지역사회 정신건강 이념에 대한 인식과 그에 따른 역할 인식 정도

본 연구는 간호학을 전공하는 대학원 학생들의 지역사회 정신건강 이념에 대한 인식정도를 알아보고 이 이념에 영향을 주는 요인들을 분석 규명하며, 지역사회 정신건강 이념과 역할인식과의 관계를 분석하기 위해 시도된 것이다.

사회 발전으로 인간생활과 인간관계가 복잡해 짐에 따라 정신건강의 문제가 크게 대두되고 있으나, 종래의 개인환자 중심의 치료형태로는 도저히 늘어나는 정신건강 문제를 해결할 수 없게되어 최근에는 지역사회 정신건강 이념을 바탕으로 한 정신건강 사업이 강조되고 있다.

이에 이미 지역사회 건강사업에 적절히 훈련되어 있는 간호원은 지역사회 정신건강 이념을 중심으로 지역사회 정신건강 사업을 펴나가는데 중심인력이 되어 줄 것이 기대되고 있으며 ,본 연구를 통해 이 가능성을 추정해 보고 앞으로의 지역사회 정신건강 간호사업

의 방향을 모색하기 위한 참고자료를 제시하고자 하였다.

본 연구의 대상은 서울시내에 있는 간호학을 전공하는 대학원 학생 전수 131명 중, 설문지 조사가 가능한 91명으로 선정하였으며, 1980년 1월 11일에서 27일까지 설문지를 배부, 회수하여 자료를 수집하였다.

본 연구의 도구로는 Backer와 Schulberg의 지역사회 정신건강 이념척도와 Backer와 Howard의 역할인식 항목을 사용하였다.

본 연구의 결과는 다음과 같다.

1. 지역사회 정신건강 이념과 역할인식에 대한 인식정도는 이념과 역할인식의 대부분의 항목에서 높은 인식정도를 나타냈으며, 타 연구의 다른 건강 전문인 단체(정신과 전문의, 정신분석학자등) 보다 높은 인식정도를 보여주었다.

2. 지역사회 정신건강 이념과 역할인식에 영향을 미치는 특성간의 관계분석에 있어서는 나이가 젊은 집단, 기독교인, 근무경험이 없는 경우, 정신건강에 대해 흥미를 갖고 있는 경우, 친척이나 친지중에 정신질환자를 경험한 경우에 있어서 지역사회 정신건강 이념과 역할인식정도가 높은 경향으로 나타났는데, 그 중에 종교, 정신건강에 대한 흥미여부에 있어서만 유의한 차이로 높은 인식 정도를 나타냈다. (p<0.05)

3. 교육과정과 지역사회 정신건강 이념 및 역할인식과의 관계분석에 있어서는 일차건강관리에 대한 교육을 받은 경우에 지역사회 정신건강 이념과 역할인식에서 유의한 차이로 높은 인식정도를 나타냈다. (p<0.05) 실습내용에 대해서는 정신간호학 실습중 집단요법, 놀이요법, 가족요법의 실습내용을 경험한 경우에 역할인식에 대해 유의한 차이로 높은 인식 정도를 나타냈다. (p<0.05) 지역사회 간호학 실습중에는 지역사회 집단 및 보건사업의 계획, 가족건강관리, 추후관리의 실습내용을 경험한 경우에 지역사회 정신건강 이념에 대해 유의한 차이로 높은 인식 정도를 나타냈다. (p<0.05)

4. 지역사회 정신건강 이념과 역할인식과의 관계분석에 있어서는 지역사회 정신건강 이념에 대한 인식정도가 높을수록 역할인식도 비례해서 높아진다는 것을 알 수 있었다.(r=0.646, p<0.001)

결론적으로 본 연구를 통하여 간호학을 전공하는 대학원 학생들이 지역사회 정신건강 이념에 대해 높은 인식 정도를 갖고 있고, 지역사회 정신건강을 위한 주도적 역할을 맡으려는 의지를 갖고 있다는 것을 알 수 있었다. 지역사회 중심의 일차건강관리의 교육 내용이 이 이념과 역할인식에 긍정적인 영향을 주는 요인이 되었다는 것을 알 수 있었으며, 또한 이념이 변화함에 따라 역할인식도 비례하여 변화한다는 것을 알 수 있었다.

This study was done to determine the perception level about community mental health ideology, factor influencing this ideology and the relationships between community mental health ideology and role perception.

As society develops and changes take place in human life and relationships, mental health problems increase. But with the usual method of treating only the individual patient, mental health problems can not be solved.

Therefore recently, mental health services based on community mental health ideology have been greatly emphasized.

So nurses who are already well trained in community mental health services are the expected manpower core for community mental health services.

The purpose of this study was to determine the possibility of using these well trained nurses to accomplish this goal and to use this basic data to aid in determining the future direction of community mental health service.

The subjects for this study were 91 students in graduate nurse program in Seoul. Data were collected by means of a questionnaire from November 11th to November 27th 1980.

The questionnaire consisted of Baker & Schulbefg's C.M.H.I. Scale nad Baker & Howard's role perception.

The finding of this study were as follows :

1. Perception level about ideology and role in community mental health services was high for most items. The perception level of the respondents in this study was higher than the perception level of respondents in other studies (psychiatrists, psychoanalysts).

2. Analysis of the relationship of influencing factors to community mental health ideology and role perception ; respondents who were young, christian, without clinical experience, interested in mental health, or who had a relative or an intimate friend with a mental disorder had a high perception level about community mental health ideology and role perception. Among thes e factors, the perception level of respondents who were christian and interested in mental health was significantly high (p<0.05)

3. Analysis of relationship between curriculum and community mental health ideology and curriculum and role perception ; respondents who had primary health card education had a significantly high perception level about community mental health ideology and role perception.(p<0.05). Those respondents whose phychiatric nursing clinical experience included experience in group therapy, play therapy and family therapy had a significantly high role perception level (p<0.05). Respondents whose community health nursing clinical experience included experience in community diagnosis and public health service plan, family health care, follow-up care had a significantly high perception level about community mental health ideology. (p<0.05)

4. Relationship between community mental health ideology and role perception ; the higher the perception level about community mental health ideology, the higher the role perception level about community mental health ideology, the higher the role perception level (r=0.646, -<0.001)/

In conclusion, gradute nursing students who have a high perception level about community mental health ideology, will be able to serve as leader for community mental health. And since graduate students whose nursing curriculum included primary health card had a high level of ideology and role perception. Also the higher the perception level about community mental health ideology, the higher the role perception level.
