비 혈연간 생체 신장이식 후 이식신장 생존율에 영향을 미치는 위험인자에 관한 연구

본 연구는 말기 신장부전환자의 주요 치료법으로 인식되어진 신장이식에 있어 비혈연간 생체 신장이식 후 이식신장 생존율에 영향을 미치는 위험인자에 관해 연구하여 공여 장기 부족에 대해 비혈연간 신장 이식의 활성화에 이론적인 근거를 제시하고자 하였다. 1979년부터 2002년 6월까지 서울 S병원에서 시행한 2115예의 환자 중 뇌사자를 이용한 신장이식 55예 아자시오프린을 주면역억제제로 사용한 115예를 제외한 1945예 중 비혈연간 생체 신장이식 환자 967예를 대상으로 후향적으로 분석하였다.

최소 추적기간은 12개월이며 평균 추적 기간은 87.8개월이었으며 978예가 혈연간 신장이식이었고 967예가 비혈연간 신장이식을 시행하였다. 공여자의 구성원은 순수 기증자 781명, 교환이식 공여자 86명, 방계혈족 54명, 배우자 43명, 친구 3명이었고 수여자의 성별은 남자 640명, 여자 327명이었고, 공여자의 성별은 남자 707명, 여자 260명이었다. 수여자의 평균연령은 39.7±10.9이었고, 기증자의 평균연령은 33.7±8.6이었다. 5년 이식신 장생존율은 조직적합항원 완전일치군, 절반일치군, 비혈연간 신이식군 각각 93.7%, 87.6%, 83.4%이었으며 10년 이식신장 생존율은 각각 88.2%, 70.8%, 66.9%였으며 다변량 분석에서 통계적으로 의의 있는 차이는 없었다. 5년 환자 생존율은 혈연간 신이식군은 95.8%, 비혈연간 신이식군은 91.8%이며 10년 환자 생존율은 각각 92.1%, 84.0%로 다변량 분석에서 의의있는 차이는 보이지 않았다. 환자 사망의 주요원인은 감염(39.9%), 뇌혈관질환(13.1%), 악성종양(9.6%), 심혈관질환(8.6%)이었다. 다변량 분석 시 이식신장 생존율에 미치는 위험인자는 급성거부반응의 기왕력, 공여자의 연령 55세 이상, 수여자의 55세 이상, 이식전 당뇨병 및 B형 간염의 과거력 여부이었으며 ABO혈액형의 적합 정도와 조직적합항원의 적합성 정도는 통계적으로 유의한 인자는 아니었다. 본 연구를 통하여 비혈연간 생체 신장이식에 영향을 미치는 위험인자를 확인하였으며 비혈연간 생체 신장이식의 성적이 혈연간 신장이식의 성적과 큰 차이를 보이지 않아 향 후 공여 장기 부족에 대한 대처 방안으로 비혈연간 신장이식이 시행됨에 있어 이론적인 근거를 제시하는 자료가 될 수 있을 것이다.

[영문]Shortage of donor organs is one of the major barriers to transplantation worldwide, especially, in countries where cadaveric organ donation is yet limited.

From 1979 to 1984, we used azathioprine and prednisone as the main immunosuppressive therapy in the first 115consecutive kidney transplant patients. Since then, cyclosporine or tacrolimus has been used as a main immunosuppressive agent. From 1979 to June 2002, 2115 kidney transplantations were performed at Yonsei University Medical Center. Among 2115 cases, total 1945 living donor transplantation cases excluding 55 cadaveric donor transplantation cases and 115 cases using AZA as the main immunosuppressive agents were identified as a study cohort.

The minimum and mean follow-up periods were 12 and 87.8 months, respectively. Of these, 978 used LRDs and 967 LURDs. LURDs were composed of volunteer (781), exchange donor (86), distant relative (54), spouse (43), and friend (3). The mean recipient age was 39.7±10.9 years. The numbers of male and female were 707 and 260, respectively.

The actuarial 5- and 10-year graft survival rates were 93.7% vs. 87.6% vs. 83.4% at 5 years, and 88.2% vs. 70.8% vs. 66.9% at10 years in the HLA-identical, haploidentical and unrelated groups of patients, respectively. No graft survival difference was seen on multivariate analysis. The actuarial 5- and 10-year graft survival rates were 95.8% vs. 91.8% at 5 years and 92.1% vs. 84.0% at 10 years in the related and unrelated groups of patients, respectively but this difference was not statistically significant on multivariate analysis. Common causes of mortality were infection(39.9%), cerebrovascular disease (13.1%), malignancy(9.6%), and cardiovascular disease (8.6%). but this difference was not statistically significant.

On multivariate analysis, risk factors affecting the graft survival in the LURD groups were history of acute rejection, recipient age over 55 years old, preoperative history of DM and hepatitis B viral carrier. But, ABO compatibility and degree of HLA matching were not significant risk factors.

Good long-term graft survival was achieved in LURD kidney transplantation compared with that of LRD kidney transplantation. Because of the striking discrepancy between organ donation and the increasing demand for transplantation, LURD renal transplantation should be considered as an alternative way to increase the number of available donors when accompanied by a careful evaluation process.
