지라의 국소해부 및 조직 구성성분에 대한 형태계측학적 연구

지라에 대한 국소해부학적 지식은 지라 처치시 중요한 자료가 되며 지라에 대한 형태계측학적자료는 병리적 판단을 할 때 비교 자료가 될 수 있다. 지라보존술의 일환으로 지라부분절제를 할 경우, 혈관에 기준한 구역에 의해 시행을 하는데 이때 각 부위에서 구성성분의 분포 비율은 기초자료로서 중요하다. 한국인 지라 70개(남 44, 여 14, 미상 12, 나이 평균 38살)를 재료로 해부하여 지라의 모양과 혈관에 대한 형태학적 특징을 조사하였다. 이중 사망 후 2일 이내의 지라 13개(남 7, 여 6)는 따로 지라내 세 가지 구성성분(백색속질, 잔기둥, 적색속질)을 분석하기 위하여 다섯 부위(위, 문, 중간, 바깥, 아래)에서 조직을 떼어 파라핀 표본제작을 하였고 따로 2개(남 1,여 1)는 온 지라 셀로이딘포매 표본제작을 하여 H-E 염색을 하였다. 제작된 조직표본에서 체계적 표본 추출 방법을 적용하여 저배율 현미경 시야에서 접안격자를 이용, 각 구성성분에 대한 비율을 구하는 형태계측학적 연구를 통해 다음과 같은 결과를 얻었다.

1. 지라의 길이, 너비, 두께 평균은 각각 9.9cm, 6.8cm, 3.7cm였으며 개인간 차가 심하였으며 남자가 여자보다 컸다. 무게와 부피는 평균이 각각 105gm, 98ml였으며 역시 개인차가 심하였고 남자가 여자보다 컸다.

2. 모양은 세모형과 둥근형이 대부분이었으며 뒤 모서리가 앞 모서리보다 더 날카로운 특징을 보였다. 지라의 틈새는 위, 아래간에 차이는 없으나 뒤 모서리에서 앞 모서리보다 많이 발생하였다.

3. 지라의 복강내 위치에서 위 모서리는 아홉째갈비뼈, 또는 아홉째갈비사이공간에 이르는 경우가 21/44(47.7%)로 가장 빈도가 높았으며 아래모서리는 열한째갈비뼈, 또는 열한째갈비사이공간에 위치하는 경우가 20/44(45.5%)로 가장 빈도가 높았다.

4. 지라동맥의 굵기는 평균 0.34cm였으며 지라동맥이 나뉘는 위치는 지라문에서 직선 거리를 평균 2.35cm였으며 지라문으로부터 4.5cm 이내에서 모두 나뉘어졌다.

5. 이자꼬리가 지라문으로 들어와 있는 경우는 60%였다. 덧지라는 8%에서 관찰되었다.

6. 지라동맥의 가지수는 평균 4.8개였다.

7. 문부위에서 지라동맥 가지들은 위아래 선을 기준으로 가운데 위치하며 앞뒤선을 기준으로는 약간 앞으로 치우쳐 있다. 가지들은 위아래로 길쭉하고 앞뒤로 좁게 배열되었다.

8. 문부위에서 지라동맥의 나뉘는 형태는 굵은 가지형이 대부분 이였다.

9. 구성성분의 부피 비율은 백색속질, 잔기둥, 적색속질이 각각 10.9%, 5.9%, 83.2%였으며 지라내에서 부위에 따른 구성비율의 차이는 없었으며 지라 부피에 따른 구성성분의 차이도 없었다. 셀로이딘포매 온지라 조직표본의 경우 백색속질, 잔기둥, 적색속질이 각각 13.4%, 6.5%, 80.1%로 파라핀포매 조직표본의 계측 값과는 차이가 있었다.

이상의 결과로부터 한국인 지라의 크기나 무게는 기존 서양 교과서에 기술된 수치들과 차이가 있음을 알 수 있었으며 지라에 분포하는 혈관도 서양 사람들의 그것과는 다른 점이 있음을 확인하였다. 그리고 정상 지라의 구성성분의 비율을 구함으로서 이를 기본 자료로 하여 병적 변화가 온 지라의 생검 예에서 비교할 수 있는 자료로 삼을 수 있을 것을 기대한다.

This study was performed to identify the topographic variations of the spleen and its supplying artery and to find out the distribution of the splenic components. Spleens from 70 Korean adults(male 44, female 14, unknown 12, mean age 38) were dissected and observed under a surgical microscope. The results were as follows:

1. The length, breadth and thickness of the spleen were 9.9cm, 6.8cm, 3.7cm respectively. The male spleen were larger than female's(p<0.05). And the individual difference was noted.

2. The weight and volume of the spleen were 105gm and 98ml, respectively. The male spleen were larger than female's(p<0.05). And the individual difference was noted.

3. The triangular and the round shape were most frequent(above 75%).

4. In the number of notches, there was a significant difference between the anterior and posterior border.

5. The upper border of the spleens were located on the 9th rib and 9th intercostal space in 47.7%, the lower border of the spleens were located on the 11th rib and 11th intercostal space in 45.5%.

6. The mean thickness of the splenic artery was 0.34cm. And the mean bifurcation distance from the parenchyma was 2.35can.

7. The tale of pancreas were invaginated into hilum in 60%. And the accessory spleen were observed in 8%.

8. The mean numbers of the splenic arterial branches were 4.8.

9. The shapes of the hilum were mostly slit-like appearance in supero-inferior direction.

10. The most frequent type of the splenic artery was magistral type(65%).

11. The percentages of white pulp, trabeculae and red pulp of the paraffin embedding preparations were 10.9%, 5.9%, 83.2% respectively. There was no significant difference between the five region(upper, hilum, intermediate, outer, lower). But those of the celloidine embedding preparations of whole splenic slices(upper, middle, lower) were 13.4%, 6.5%, 80.1% respectively.
