그러면서도 나중에 자신이 결혼을 하게되었을 때는 장애자녀를 출산할까 두려워한다. 또한 장애자녀를 가진 부모의 보상심리에 따른 학업성취요구와 기대로 인해 자신이 꼭 성공하여야 하며 공부도 잘 해야 한다는 부담감이 있다. 장애형제로 인한 속상함이나 갈등을 아무에게도 드러내지 못하고 혼자 고민을 삭히지만 살면서 부모에게 무슨 일이 생기면 그땐 자신이 다운형제자매의 부모역할을 대행하여야한다는 사실에 큰 두려움을 보였다.

장애형제의 특이한 외모와 유별난 행동, 주위의 시선에 창피해하고 속상해하고, 부모가 슬퍼할까봐 궁금한 것을 묻지 않아 다운증후군에 대해 의도적으로 무관심한 것처럼 표현하지만 일상생활에서 장애형제자매를 돌보는 경험으로 많은 것을 알아간다. 장애형제가 성장하여 무엇을 하며 어떤 모습으로 있든지 건강을 최우선으로 바랬으며 초·중·고등학교 진학에 대해서도 장애형제자매가 특수학교로 진학하기를 간절히 바란다. 부족한 의사소통능력과 이해력으로 일반학교에서 아이들에게 놀림당하며 지내는 것은 같은 형제자매로서 보기 힘들기 때문이다. 또한 자신의 미래에 장애형제자매를 깊숙이 개입시킨다.

장애형제자매로 인해 부모가 자신에게 관심을 많이 못 갖고 상대적으로 무관심속에 방치되어 있다고 느끼며 이에 대해 서러움과 섭섭함, 원망감이 있다. 그러면서 나이터울이 작은 다른 정상형제자매가 부모의 관심을 못 받는 것을 안쓰러워하고 장애형제자매로 인해 상처받고 힘들어하는 어머니에 대해 연민을 갖는다. 장애형제자매로 인해 가족이 서로를 지지하며 돌봐주며 하나되기 시작한다. 또래 간에는 장애형제자매의 이야기를 꺼리고 안으로 숨기면서 친구와 거리를 두지만, 어느 정도 친구들과의 이야기를 통해 이해받으며 장애형제에 대한 스트레스로부터 벗어난다.

그러나 이들도 다른 일반청소년들과 똑같이 집에서는 부모들과 세대차이로 말이 잘 안통하고 다른 형제자매와의 관계에서 기인된 스트레스로 힘들어하고 있었다. 또한 학교에서의 생활에 선생님이 큰 영향을 미치며 학업에 특별히 관심없이 학교를 다니지만 그중 자신에게 관심을 갖어 주는 선생님으로 인해 심리적 지지를 받기도 한다.

본 연구를 통해 다운형제자매를 가진 청소년이 그들의 가족 안에서, 사회 안에서 경험하는 것을 조사하였다. 본 연구결과는 그들의 건강한 성장발달을 도모하고 학교나 사회활동에서의 적응력을 높일 수 있는 중재방안을 마련하는 기초 자료로 사용할 수 있을 것이다.

[영문]The main purpose of this study was to describe experience of the adolescents with a sibling with Down syndrome. ''The participants in this study were three girls and two boys, whose siblings were enrolled in one of the community center for Down syndrome children. In depth interview using a semi-structured questionnaire was done to collect data from October 1 to November 20, 2004. The content of interview was tape recorded under the permission of the participants and thematic analysis was done.

The analysis resulted 17 themes under 5 categories. The five categories were; acceptance of initial shock and embarrassement with mother''s pregnancy and the birth of Down syndrome sibling, daily care of a sibling and concerns, becoming a family and conflicts and anger toward parents, keeping a distance from friends, and normal developmental conflict of adolescent.

Many adolescents were initially shocked and embarrassed by the pregnancy of their mothers at relatively late age and the birth of a sibling with Down syndrome. However, they got used to their siblings and involved in their care. They worry about future of their siblings and want them to be healthy. When they plan for their future, they include their sibling and accept the responsibility to take care of them when something happen to their parents. They have high stress from their parents'' high expectations on academic achievement in compensation of Down sibling. They maintain a distance from their friends and talk about their sibling only with the closest friends. They worry about their own children and marriage. With all these conflicts and concerns related to having a sibling with Down syndrome, they go through the developmental crisis of adolescent. As a result, they have deeper understandings on handicapped individuals and hardships of their parents.

This study provides comprehensive description of experience of adolescent with a sibling with Down syndrome. It is suggested to develop a support program to manage their stress for the siblings of Down syndrome children.
