성인 급성골수성백혈병에서 과립구 및 과립-단핵구 집락촉진인자가 감염증에 미치는 영향

dc.description.abstract[한글] 성인 급성골수성백혈병은 최근 항암화학요법과 골수이식의 발전으로 높은 관해유도율 및 장기간의 무병생존율을 보여주고 있다. 그러나 이러한 치료성적을 위해서는 강력한 항암요법이 요구되며, 이에 따른 장기간의 골수 억압 및 감염증은 백혈병 치료의 중요한 장애가 되고 있다. 최근 들어 이러한 과립구 감소기간 및 감염증 병발율을 감소시키고자 하는 목적으로 집락촉진인자, 특히 과립구 및 과립-단핵구 집락촉진인자(rhG/GM-CSF; recom binant human granulocyte/granulocyte-macrophage colony-stimulating factor)를 항암화학요법과 병용하여 사용하고 있다. 지금까지의 연구결과는 항암화학요법과 병용하여 rhG/GM-CSF를 투여하였을 때 감소된 과립구수가 정상으로 회복되는 기간을 유의하게 단축시킨다는 데는 일치하였으나, 감염증 병발율, 발열기간 및 감염중의 치료반응율에는 보고자마다 상이한 결과를 나타내고 있다. 이에 본 연구자는 급성골수성백혈병환자를 대상으로 하여 rhG/GM-CSF 사용이 감염증에 미치는 영향에 대하여 알아보고자 하였다. 대상환자는 1990년 1월부터 1995년 6월까지 연세 의대 세브란스병원 내과에서 성인 급성골수성백혈병으로 진단받은 121명의 262회 항암치료를 대상으로 하였다. 262회의 항암치료 중 85회 항암치료에서 rhG/GM-CSF를 사용하였으며, 나머지 177회에서는 rhG/GM-CSF를 사용하지 않았다. 투여군에서 G-CSF를 사용한 경우는 37회, GM-CSF를 사용한 경우는 48회였으며, 감염증 병발 전에 예방적으로 rhG/GM-CSF를 사용한 경우는 49회(57.6%), 확인된 감염 후 감염증의 치료 효과를 높이기 위하여 사용된 경우는 36회(42.4%)였다. 1. rhG/GM-CSF투여군에서 항암화학요법시작 후 과립구수가 500, 1,000, 1,500/㎕ 이상으로 회복되는 기간은 통계적으로 유의하게 대조군보다 짧았다(p-value < 0.01). 2. 감염증 병발 전에 예방적으로 rhG/GM-CSF를 사용한 49회 중 22회에서 감염증이 병발되었고(44.9%), 이는 대조군보다(67.8%) 유의하게 낮았다(p-value = 0.03). 감염증 유발여부를 예측할 수 있는 회귀분석모형 에서 가장 강력한 예측인자는 rhG/GM-CSF 투여여부 였으며 (p-value = 0.03), 그 다음으로 중요한 인자는 투여된 항암화학요법의 종류였다(p-value = 0.05). 3. rhG/GM-CSF 투여군과 대조군간의 감염증 병발시 발열기간 및 감염증 종류의 차이는 관찰되지 않았으며, 미생물학적 확인감염에서 동정균주의 차이도 관찰되지 않았다. 4, 확인된 감염증에서 항생제와 병용하여 rhG/GM-CSF를 투여한 군의 감염증의 치료반응율은 83.6%로, rhG/GM-CSF를 투여하지 않고 항생제만 투여된군(82.4%)과 유의한 차이는 없었다. 5. 항암화학요법시작 후 말초혈액 저세포 유도시기(1주 이내, 1주 이후)에 따른 감염증 병발율 및 감염증 치료반응율의 유의한 차이는 없었다. 이상의 결과로 급성골수성백혈병환자의 항암화학요법에서 rhG/GM-CSF를 투여하였을 때 의미있는 과립구 감소기간의 단축과 감염증 병발율의 감소를 관찰할 수 있었으며, 발열기간 및 감염증의 중증도 그리고 감염증의 치료반응율의 향상을 위해서는 rhG/GM-CSF의 조 기 투여가 바람직할 것으로 사료된다. [영문] Through recent progress in chemotherapy and bone marrow transplantation, patients with acute myeloid leukemia(AML) have shown a long term, disease free survival. It has become clear, however, that intensive therapy is required to obtain these results. Myelosuppression from therapy and infections have been the major obstacles in the treatment of acute leukemia. Recently, in order to shorten the duration of neutropenia and reduce the rate of infection, colony-stimulating factors (CSFs), especially rhG/GM-CSF(recombinant human granulocyte/granulocytemacrophage colony-stimulating factor) have been used in clinical trials. It has been apparent from earlier studies that rhG/GM-CSF accelerate the recovery of neutrophils after chemotherapy, but there were variable results in the rate of infection, febrile duration, and clinical responses to treatment for infection. This study reviewed the 262 chemotherapy courses of 121 cases with AML from January 1990 to lune 1995 to evaluate the effect of rhG/GM-CSF on infection. Of 262 chemotherapy courses, rhG/GM-CSF was administrated in eighty-five chemotherapy courses, and 177 courses were not. In the rhG/GM-CSF group, rhG-CSF was administrated in 37 chemotherapy courses, rhGM-CSF in 48 courses. And rhG/GM-CSF was administrated for prophylaxis of infection in 49 courses. In the remaining 36 courses, it was used for enhancing the effect of treatment on documented infection. 1. Recovery of neutrophils over 500/㎕, 1,000/㎕ and 1,500㎕, respectively, was significantly shorter in the rhG/GM-CSF group than in the control group(p-value <0.01). 2. The incidence of documented infection was significantly lower in the rhG/GM-CSF than in the control group(p-value = 0.03), and the recovery from neutropenia induced by administration of rhG/GM-CSF is the most important predictor in rate of infection(p-value = 0.03). The second important factor is the type of chemotherapy(p-value = 0.05). 3. The duration of febrile days and type of infection were not different between the two groups. Also, the organisms of microbiologically documented infection were not different. 4. In fifty-five courses with documented infection, rhG/GM-CSF was administrated in combination with antimicrobials. The clinical response to treatment of documented infection in this group was 83.6%, not significantly different from those in the control group(82.4%). 5. There was no statistical difference in incidence of infection and clinical response according to induction timing of hypoplasia after chemotherapy. With the above results, we have concluded that rhG/GM-CSF administration after chemotherapy in AML lowers the incidence of infection by accelerating the recovery of neutrophil counts. There is a need to search for earlier administration of rhG/GM-CSF to improve clinical response to treatment of documented infection.-
