척수 손상 환자에서 표면전극을 이용한 기능적 전기 자극이 체표 온도와 심혈관계에 미치는 영향

기능적 전기 자극은 신경에 전기적 자극을 줌으로써 마비된 근육을 수축시켜 완전 하지 마비 환자에서 근육의 위축을 막고 근력을 증가시키는 작용을 할 수있으며, 관절 구축 및 골다공증을 예방하는 등 여러 가지 효과들이 있는 것으로 알려져 있다.

본 연구는 기능적 전기 자극이 척수 손상 환자의 심혈관계 및 체표 온도에 미치는 영향을 알아보고자 한 것이다. 20명의 척수 손상 환자(남자 14명, 여자6명)를 대상으로 6주간의 훈련 기간이 지난후 기능적 전기 자극을 실시하고 컴퓨터 적외선 체열 촬영기를 이용하여 체표 온도를 측정하였으며, 심혈관계에 대한 변수로 심박동수와 수축기 및 확장기 혈압을 측정한 결과 다음과 같은 결과를 얻었다.

1. 30분간의 기능적 전기 자극이 끝난 후 전기 자극이 가해진 일측 하지의 대퇴부와 전각부에서 체표 온도가 유의하게 증가하였고, 자극이 끝난 후에는 자극전의 상태로 환원되었으며, 자극을 가하지 않은 반대측 하지에서는 별다른 온도 변화를 보이지 않았다.

2. 일측 하지에 대한 기능적 전기 자극 전후의 맥박수와 수축기 및 확장기 혈압은 의미있는 변화를 보이지 않았다.

이상의 결과로 기능적 전기 자극은 체표 온도를 중가시키는 효과가 있다는 것을 알 수 있었다. 심혈관계에 대한 영향에 대하여는 보다 구체적이고 적극적인 연구가 필요할 것으로 사료된다.

Functional electrical stimulation(FES) has been known to increase muscle bulk and strength and prevent muscle atrophy in complete paraplegic patients. But there are some controversies about the effects of FES on skin temperature and cardiovascular system. The purpose of this study is to assess the effects of FES on skin temperature and cardiovascular system in spinal cord injury patients. In 20 thoracic spinal cord injury patients(male 14, female 6), functional electrical stimulation was done fur 30 minutes, and then skin temperature was checked using digital infrared thermal imaging system, and pulse rate, systolic and diastolic blood pressure were checked to evaluate cardiovascular system. Skin temperature of stimulating site was increased significantly after FES(p<0.05) and then decreased to pre-stimulating temperature.

But heart rate, systolic and diastolic blood pressures were not changed significantly after FES on unilateral leg.
